A rare synchrony of adenocarcinoma of the ampulla with an ileal gastrointestinal stromal tumor: A case report

bACKGROUND This is a unique case of a patient who was found to have two extremely rare primary malignancies synchronously,i.e.,an ampullary adenocarcinoma arising from a high-grade dysplastic tubulovillous adenoma of the ampulla of Vater(TVAoA)with a high-grade ileal gastrointestinal stromal tumor(GIST).based on a literature review and to the best of our knowledge,this is the first report of this *** ampullary tumors are extremely rare,with an incidence of four cases per million population,which is approximately 0.0004%.Distal duodenal polyps are uncommon and have a preponderance of occurring around the ampulla of *** adenoma of the ampulla(AoA)may occur sporadically or with a familial inheritance pattern,as in hereditary genetic polyposis syndrome such as familial adenomatous polyposis syndrome(FAPS).We report a case of a 77-year-old male who was admitted for painless obstructive jaundice with a 40-pound weight loss over a two-month period and who was subsequently diagnosed with two extremely rare primary malignancies,i.e.,an adenocarcinoma of the ampulla arising from a high-grade TVAoA and a high-grade ileal GIST found *** SUMMARY A 77-year-old male was admitted for generalized weakness with an associated weight loss of 40 pounds in the previous two months and was noted to have painless obstructive *** physical examination was benign except for bilateral scleral and palmar *** results were significant for an obstructive pattern on liver *** lipase and carbohydrate antigen-19-9 levels were *** tomography(CT)of the abdomen and pelvis and magnetic resonance cholangiopancreatography were consistent with a polypoid mass at the level of the common bile duct(CbD)and the ampulla of Vater with CbD *** same lesions were visualized with endoscopic retrograde *** of endoscopic forceps biopsy showed *** of the surgical specimen of the

Cholestatic liver diseases:An era of emerging therapies

Recently the field of cholestasis has expanded enormously reflecting an improved understanding of the molecular mechanisms underlying bile secretion and its perturbation in chronic cholestatic disease. Novel anti-cholestatic therapeutic options have been developed for patients not favorably responding to ursodeoxycholic acid (UDCA), the current standard treatment for cholestatic liver disease. Important novel treatment targets now also include nuclear receptors involved in bile acid (bA) homoeostasis like farnesoid X receptor and G proteincoupled receptors e.g., the G-protein-coupled bA receptor “transmembrane G coupled receptor 5”. Fibroblast growth factor-19 and enterohepatic bA transporters also deserve attention as additional drug targets as does the potential treatment agent norUDCA. In this review, we discuss recent and future promising therapeutic agents and their potential molecular mechanisms in cholestatic liver disorders.

In this article,a numerical solution of the modified Kawahara equation is presented by septic b-spline collocation *** the von-Neumann stability analysis,the present method is shown to be unconditionally stable.L 2 and L∞error norms and conserved quantities are given at selected *** accuracy of the proposed method is checked by test problems including motion of the single solitary wave,interaction of solitary waves and evolution of solitons.

Etiological role of brucellosis in autoimmune hepatitis

To show that brucellosis may trigger autoimmune hepatitis(AIH), in addition to nonspecific liver involvement and toxic hepatitis, due to a class effect of tetracycline family used for treatment. We present a female patient admitted to our hospital due to partially improved fatigue and elevated liver enzymes following doxycycline and streptomycin usage for brucellosis. brucellosis is endemic in our country, Turkey. It may involve any organ in the body. Liver is frequently involved. Doxycycline used for treatment occasionally may lead to hepatotoxicity. AIH is a necroinflammatory disease of the liver. Certain drugs (e.g. Minocycline), toxins, and viruses (hepatitis b, hepatitis C, EbV, etc.) can trigger AIH. Only one case of AIH probably caused by doxycycline and brucellosis was reported. We discuss the relationship between brucellosis, AIH, and hepatotoxicity of doxycycline. brucellosis may trigger AIH.

Septic b-Spline Collocation Method for the Numerical Solution of the Modified Equal Width Wave Equation

Numerical solutions of the modified equal width wave equation are obtained by using collocation method with septic b-spline finite elements with three different linearization techniques. The motion of a single solitary wave, interaction of two solitary waves and birth of solitons are studied using the proposed method. Accuracy of the method is discussed by computing the numerical conserved laws error norms L2 and L∞. The numerical results show that the present method is a remarkably successful numerical technique for solving the MEW equation. A linear stability analysis shows that this numerical scheme, based on a Crank Nicolson approximation in time, is unconditionally stable.

Effect of oophorectomy and exogenous estrogen replacement on liver injury in experimental obstructive jaundice

AIM: To investigate the role of estrogen on liver injury in an experimental obstructive jaundice ***: Three groups of female rats were constituted; group 1 was oophorectomized and given E2 (n = 14), group 2 was oophorectomized and given placebo (n = 14), and group 3 was sham operated (n = 14). Fourteen days following constitution of bile duct ligation, all groups were compared in terms of serum tests, histopathologic parameters, and tissue levels of IFN-γ and ***: The parameters representing both the injury and/or the reactive response and healing were more pronounced in groups 1 and 2 (χ2 = 17.2, χ2 = 10.20; χ2 = 12.4, P bo group (P bserved to be more pronounced in the groups 1 and 3 (χ2 = 6.13, P both the liver damage and healing. In the conditions of bile duct obstruction, estrogen significantly changed the cytokine milieu in the liver.

Predictive Factors of Hyponatremia in Under-Five Severely Malnourished Children with Pneumonia Admitted to a Large Urban Hospital in Dhaka, bangladesh: A Nested Case-Control Design

background: Hyponatremia is the most common electrolyte imbalance encountered in the management of diarrheal children. Common ramifications include cerebral edema and fatal outcomes. However, pediatric data remain lacking, particularly in developmental contexts where resources are limited and associated conditions like malnutrition and pneumonia are common. Aim: This study aimed to evaluate predicting factors associated with hyponatremia in children under five years of age with severe acute malnutrition (SAM) in bangladesh. Methods: Using a nested case-control design, we compared clinical and laboratory characteristics of children with (n = 61) and without hyponatremia (n = 183) taken from a parent population of all children under five with SAM and clinical or radiological pneumonia admitted to Dhaka Hospital of icddr,b between April 2011 and June 2012 (n = 407). Results: Logistic regression analysis adjusting for potential confounders such as lack of breast feeding, duration of vomiting (days), and severe wasting revealed that older age (OR 1.05, 95%CI 1.02 - 1.07, p = 0.001) (5% increase in the relative odds of hyponatremia for each additional month of age), presence of diarrhea (OR 2.43, 95%CI 1.0 - 6.0, p = 0.05), and difficulty breathing (OR 1.52, 95%CI 1.0 - 2.05, p = 0.05) were significantly associated with hyponatremia. Conclusion: Our data suggest that older age, presence of diarrhea, and difficulty breathing in under-five children with SAM is independent predictors of hyponatremia. These findings underscore the importance of detecting simple clinical predictors early in order to facilitate appropriate management and to prevent potential ramifications of hyponatremia in SAM children, especially in resource-poor settings.

Critical Finite Size Scaling Relation of the Order-Parameter Probability Distribution for the Three-Dimensional Ising Model on the Creutz Cellular Automaton

We study the order parameter probability distribution at the critical point for the three-dimensional spin-1/2 and spin-1 Ising models on the simple cubic lattice under periodic boundary conditions. The finite size scaling relation for the order parameter probability distribution is tested and verified numerically by microcanonical Creutz cellular automata simulations. The state critical exponent δ, which characterizes the far tail regime of the scaling order parameter probability distribution, is estimated for three-dimensional Ising models using the cellular automaton simulations at the critical temperature. The results are in good agreement with the Monte Carlo calculations.

Critical Exponents for the Re-entrant Phase Transitions in the Three-Dimensional blume-Emery-Grifilths Model on the Cellular Automaton

The critical behaviour of the three-dimensional blume-Emery-Griffiths （bEG） model ＆ investigated at D/ J = O, -0.25 and -1 in the range of-1 ≤ K/J ≤ 0 for J -- 100. The simulations are carried out on a simple cubic lattice using the heating algorithm improved from the Creutz cellular automaton （CCA ） under periodic boundary conditions. The universality of the model are obtained for re-entrant and double re-entrant phase transitions which occur at certain D/J and K/J parameters, with J and K representing the nearest-neighbour bilinear and biquadratic interactions, and D being the single-ion anisotropy parameter. The values of static critical exponents β, γ and v are estimated within the framework of the finite-size scaling theory. The results are compatible with the universal Ising critical behaviour for all continuous phase transitions in these ranges.
