Risk factors and mortality rates of carbapenem-resistant Gram-negative bacterial infections in intensive care units

Background The prevalence of hospital-acquired infections caused by carbapenem-resistant gram-negative bacteria(CRGNB)is increasing *** risk factors have been associated with such *** present study aimed to identify risk factors and determine the mortality rates associated with CRGNB infections in intensive care *** This retrospective case-control study was conducted at Erciyes University Hospital(Kayseri,Turkey)between January 2017 and December *** and laboratory data were obtained from the Infection Control Committee data and record *** who had CRGNB infection 48–72 h after hospitalization were assigned to the case group,while those who were not infected with CRGNB during hospitalization formed the control *** factors,comorbidity,demographic data,and mortality rates were compared between the two *** Approximately 1449 patients(8.97%)were monitored during the active follow-up period;of those,1171 patients were included in this *** infection developed in 14 patients(70.00%)who had CRGNB colonization at admission;in 162(78.26%)were colonized during hospitalization,whereas 515(54.56%)were not *** was no significant difference in age,sex(male/female)or *** total length of hospital stay was statistically significantly longer(P=0.001)in the case group(median:24[interquartile range:3–378]days)than the control group(median:16[interquartile range:3–135]days).The rates of colonization at admission(25.5%;vs.10.6%,P=0.001)and mortality(64.4%vs.45.8%,P=0.001)were also significantly higher in the cases than in the control group,*** the univariate analysis,prolonged hospitalization,the time from intensive care unit admission to the development of infection,presence of CRGNB colonization at admission,transfer from other hospitals,previous antibiotic use,enteral nutrition,transfusion,hemodialysis,mechanical ventilation,tracheostomy,reintubation,central veno

A Collocation Technique via Pell-Lucas Polynomials to Solve Fractional Differential EquationModel for HIV/AIDS with Treatment Compartment

In this study,a numerical method based on the Pell-Lucas polynomials(PLPs)is developed to solve the fractional order HIV/AIDS epidemic model with a treatment *** HIV/AIDS mathematical model with a treatment compartment is divided into five classes,namely,susceptible patients(S),HIV-positive individuals(I),individuals with full-blown AIDS but not receiving ARV treatment(A),individuals being treated(T),and individuals who have changed their sexual habits sufficiently(R).According to the method,by utilizing the PLPs and the collocation points,we convert the fractional order HIV/AIDS epidemic model with a treatment compartment into a nonlinear system of the algebraic ***,the error analysis is presented for the Pell-Lucas approximation *** aim of this study is to observe the behavior of five populations after 200 days when drug treatment is applied to HIV-infectious and full-blown AIDS *** demonstrate the usefulness of this method,the applications are made on the numerical example with the help of *** addition,four cases of the fractional order derivative(p=1,p=0.95,p=0.9,p=0.85)are examined in the range[0,200].Owing to applications,we figured out that the outcomes have quite decent ***,we understand that the errors decrease when the value of N *** figures in this study are created in *** outcomes indicate that the presented method is reasonably sufficient and correct.

Hand bone mass in rheumatoid arthritis:A review of the literature

Rheumatoid arthritis(RA) is a common chronic inflammatory disease and periarticular osteoporosis or osteopenia of the inflamed hand joints is an early feature of RA Quantitative measurement of hand bone loss may be an outcome measure for the detection of joint destruction and disease progression in early RA. This systematic review examines the published literature reporting hand bone mass in patients with RA, particularly those using the dual X-ray absorptiometry(DXA) methods The majority of the studies reported that hand bone loss is associated with disease activity, functional statusand radiological progression in early RA. Quantitative measurement of hand bone mineral density by DXA may be a useful and practical outcome measure in RA and may be predictive for radiographic progression or functional status in patients with early RA.

Trial productions of freeze-dried Lactobacillus plantarum culture using dairy by-products as cryoprotectants:Viability and characterization of cultures

This study is a research on the utilization of dairy industry by-products in the practical production and long-term preservation of industrial microbial ***,lyophilized cultures were obtained by freeze-drying of Lactobacillus plantarum bacteria in large quantities and for a constant period of time in skimmed milk-based medium containing 5%acid casein,rennet casein,whey and demineralized *** cryoprotectants did not have significant protection against freeze-drying process,moreover,the use of rennet casein significantly reduced *** efficiency of the preservatives in preserving the viability during storage was very successful compared to the control ***-enriched cultures had high water activity and moisture,but high viability was maintained during *** powder morphologies of the cultures were differentiated by the use of different *** sizes of the cultures were parallel to the moisture *** size and distribution increased with *** thermogravimetric analysis(TGA)and differential scanning calorimetry(DSC)analysis,it was determined that the addition of casein protein increased the degradation temperature of the *** production of freeze-dried cultures became difficult with the use of ***,it was generally concluded that the protective properties of skimmed milk used for the preservation of *** could be improved by the addition of dairy by-products as a protective agent.

Diabetic foot ulcers: A devastating complication of diabetes mellitus continues non-stop in spite of new medical treatment modalities

Diabetic foot ulcer is a devastating complication of diabetes mellitus and significant cause of mortality and morbidity all over the world and can be complex and *** development of foot ulcer in a diabetic patient has been estimated to be 19%-34%through their *** pathophysiology of diabetic foot ulcer consist of neuropathy,trauma and,in many patients,additional peripheral arterial *** particular,diabetic neuropathy leads to foot deformity,callus formation,and insensitivity to trauma or *** standard algorithms in diabetic foot ulcer management include assessing the ulcer grade classification,surgical debridement,dressing to facilitate wound healing,offloading,vascular assessment(status and presence of a chance for interventional vascular correction),and infection and glycemic *** especially surgical procedures are sometimes inevitable,they are poor predictive factors for the prognosis of diabetic foot *** novel treatment modalities such as nonsurgical debridement agents,oxygen therapies,and negative pressure wound therapy,topical drugs,cellular bioproducts,human growth factors,energy-based therapies,and systematic therapies have been available for patients with diabetic foot ***,it is uncertain whether they are effective in terms of promoting wound healing related with a limited number of randomized controlled *** review aims at evaluating diabetic foot ulcer with regard to all *** will also focus on conventional and novel adjunctive therapy in diabetic foot management.

A structural equation model to analyze the use of a new multi media platform for increasing awareness of cultural heritage

Dedicated multi media platforms have the potential to enhance experiences and increase people’s awareness of cultural *** is,however,not clear to what extent and by which functions these platforms accomplish ***,the aim of this research is to investigate the influence of functionalities offered by a well-designed multi-media web-platform on the awareness and experience of cultural *** multi media platform used is focused on Strijp-S,an industrial cultural heritage in Eindhoven,the *** platform was created as a prototype based on a study of users’preferences for contents and media types *** collect data,a sample of 302 respondents used the platform to explore and view *** test assumed relationships between different functionalities,awareness and experience in an integrated fashion,the data are analyzed using Structural Equation Modelling(SEM).The results show that information-collection funtionality plays a significant role in improving awareness of cultural heritage,whereas auxiliary functions are particularly helpful for enhancing *** perceived helpfulness of these two sets of functions differ however between users depending on theitr level of prior *** and urban planners can use these results to create their own platform based on local heritage to increase the citizens’awareness of cultural heritage.

Effectiveness of one-to-one feedback methods of teaching with proper metered-dose inhaler technique and correcting errors in chronic obstructive lung disease patients

Objective:To determine the most common mistakes made during the use of a metered-dose inhaler(MDI),and the effect of the repeated trainings performed with demonstration method by providing one-to-one feedback on these ***:This is a quasi-experimental study with a control group.A total of 100 chronic obstructive pulmonary disease(COPD)patients(50 in the control group and 50 in the experimental group)were included in the research.“Patient Information Form(PIF)”(to determine the descriptive characteristics of patients)through the face-to-face interview method,using“MDI Skill Assessment Form”(MDISAF)(it is composed of 10 skill steps about the use of MDI)through observation method was *** in both the groups were asked to use MDI and their abilities regarding use of MDI were *** in the intervention group,usage of MDI was explained by a nurse via demonstration and placebo *** were repeated on days 1,3,and 5 as from hospitalization of the *** the intervention group,three methods were used in this study:“face-to-face training,”“one-to-one,”and“with feedbacks and repeated.”Routine training regarding use of MDI was given by the nurses in the clinic to patients in the control *** use of an MDI was assessed using MDISAF before training and after the training on the first,third and fifth days of *** the seventh day,the last measurement was ***,chi square,and mean were used to assess the ***:After repetitive training with one-to-one feedback,several differences between the groups in favor of the experimental group were found in 7 of the 10 skill levels of the *** was a significant difference after“training”between the groups in the third,fourth,fifth,sixth,seventh,eighth and ninth MDI steps in posttest measurement(P<0.05).Evaluating the skills of the groups to use MDI from pretest to posttest,it was determined that while the intervention group made less mistakes in steps in which

Degradation of refractory organics in concentrated leachate by the Fenton process: Central composite design for process optimization

The primary aim of this study is inert COD removal from leachate nanofiltration concentrate because of its high concentration of resistant organic *** this framework,this study focuses on the treatability of leachate nanofiltration concentrate through Fenton oxidation and optimization of process parameters to reach the maximum pollutant removal by using response surface methodology(RSM).Initial pH,Fe2+concentration,H_(2)O_(2)/Fe^(2+)molar ratio and oxidation time are selected as the independent variables,whereas total COD,color,inert COD and UV254 removal are selected as the *** to the ANOVA results,the R^(2) values of all responses are found to be over 95%.Under the optimum conditions determined by the model(pH:3.99,Fe^(2+):150 mmol/L,H_(2)O_(2)/Fe^(2+):3.27 and oxidation time:84.8 min),the maximum COD removal efficiency is determined as 91.4%by the *** color,inert COD and UV254 removal efficiencies are determined to be 99.9%,97.2%and 99.5%,respectively,by the model,whereas the total COD,color,inert COD and UV254 removal efficiencies are found respectively to be 90%,96.5%,95.3%and 97.2%,experimentally under the optimum operating *** Fenton process improves the biodegradability of the leachate NF concentrate,increasing the BOD5/COD ratio from the value of 0.04 to the value of *** operational cost of the process is calculated to be 0.238€/g COD_(removed).The results indicate that the Fenton oxidation process is an efficient and economical technology in improvement of the biological degradability of leachate nanofiltration concentrate and in removal of resistant organic pollutants.

Background:Shunt systems are used to provide cerebrospinal fluid drainage in the treatment of ***,antibiotic-impregnated shunt systems are used to prevent colonization in the ventriculoperitoneal ***-resistant Staphylococcus aureus(MRSA)is the most common causative microorganism of shunt *** aim of the study is to investigate effects of several substances on MRSA biofilms in the ventriculoperitoneal ***:The present study consists of mainly eight groups(each has two subgroups as antibiotic-impregnated and nonantibiotic-impregnated catheters).In addition,each group contains six molds using MRSA *** this study,daptomycin(DAPT)(2 mg/ml),vancomycin(VAN)(10 mg/ml),linezolid(LIN)(2 mg/ml),N-acetylcysteine(NAC)(6 mg/ml),and various combinations of these substances were used to evaluate the treatment against MRSA using scanning electron microscope(SEM)images and microbiological ***:The colony count in the antibiotic-impregnated samples significantly decreased compared to nonantibiotic-impregnated samples in the MRSA,MRSA+DAPT,and MRSA+LIN groups(p<0.01),***,the colony count in antibiotic-impregnated samples significantly increased compared to nonantibiotic-impregnated samples in NAC+DAPT and NAC+VAN groups(p<0.01),***:The results showed that the use of antibiotic-impregnated catheters has a significant impact on the prevention of infection whereas the combination of NAC and DAPT showed better antibiofilm and antibacterial effects than other combinations on the prevention and treatment of nonantibiotic-impregnated catheter infections.
