Surgical management of invasive fungal infections in adult leukemia patients:experience from a large tertiary center in Southeast-Asia

Objectives:Invasive fungal infections(IFIs)are a major cause of morbidity and mortality in acute leukemia patients undergoing chemotherapy or hematopoietic stem cell transplantation(HSCT).Surgical interventions may be necessary to improve the survival outcomes of these *** aim of this study is to report a single-center experience using surgical intervention as adjunctive treatment for IFI in adult leukemia ***:A retrospective review was conducted to obtain clinical characteristics and outcomes of surgically managed IFI patients diagnosed between January 2005 and December 2015 in our ***:Nineteen acute leukemia patients,median age 46 years(range 19–65),underwent 20 surgical procedures as management for *** patients had proven IFI diagnoses prior to *** patients underwent surgery for both diagnostic and therapeutic ***-surgery,the diagnostic yield for proven IFI increased by a factor of 5,and 15 patients had definitive IFI *** complications included 2 pleural effusions,4 pneumothoraxes,and 1 *** median duration of hospitalization for patients with complications was 9 days(range 3–64).Thirteen patients benefited overall from the procedure,3 had temporary clinical benefits,and 2 had progression of *** surgery,the 3-month and 2-year overall survival rates were 89.5%and 57.9%,*** median time from surgery to resumption of chemotherapy or HSCT was 25 ***:Surgical interventions for IFI are feasible in selected leukemia patients,as they yield valuable information to guide antifungal therapy or enable therapeutic outcomes with acceptable risk,thereby allowing patients to proceed with curative chemotherapy and HSCT.

Convergence of Hybrid Steepest-Descent Methods for Generalized Variational Inequalities

In this paper, we consider the generalized variational inequality GVI（F, g, C）, where F and g are mappings from a Hilbert space into itself and C is the fixed point set of a nonexpansive mapping. We propose two iterative algorithms to find approximate solutions of the GVI（F,g, C）. Strong convergence results are established and applications to constrained generalized pseudo-inverse are included.

Misperceptions among patients with chronic hepatitis B in Singapore

AIM： To identify the misperceptions among CHB patients,as well as to determine the factors associated with ***： A telephone interview was conducted on 292 adult CHB patients, who earlier responded to an advertisement for free screening. The questionnaire included items about socio-demographic factors and a24-item quiz on knowledge of general aspects,transmission, and management of HBV ***： The mean knowledge score on HBV was 10.4/14. Common misperceptions included availability of treatment for HBV infection and early liver cancer, as well as on transmission. Having completed tertiary education was the only independent factor associated with a high knowledge score, after controlling other demographic ***： More educational efforts should be focused on patients＇ misperceptions and target the less educated HBV carriers.

Protein surface recognition of the novel tetra-carboxylphenyl calix[4]arene to cytochrome c

The interaction of the novel tetra-carboxylphenyl calix[4]arene （TCPC） with the bovine heart cytochrome c （Cc） was first investigated by fluorescence spectroscopy and molecular modeling methods. The formation of a stable 1：1 complex was monitored by fluorescence titration, and its binding constant is 1.916 ×10^7 L mol^-1. Molecular modeling reveals the recognition mechanism of TCPC to the Cc surface, that is, the electrostatic interaction drives TCPC to the Cc surface, and the van der Waals interaction orientates TCPC parallel to the cleft of Cc.

Rational catalyst design and mechanistic evaluation for electrochemical nitrogen reduction at ambient conditions

Ammonia(NH3), a carbon-free hydrogen carrier, is an important commodity for the food supply chain owing to its high energy capacity and ease of storage and transport. The Haber-Bosch process is currently the favored industrial method for large-scale ammonia production but requires energy-intensive and sophisticated infrastructure which hampers its utilization in a sustainable and decentralized system of *** electrochemical nitrogen reduction reaction(eNRR) at ambient conditions holds great potential for sustainable production of ammonia using electricity generated from renewable energy sources such as solar and wind. However, this approach is limited by a low rate of ammonia production with high overpotential and the competing hydrogen evolution reaction(HER). For a better understanding and utilization of eNRR as a sustainable process, insight into rational catalyst design and mechanistic evaluations by a theoretically-directed experimental approach is imperative. Herein, recent insights into rational catalyst design and mechanisms, based on intrinsic and extrinsic catalytic activity are *** the elucidation of basic principles and mechanisms, a framework supplied by theoretical studies that lead to the optimal selection and development of eNRR catalysts is presented. Following a discussion of recently developed electrocatalysts for eNRR, we outline various recently-used theoretical and experimental methodologies to improve the intrinsic and extrinsic catalytic activity of advanced *** review is anticipated to contribute to the development of active, selective, and efficient catalysts for nitrogen reduction.

Using typical endoscopic features to diagnose esophagealsquamous papilloma

AIM: To better understand some of the superficial tiny lesions that are recognized as squamous papilloma of the esophagus(SPE) and receive a different pathological ***: All consecutive patients with esophageal polypoid lesions detected by routine endoscopy at our Endoscopy Centre between October 2009 and June 2014 were retrospectively analysed. We enrolled patients with SPE or other superficial lesions to investigate four key endoscopic appearances(whitish color, exophytic growth, wart-like shape, and surface vessels) and used narrow band imaging(NBI) to distinguish their differences. These series endoscopic images of each patient were retrospectively reviewed by three experienced endoscopists with no prior access to the images. All lesion specimens obtained by forceps biopsy were fixed in formalin and processed for pathological examination. The following data were collected from patient medical records: gender, age, indications for esophagogastroduodenoscopy, and endoscopic characteristics including lesion location, number, color, size, surface morphology, surrounding mucosa, and surface vessels under NBI. Clinicopathological features were also ***: During the study period, 41 esophageal polypoid lesions from 5698 endoscopic examinations were identified retrospectively. These included 24 patients with pathologically confirmed SPE, 11 patients with squamous hyperplasia, three patients with glycogenic acanthosis, two patients with ectopic sebaceous glands, and one patient with a xanthoma. In the χ2 test, exophytic growth(P = 0.003), a wartlike shape(P < 0.001), and crossing surface vessels under NBI(P = 0.001) were more frequently observed in SPE than in other lesion types. By contrast, there was no significant difference regarding the appearance of a whitish color between SPE and other lesion types(P = 0.872). We present a rare case of invasive liver abscess syndrome due to Klebsiella pneumoniae (K. pneumoniae) with metastatic meningitis and septic shock. A previously healthy, 55-year-old female patient developed fever, liver abscess, septic shock, purulent meningitis and metastatic hydrocephalus. Upon admission, the clinical manifestations, laboratory and imaging examinations were compatible with a diagnosis of K. pneumoniae primary liver abscess. Her distal metastasis infection involved meningitis and hydrocephalus, which could flare abruptly and be life threatening. Even with early adequate drainage and antibiotic therapy, the patient’s condition deteriorated and she ultimately died. To the best of our knowledge, this is the first case of K. pneumoniae invasive liver abscess syndrome with septic meningitis reported in China's Mainland. Our findings reflect the need for a better understanding of the epidemiology, risk factors, complications, comorbid medical conditions and treatment of this disease.
