Coblation Intracapsular Tonsillectomy and Coblation Complete Tonsillectomy for Obstructive Sleep Apnea

Objective: Total tonsillectomy and intracapsular tonsillectomy are common procedures for the treatment of obstructive sleep apnea (OSA) in children. The objective of this study was to compare the effectiveness of coblation intracapsular tonsillectomy (ICT) and coblation complete tonsillectomy (CT) as treatments for OSA. Study design: A retrospective study of all the children ages 2 - 18 years with OSA who underwent coblation intracapsular tonsillectomy (ICT) or coblation complete tonsillectomy (CT) from January 2007 to August 2010 by the same surgeons at one institution. Methods: Data were retrieved from children’s charts and from telephone interviews with children’s parents, regarding pre and postoperative OSA-18 scores, postoperative pain, postoperative complications, use of analgesic drugs, and time to return to a solid food diet. Results: All 43 children who underwent ICT and 37 children who underwent CT suffered from OSA before surgery, and none did postoperatively. There were no minor complications in the ICT group, compared to 13.5% in the CT group (p = 0.01). According to parental report, 72% and 21% suffered a low level of postoperative pain, and 9% and 33% severe pain in the ICT and CT groups, respectively. For these respective groups, 49% and 73% needed analgesic drugs (p < 0.05);and 65% and 35% ate solid food during the first 3 days post surgery. Conclusions: Both ICT and CT were safe, with few complications;however recovery was faster in the ICT group, as demonstrated by less pain, and more rapid return to a solid food diet.

Laryngeal cyst causing neonatal airway obstruction during labor is a very rare condition [1]. Congenital laryngeal cysts are a rare cause of neonatal airway obstruction. Traditionally, these cysts have been treated surgically by endoscopic excision or marsupialization. However, the cyst often extends beyond the larynx. We describe a case of a newborn that, during delivery, became cyanotic due to airway obstruction and respiratory distress. To the best of our knowledge this is the first report of a saccular cyst obstructing airway during birth prior to intubation. The immediate and late treatments together with a literature review are described.

Although islet transplantation for individuals with type I diabetes has been shown to yield superior blood glucose control, it remains inadequate for long-term control. This is partly due to islet injuries and stresses that can lead to beta cell loss. Inhibition of excess IL-1β activity might minimize islet injuries, thus preserving function. The IL-1 receptor antagonist （IL-1Ra）, an endogenous inhibitor of IL-1β, protects islets from cytokine-induced necrosis and apoptosis. Therefore, an imbalance between IL-1β and IL-1Ra might influence the courses of allogeneic and autoimmune responses to islets. Our group previously demonstrated that the circulating serine-protease inhibitor human alpha-1-antitrypsin （hAAT）, the levels of which increase in circulation during acute-phase immune responses, exhibits anti-inflammatory and islet-protective properties, as well as immunomodulatory activity. In the present study, we sought to determine whether the pancreatic islet allograft-protective activity of hAAT was mediated by IL- 1Ra induction. Our results demonstrated that hAAT led to a 2.04-fold increase in IL-1Ra expression in stimulated macrophages and that hAAT-pre-treated islet grafts exhibited a 4.851-fold increase in IL-1Ra transcript levels, which were associated with a moderate inflammatory profile. Unexpectedly, islets that were isolated from IL-1Ra-knockout mice and pre-treated with hAAT before grafting into wild-type mice yielded an increase in intragraft IL-1Ra expression that was presumably derived from infiltrating host cells, albeit in the absence of hAAT treatment of the host. Indeed, hAAT-pre-treated islets generated hAAT-free conditioned medium that could induce IL-1Ra production in cultured macrophages. Finally, we demonstrated that hAAT promoted a distinct phosphorylation and nuclear translocation pattern for p65, a key transcription factor required for IL- 1Ra expression.

Virtual nonenhanced abdominal dual-energy MDCT:Analysis of image characteristics

AIM: To evaluate abdominal and pelvic image characteristics and artifacts on virtual nonenhanced (VNE) images generated from contrast-enhanced dual-energy multidetector computed tomography (MDCT) studies. METHODS: Hadassah-Hebrew University Medical Institutional Review Board approval was obtained; 22 patients underwent clinically-indicated abdominal and pelvic single-source dual-energy MDCT (Philips Healthcare, Cleveland, OH, USA), pre- and post-IV administration of Omnipaque 300 contrast (100 cc). Various solid and vascular structures were evaluated. VNE images were generated from the portal contrast-enhanced phase using probabilistic separation. Contrast-enhanced-, regular nonenhanced (RNE)-, and VNE images were evaluated with a total of 1494 density measurements. The ratio of iodine contrast deletion was calculated. Visualization of calcifications, urinary tract stones, and image artifacts in VNE images were assessed. RESULTS: VNE images were successfully generated in all patients. Significant portal-phase iodine contrast deletion was seen in the kidney (61.7%), adrenal gland (55.3%), iliac artery (55.0%), aorta (51.6%), and spleen (34.5%). Contrast deletion was also significant in the right atrium (RA) (51.5%) and portal vein (39.3%), but insignificant in the iliac vein and inferior vena cava (IVC). Average post contrast-to-VNE HU differences were significant (P < 0.05) in the: RA -135.3 (SD 121.8), aorta -114.1 (SD 48.5), iliac artery -104.6 (SD 53.7), kidney -30.3 (SD 34.9), spleen -9.2 (SD 8.8), and portal vein -7.7 (SD 13.2). Average VNE-to-RNE HU differences were significant in all organs but the prostate and subcutaneous fat: aorta 38.0 (SD 9.3), RA 37.8 (SD 16.1), portal vein 21.8 (SD 12.0), IVC 12.2 (SD 11.6), muscle 3.3 (SD 4.9), liver 5.7 (SD 6.4), spleen 22.3 (SD 9.8), kidney 40.5 (SD 6.8), and adrenal 20.7 (SD 13.5). Preliminary Study: A New Approach for Improving the Cryopreservation of Mammalian Sperm

Gamete preservation is a necessary and routine procedure practiced in the 21st century in both humans and animals using the cryopreservation technique. However, cryopreservation methods can cause cryoinjury. Therefore, new approaches to help extend the viability of mammalian sperm in vitro are essential. This preliminary study explored the effect of reproductive fluids (RFs) and body fluids (BFs) from two species of desert snails—Sphincterochila zonata and Sphincterochila prophetarum—on mammalian cryopreserved sperm parameters. These desert snails are active only 5% of the year. Spermatogenesis occurs during the aestivation ecophysiological stage when testosterone levels are high, and sperm is preserved in the oviduct until mating during the active ecophysiological stage in winter. RFs from S. zonata and S. prophetarum during the aestivation ecophysiological stage reduced sperm motility to 0%, while sperm viability (SV) was similar to the controls. Moreover, the motility of thawed mouse sperm was 1.34- and 2.02-fold higher (p S. zonata in the active ecophysiological stage than in the control medium after 5- and 30-min incubation. SV was higher in S. zonata RF medium than in control after 30 min incubation. Our results indicate the potential protective effect of desert snail RFs on cryopreserved and thawed mammalian sperm cells. Further study should be conducted to advance the fulfillment of RF potential in reproductive technologies.

Visual guidelines and tutoring in pediatric urological surgery

Background:The aim of this prospective randomized study was to evaluate the impact of visual guidelines(picture book)and parents tutoring on pediatric urological surgery on parent's stress and anxiety,the number of postoperative contacts,and *** and methods:Following institutional ethical committee approval,a special picture book reflecting different stages of the convalescent period following multiple types of pediatric urological surgery was *** were randomly divided into 2 groups in which 33 parents in Group 1 received the picture book in addition to routine instructions prior to the surgery and 31 in Group 2 received only routine postoperative *** parents were asked to answer a questionnaire(Amsterdam Preoperative Anxiety and Information Scale)regarding the level of anxiety before surgery and immediately after surgery in the recovery *** number of postoperative parent's calls,nonplaned emergency room visits,and complications were ***:No statistically significant difference in perioperative parental anxiety was found(p=0.88).The visual tutoring group had a significantly lower rate of emergency room admissions(6.6%vs.18.6%,p=0.0433),however parents from this group made a higher number of postoperative calls(9.9%vs.3.1%,p=0.38).Two(6.6%)from the tutoring group expressed their desire to omit visual counseling in future surgical preparation and 4(13.2%)did not have an *** satisfaction with regards to the preoperative counseling and information and the number of postoperative complications was similar in both ***:Visual tutoring does not add any value to parental anxiety but seems helpful in reducing postoperative emergency room *** parents preferred to exclude visual information from future preoperative counseling.

Radio Frequency-Microchannels for Transdermal Delivery: Characterization of Skin Recovery and Delivery Window

Transdermal delivery through Radio-Frequency-MicroChannels (RF-MCs) was proven to be a promising delivery method for hydrophilic drugs and macromolecules that must be injected. An important issue in assessing this technology is the life span of the microchannels (MCs). The time window in which the MCs remain open affects the delivery rate and determine the effective delivery duration. The present work focused on the characterization of the ViaDor-MCs recovery and closure process by measurements of transepidermal water loss (TEWL) before and after the formation of MCs, evaluation of the delivery window, and assessment of skin histology. Testosterone-cyclodextrin complex was used as the model drug for evaluation of the transdermal delivery. In-vitro permeation system and in-vivo guinea pig animal model were used in the delivery studies. Our findings demonstrate the recovery process of MCs created by the RF ablation technology. The observed gradual skin recovery affected the transdermal delivery rate. A significant transdermal delivery was shown up to 24 hrs post device application suggesting that an extended delivery of water soluble drugs, including macromolecules, is possible. The histology assessments demonstrated repair and healing of the induced MCs indicating that the RF micro-channeling technology is minimally invasive, transient in nature with no resulting skin trauma.
