Introduction: Healthcare workers in Mogadishu, Somalia face significant occupational injury risks, particularly needle stick injuries, with 61.1% reporting incidents. This poses a serious threat to their health, leading to infections such as hepatitis B, hepatitis C, and HIV. Despite the high prevalence of injuries, awareness of Post-Exposure Prophylaxis (PEP) accessibility is relatively high, with 84.0% of respondents aware of it. However, there are gaps in knowledge and implementation, as evidenced by variations in availability of PEP. Improving workplace safety measures, providing comprehensive training on injury prevention and PEP protocols, and ensuring consistent availability of PEP in healthcare facilities are crucial steps to safeguard the well-being of healthcare workers in Mogadishu, Somalia. Methods: A cross-sectional study was conducted among hospital workers in Mogadishu, Somalia, focusing on professionals from various healthcare facilities. The study targeted nurses, doctors, laboratory personnel, and pharmacists. Purposive sampling was employed, resulting in a sample size of 383 calculated using Fisher’s sample size formula. Data were collected using coded questionnaires entered into Microsoft Excel 2019 and analyzed with SPSS software to generate frequencies and proportions, presented through frequency tables and pie figures. Results: The study in Mogadishu, Somalia, examined the prevalence of occupational injuries and knowledge of Post-Exposure Prophylaxis (PEP) accessibility among healthcare workers. Findings indicate a high prevalence of injuries, with 61.1% reporting incidents, predominantly needle stick injuries (60.6%). Despite the majority seeking prompt medical attention (72.0%), work-related illnesses affected 53.2% of respondents, notably work-related stress (59.5%). Utilization of Amaranth Flour in Preparation of High Nutritional Value Bakery Products

Amaranth has become an unconventional crop that interesting of scientists and researchers due to its unique nutritional properties, in addition to bear the water lack and be suitable for all types of soil. The objective of this study was to utilize amaranth flour in preparation of high nutritional value bakery products, crackers and tortilla produced by using amaranth flour instead of corn flour (w/w) by 0%, 25%, 50%, 75% and 100% levels substitution. The prepared products were evaluated for its chemical composition, minerals content, amino acids composition, fatty acid composition, color analysis, biological active compounds analysis and sensory evaluation. Results showed that substitution with amaranth flour increased protein, fat, ash and fiber. As for minerals and amino acid, it indicated that the formula No. 3 (25% corn flour/75% amaranth flour) in crackers and the formula No. 2 (50% corn flour/50% amaranth flour) in tortilla contained higher amount of minerals such as iron, calcium, potassium, zinc, magnesium, manganese, copper and phosphorus, and of the essential amino acids such as Leucine, Lysine and valine. On the other hand the formula No. 3 in crackers had the highest unsaturated fatty acids content and lowest of total saturated fatty acids content. While the formula No. 2 in tortilla had the highest saturated fatty acids content and lowest of unsaturated fatty acids content. The results also showed that the incorporation of amaranth flour in the preparation of the crackers and tortilla led to significantly decreased yellowness and lightness compared with control. Also, results indicated that crackers and tortilla which prepared with amaranth flour exhibited higher antioxidant activity than control formulas. Concerning sensory evaluation of crackers, results showed significant decrease in appearance, color and overall acceptability while, tortilla had significant decrease in general appearance, color and total score at all formulas. Vitamin E is one of the most important lipid-soluble antioxidants. It is essential for the neurological function but its role in the central nervous system has not fully been elucidated. It is known that tocopherol acts in protecting cell membranes from oxidative damage and it can act as an anti-in?ammatory agent, which may also be neuroprotective, as well as regulating speci?c enzymes. There is growing evidence that oxidative stress plays a key role in the pathophysiology of several neurodegenerative disorders. These diseases are defined by the progressive loss of speci?c neuronal cell populations and are associated with protein aggregates. We reviewed some aspects related to the role of antioxidant properties of Vitamin E in preventing and/or curing neurodegenerative disorders such as the Alzheimer’s disease, Parkinson’s disease, amyotrophic lateral sclerosis, ataxia, tardive dyskinesia and Huntington’s disease.

Risk of Autism Spectrum Disorder According to the Dose and Trimester of Exposure to Antiseizure Medications: A Systematic Review and Meta-Analysis

Background: The association between prenatal exposure to antiseizure medications (ASM) and autism spectrum disorder has been documented. This study sought to examine and synthesize evidence from studies that have evaluated these associations, with particular focus on the trimester of pregnancy and dosage of exposure. Methodology: PubMed, Embase, and PsycINFO databases were searched following strict inclusion/exclusion criteria. 10 studies were recruited involving children born to mothers with epilepsy who took ASM during pregnancy as cases, and those with epilepsy who did not take any ASM in pregnancy. Results: The relative risk of developing ASD among children exposed to valproic acid (RR, 3.90 [95% CI: 2.36 - 6.44], p eloping ASD in children. The relative risk was twice as high in those exposed to valproic acid compared to those exposed to carbamazepine or lamotrigine. Trimester of pregnancy and dosage of ASM used by the mothers were not significant.

A Review of Autism Spectrum Disorder among the Somali Diaspora: Implications for Autism Care in Somalia

Despite the dramatic increase in autism spectrum disorders (ASD) globally, no research has been conducted in Somalia regarding ASD. However, research studies from Somali immigrants (diaspora) living in Sweden, the US, the UK, etc., have been major contributors to the subject of ASD among people of Somali descent. In this review, we aimed to examine ASD among the Somali diaspora community in terms of its prevalence, possible causes, knowledge of the diseases among the diaspora community, and the challenges faced in raising ASD-affected children in a foreign country. These findings create a general picture of the magnitude of the burden of ASD diagnosis and management and the coping mechanisms adopted by the Somali diaspora community, which are vital lessons for policymakers, child health non-governmental organizations, and the professional medical bodies aiming to tackle ASD back home in Somalia. The study found that ASD is three to five times more prevalent among children of Somali descent than their peers from other backgrounds and that Somali children were generally diagnosed much later than their peers and often presented with lower intellectual abilities than their peers. Furthermore, Somali immigrants were found to have low levels of knowledge about autism, faced stigma and discrimination and often resorted to religion and a small tight circle of friends and family for social support. They faced a huge challenge of seeking access to healthcare and schools for their autistic children and have a mistrust of social services for fear of the government taking away their children. These findings raise the possibility that neglected ASD cases may be prevalent in Somalia and thus makes recommendations for future research, social policy development, and early intervention services for individuals with autism in Somalia.

Prevalence of Rifampicin Resistance and Associated Risk Factors among Suspected Multidrug Resistant Tuberculosis Cases in TB Centers Mogadishu-Somalia: Descriptive Study

Introduction and Background: Multidrug-resistant tuberculosis (MDR-TB) has emerged as a significant global health concern. Patients who had failed previous TB treatment, relapsed after treatment, contacted known MDRTB patients or defaulted during previous treatment, and HIV patients are considered to be suspected and at high risk for developing drug resistant TB specially MDR-TB. However, there is little data available on the prevalence and trends of MDR tuberculosis in national level but luck of it in Mogadishu is our main concern. Methodology: This was a crosssectional, descriptive study involving all suspected MDR TB patients attended at the Mogadishu three Tb centers. Results: A total of 138 cases of suspected MDRTB patients were included in the study. Of these, 70 patients (51%) had rifampicin resistant-TB. Of the 138 study participants, 94 (68.62%) were between 21 - 40 years old that indicates the dominance of productive age group (21 - 40 years). Previous Tuberculosis treatment has been noted to be a major risk factor for development of multidrug resistance tuberculosis. MDR-TB prevalence is significantly higher in male than female patients. Conclusion and Interpretation: The prevalence of Rifampicin resistance among these high risk groups was significant. The high association of previous TB treatment to MDR-TB might be explained due to inappropriate anti-tubercular regimens, sub-optimal drugs, inadequate or irregular drug supply, unsatisfactory patient or clinician compliance, lack of supervision of treatment and absence of infection control measures in healthcare facilities. As the prevalence of MDRTB is high and yet the cases remain un-isolated in the community we recommend the MOH/NTP and funding agencies to facilitate establishment of MDRTB management centers earlier in Mogadishu in order to treat the MDRTB case otherwise it might Amplify of the incidence of this Emerging Disease.

Introduction: The treatment of metastatic breast cancer (MBC) is still challenging. Many studies documented the efficacy of pegylated liposomal doxorubicin (PLD) in patients with MBC, but there is a limited data about the cardiac safety with high cumulative dose (HCD) of PLD. Aim of the work: We conducted this trial to outline the cardiac safety of HCD of PLD in patients with MBC who previously received conventional anthracyclines. Methods: During the period of nine years (January 2011 to December 2019). We extracted the data of the patients with MBC receiving PLD at Medical Oncology Department, South Egypt Cancer Institute, Assiut University. These included patients’ demographics and therapeutic data including the full data of PLD, prior conventional anthracyclines, prior trastuzumab, and prior radiotherapy. Background: Chondrocytes in the growth plate(GP) undergo increases in volume during different cascades of cell differentiation during longitudinal bone growth. The volume increase is reported to be the most significant variable in understanding the mechanism of long bone ***: Forty-five postnatal Sprague-Dawley rat pups, 7-15 days old were divided into nine age groups(P7-P15). Five pups were allocated to each group. The rats were sacrificed and tibia and metatarsal bones were harvested. Bone lengths were measured after 0, 24, 48, and 72 hours of ex vivo incubation. Histology of bones was carried out, and GP lengths and chondrocyte densities were ***: There were significant differences in bone length among the age groups after 0 and 72 hours of incubation. Histological sectioning was possible in metatarsal bone from all age groups, and in tibia from 7-to 13-day-old rats. No significant differences in tibia and metatarsal GP lengths were seen among different age groups at 0 and 72 hours of incubation. Significant differences in chondrocyte densities along the epiphyseal GP of the bones between 0 and 72 hours of incubation were observed in most of the age ***: Ex vivo growth of tibia and metatarsal bones of rats aged 7-15 days old is possible, with percentage growth rates of 23.87 ± 0.80% and 40.38 ± 0.95%measured in tibia and metatarsal bone, respectively. Histological sectioning of bones was carried out without the need for decalcification in P7-P13 tibia and P7-P15 metatarsal bone. Increases in chondrocyte density along the GP influence overall bone elongation.

A Convolutional Deep Neural Network Approach for miRNA Clustering

The regulatory role of the Micro-RNAs (miRNAs) in the messenger RNAs (mRNAs) gene expression is well understood by the biologists since some decades, even though the delving into specific aspects is in progress. Clustering is a cornerstone in bioinformatics research, offering a potent computational tool for analyzing diverse types of data encountered in genomics and related fields. MiRNA clustering plays a pivotal role in deciphering the intricate regulatory roles of miRNAs in biological systems. It uncovers novel biomarkers for disease diagnosis and prognosis and advances our understanding of gene regulatory networks and pathways implicated in health and disease, as well as drug discovery. Namely, we have implemented clustering procedure to find interrelations among miRNAs within clusters, and their relations to diseases. Deep clustering (DC) algorithms signify a departure from traditional clustering methods towards more sophisticated techniques, that can uncover intricate patterns and relationships within gene expression data. Deep learning (DL) models have shown remarkable success in various domains, and their application in genomics, especially for tasks like clustering, holding immense promise. The deep convolutional clustering procedure used is different from other traditional methods, demonstrating unbiased clustering results. In the paper, we implement the procedure on a Multiple Myeloma miRNA dataset publicly available on GEO platform, as a template of a cancer instance analysis, and hazard some biological issues.

Cardio-Protective Effects of Oral Nicorandil in Patients Undergoing Cardiac Valve Surgery

Background: Reduction of myocardial reperfusion injury during cardiopulmonary bypass is an essential requirement for increasing the success rate, decreasing morbidity and mortality of open-heart surgery. Aim: To study the role of pre-operative oral nicorandil in decreasing reperfusion cardiac injury in patients subjected to cardiac valve surgery. Patients and Methods: The study included 62 patients, who were equally randomized into two groups: nicorandil group and control group. Pre-operative, intra-operative and post- operative data were reported and analyzed. Left Ventricle Ejection Fraction (LVEF) was estimated pre-operatively and postoperatively for both groups. Troponin I, creatine kinase-muscle/brain (CK-MB), interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) were measured before surgery by 24 hours then 4, 12 and 48 hours after aortic cross clamp removal. Results: Nicorandil considerably decreased TNF-α and IL-6 after 4 and 12 hours following the removal of aortic clamping. It also reduced troponin-I and CKMB at the same time points. However, there were no important changes in IL-6, TNF-α, troponin-I and CK-MB levels in control group in comparison to nicorandil group in the next 48 hours following the removal of aortic clamping. Conclusions: Pre-operative oral nicorandil expressively decreased myocardial reperfusion damage during open heart valve operations, this evidenced by the decrease in the postoperative use of inotropic drugs, considerable reduction of postoperative elevation of cardiac enzymes and inflammatory cytokines with no reported complications.
