Objective:Quality assurance is crucial for oncological surgical treatment *** rare diseases,singlequality indicators are not *** aim to develop a comprehensive and reproducible measurement,called the"Textbook Outcome"(TO),to assess the quality of surgical treatment and prognosis of gastric neuroendocrine carcinoma(G-NEC)***:Data from patients with primary diagnosed G-NEC included in 24 high-volume Chinese hospitals from October 2005 to September 2018 were *** included receiving a curative resection,≥15 lymph nodes examined,no severe postoperative complications,hospital stay≤21 d,and no hospital readmission≤30 d after *** variation in TO was analyzed using a case mix-adjusted funnel *** factors of survival and risk factors for non-Textbook Outcome(non-TO)were analyzed using Cox and logistic models,***:TO was achieved in 56.6%of 860 G-NEC *** patients had better overall survival(OS),disease-free survival(DFS),and recurrence-free survival(RFS)than non-TO patients(Phospital *** logistic regression revealed that nonlower tumors,open surgery,and>200 mL blood loss were independent risk factors for non-TO patients(Plity and prognosis for G-NEC *** predicting non-TO are identified,which may help guide strategies to optimize G-NEC outcomes.

背景TSC2/PKD1邻接基因综合征(TSC2/PKD1 contiguous deletion syndrome,PKDTS)是TSC2与PKD1基因缺失导致的疾病,国内鲜有报道。目的总结5例TSC2/PKD1邻接基因综合征患儿的临床表型和遗传学特征。方法回顾性分析2015—2023年于解放军总医院儿科就诊的4例PKDTS患儿以及于山东第一医科大学附属省立医院就诊的1例PKDTS患儿病例资料,采集患儿及其父母的外周血样,分别利用不同的分子生物学方法进行基因检测。结果5例PKDTS患者均以癫痫起病,有相似的临床特征,包括皮肤色素脱失斑、颅内结节、多囊肾、肾功能异常、高血压等,伴有不同程度的发育迟缓,兼具结节性硬化(TSC)和常染色体显性多囊肾疾病(autosomal dominant polycystic kidney disease,ADPKD)的表型。雷帕霉素可以辅助控制TSC相关癫痫发作;严重肾囊肿患者需要穿刺抽液+药物凝固治疗来缓解症状。基因检测结果显示5例患儿分别存在TSC2/PKD1基因不同大小和区域的缺失:病例1,chr16:2131595-2264778区域存在133.18 kb的缺失;病例2,TSC2基因31~42号外显子和PKD1基因30~46号外显子缺失,长度17.64 kb;病例3,chr16:g.2114201-2185990区域存在缺失,长度为71.789 kb;病例4,chr16:2125441-2176668区域缺失拷贝数51.22 kb;病例5,16p13.3(16:2099825-2151077)区域缺失,覆盖51.25 kb。结论本研究发现TSC2/PKD1缺失程度与临床症状的严重程度之间没有明确关联。利用全基因组测序可以早期精准诊断PKDTS并进行干预,雷帕霉素联合抗癫痫发作药物可较好控制TSC相关癫痫。

BACKGROUND The incidence of proximal gastric cancer(GC)is increasing,and methods for the prediction of the long-term survival of proximal GC patients have not been well *** To develop nomograms for the prediction of long-term survival among proximal GC *** Between January 2007 and June 2013,we prospectively collected and retrospectively analyzed the medical records of 746 patients with proximal GC,who were divided into a training set(n=560,75%)and a validation set(n=186,25%).A Cox regression analysis was used to identify the preoperative and postoperative risk factors for overall survival(OS).RESULTS Among the 746 patients examined,the 3-and 5-year OS rates were 66.1%and 58.4%,*** the training set,preoperative T stage(cT),N stage(cN),CA19-9,tumor size,ASA core,and 3-to 6-mo weight loss were incorporated into the preoperative nomogram to predict the *** addition to these variables,lymphatic vascular infiltration(LVI),postoperative tumor size,T stage,N stage,blood transfusions,and complications were incorporated into the postoperative *** calibration curves used to determine the OS probability fit *** the training set,the preoperative nomogram achieved a C-index of 0.751[95%confidence interval(CI):0.732-0.770]in predicting OS and accurately stratified the patients into four prognostic subgroups(5-year OS rates:86.8%,73.0%,43.72%,and 20.9%,P<0.001).The postoperative nomogram had a C-index of 0.758 in predicting OS and accurately stratified the patients into four prognostic subgroups(5-year OS rates:82.6%,74.3%,45.9%,and 18.9%,P<0.001).CONCLUSION The nomograms accurately predicted the pre-and postoperative long-term survival of proximal GC patients.

Background Cardiac implantable electronic devices （CIEDs） greatly improve survival and life quality of patients. However, there are gender differences regarding both the utilization and benefit of these devices. In this prospective CIED registry, we aim to appraise the gender differences in CIED utilization in China. Methods Twenty centers from 14 provinces in China were included in our registry study. All patients who underwent a CIED implantation in these twenty centers between Jan 2015 and Dec 2016 were included. Results A total of 8570 patients were enrolled in the baseline cohort, including 7203 pacemaker, 664 implantable cardiac defibrillators （ICD） implants and 703 cardiac resynchronization therapy device （CRT/D）. Totally, 4117 （48.0%） CIED patients were female, and more than 59% pacemaker patients were female, but women account only one third of ICD or CRT/D implantation in this registry. There were significant differences between genders at pacemaker and ICD indications. Female was more likely received a pacemaker due to sick sinus syndrome （SSS）（63.9% vs. 51.0%, P 〈 0.001）. Female patients receiving an ICD were more likely due to cardiac ion channel disease （29.2% vs. 4.2%, P 〈 0.001）. The percentage of utilization of dual-chamber pacemaker in female patients was significantly higher than male （85.3% vs. 81.1%, P 〈 0.001）. But male patients were more likely received a cardiac resynchronization therapy devices with defibrillator than female （56.5% vs. 41.9%, P = 0.001）. In pacemaker patient, male was more likely to have structure heart disease （31.3% vs. 28.0%, P = 0.002）. In ICD patient, male patients were more likely to have ischemic heart disease （48.2% vs. 29.2%, P 〈 0.001）. The mean age of women at the time of CRT/D implantation was older than men （P = 0.014）. There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P<0.001).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

目的探讨基于“四化”路径的公立医院健康科普实践成效。方法以“华福科普”等主题共享体系的科普实践为依据,依托大数据、互联网+、多媒体等技术运用,打造“体系制度化”“主题分类化”“形式多样化”和“内容输出精准化”的“四化”路径的科普实践模式。结果经过样本医院的健康科普实践研究证明:“四化”路径下的健康科普实践中涌现出众多优秀的科普作品;荣获多项省级以上科普荣誉;参与科普竞赛人数增幅25%以上;线下科普讲堂1/3及以上为华山医院高级职称专家;增设多个专病门诊;特色门诊患者满意度95%以上等。结论基于“四化”路径的科普实践方法对彰显公立医院的“公益性”职能、提升专科声誉度等具有显著效果。

Objectives To evaluate feasibility, safety and clinical significance of the modified method for trans-catheter closure of patent ductus arteriosus (PDA) with Amplatzer duct occluders. Methods Sixty children with PDA were divided into 2 groups. Group A consisted of 27 children (12 males, 15 females, aged at 4.6±2.9 years), and group B 33 children (13 males, 20 females, aged at 4.9±3.3 years). Protection of Total Flavonoid Fraction from Nervilia fordii on lipopolysaccharide-induced Acute Lung Injury in Rats

Objective To investigate the effects of total flavonoid fraction(TFF) from Nervilia fordii on lipopolysaccharide(LPS)-induced acute lung injury(Ali) in rats,and to explore their protective *** LPS-induced Ali model was established by LPS(5 mg/kg) injection via left cervical *** samples were collected from the cervical artery of all rats at 5 and 6 h after LPS challenge for arterial blood gas test and cytokines measurements,and pulmonary microvascular permeability(PMP),lung wet/dry weight ratio(W/D),and pathological features were *** Phytochemical study showed that the TFF contained 67.3% of flavonoids expressed in rutin and three flavone *** TFF pretreatment(6.24 and 12.48 mg/kg) attenuated the partial arterial pressure of oxygen decline in blood significantly,and decreased the PMP and lung W/D in Ali *** addition,the TFF(6.24 and 12.48 mg/kg) also ameliorated the LPS-induced lung damages including alveolar edema,neutrophils infiltration,alveolar hemorrhage,and thickening of the alveolar ***,the treatment with the TFF(6.24 and 12.48 mg/kg) also down-regulated the levels of pro-inflammatory cytokines,such as tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and intercellular adhesion molecule-1(ICAM-1),and up-regulated the level of anti-inflammatory cytokine IL-10 in serum of Ali rats *** These results suggest that the TFF could protect LPS-induced Ali in rats,which may be mediated,at least in part,by adjusting the production of inflammatory cytokines including TNF-α,IL-6,ICAM-1,and IL-10.

Background:Shenyankangfu Tablet(SYKFT)is a Chinese patent medicine that has been used widely to decrease proteinuria and the progression of chronic kidney ***:This trial compared the efficacy and safety of SYKFT,for the control of proteinuria in primary glomerulonephritis patients,against the standard drug,losartan ***,setting,participants and intervention:This was a multicenter,double-blind,randomized,controlled clinical *** glomerulonephritis patients,aged 18-70 years,with blood pressure≤140/90 mmHg,estimated glomerular filtration rate(eGFR)>45 mL/min per 1.73 ㎡,and 24-hour proteinuria level of 0.5-3.0 g,were recruited in 41 hospitals across 19 provinces in China and were randomly divided into five groups:SYKFT,losartan potassium 50 mg or 100 mg,SYKFT plus losartan potassium 50 mg or 100 *** outcome measu res:The primary outcome was change in the 24-hour proteinuria level,after 48 weeks of ***:A total of 735 participants were *** percent decline of urine protein quantification in the SYKFT group after 48 weeks was 8.78%±2.56%(P=0.006)more than that in the losartan 50 mg group,which was 0.51%±2.54%(P=1.000)less than that in the losartan 100 mg *** with the losartan potassium 50 mg group,the SYKFT plus losartan potassium 50 mg group had a 13.39%±2.49%(Pline did not change statistically *** average change in TCM syndrome score between the patients who took SYKFT(-3.00[-6.00,-2.00])and who did not take SYKFT(-2.00[-5.00,0])was statistically significant(P=0.003).No obvious adverse reactions were observed in any ***:SYKFT decreased the proteinur

Background and Aims:Ravidasvir(RDV)is a new generation pangenotypic hepatitis C virus(HCV)NS5A inhibitor,with high barrier to baseline resistance-associated *** is the first phase 2/3 study conducted in China's Mainland confirming the efficacy and safety of RDV+ritonavir-boosted danoprevir+ribavirin for 12 weeks in treatment-naive noncirrhotic patients with genotype 1 infection in a large ***:In this multicenter,randomized,doubleblinded,placebo-controlled phase 2/3 trial(NCT03362814),we enrolled 424 treatment-nafve,noncirrhotic adult HCV genotype 1 *** patients were randomized at 3∶1 ratio to receive a combination of RDV 200mg once daily plus ritonavir-boosted danoprevir 100mg/100mg twice daily and oral ribavirin 1000/1200mg/day(body weightline NS5A resistance-associated species in the treatment group(76/76,per protocol set)achieved sustained virological response at 12 weeks after *** treatment-related serious adverse events were *** abnormalities showed mild or moderate severity(grade 1 and grade 2)in liver function ***:In treatment-na(i)ve,noncirrhotic HCV Chinese patients infected with HCV genotype 1,all-oral regimen of RDV+ritonavir-boosted danoprevir+ribavirin for 12 weeks was highly efficacious,safe,and well tolerated.
