Clinical and Demographic Profile of Women with Uterine Fibroids Liv.ng in Puerto Rico

Introduction: Uterine leiomyomas, or fibroids, are prev.lent benign tumors affecting up to 70% of reproductiv.-age women in the United States, significantly impacting productiv.ty and quality of life. Despite their commonality, the epidemiology and clinical patterns of leiomyomas in Puerto Rico remain inadequately researched. Methods: This study conducted a secondary data analysis using clinical, menstrual cycle characteristics, and demographic data from self-administered questionnaires of women undergoing diagnostic laparoscopy for endometriosis in Puerto Rico. Chi-square tests and t-tests analyzed categorical and continuous v.riables, respectiv.ly (p Results: Among 1,610 endometriosis cases, 312 included uterine fibroids. Prev.lence estimates were 10.3% (40 years). Primiparous women av.raged 22.14 years, with 40.71% hav.ng 2 - 3 pregnancies and 26.28% experiencing miscarriages. Symptoms included infertility (29.5%), dyspareunia (40.7%), sev.re pain (50%), and dysmenorrhea (75.6%). Comorbidities included ov.rian cysts (52.2%) and abnormal uterine bleeding (37.8%). Conclusions: Adv.nced age, parity, and comorbidities like diabetes and hypertension were prev.lent in our cohort. Understanding the epidemiological profile of women liv.ng in Puerto Rico with uterine fibroids is crucial for improv.ng diagnostic awareness and enhancing patient care.

Although pharmaceuticals hav. beendetected in the env.ronment only in the range from ng/L to μg/L, it has beendemonstrated that they can adv.rsely affect the health status of aquatic organisms. Lysosomal membrane stability （LMS） has prev.ously been applied as an indicator of cellularwell-being todetermine health status in biv.lv. mussels. The objectiv. of this study is to ev.luate LMS in Ruditapes philippinarum haemolymph using the neutral red retention assay （NRRA）. Clamswere exposed in laboratory conditions to caffeine （0.1, 5, 15, 50 μg/L）, ibuprofen （0.1, 5, 10, 50 μg/L）, carbamazepine and nov.biocin （both at 0.1, 1, 10, 50 μg/L） for35days. Results show adose-dependent effect of the pharmaceuticals. The neutral red retention time measured at the end of the bioassaywas significantly reduced by 50% after exposure to env.ronmental concentrations （p 〈 0.05）（caffeine = 15 μg/L; ibuprofen = 10 μg/L; carbamazepine = 1 μg/L and nov.biocin = 1 μg/L）, compared to controls. Clams exposed to these pharmaceuticalswere considered to present adiminished health status （retention time 〈45 min）, significantlyworse than controls （96 min）（p 〈 0.05）. The predicted no env.ronmental effect concentration （PNEC） results showed that these pharmaceuticals are v.ry toxic at the env.ronmental concentrations tested. Measurement of the alteration of LMS has been found to be a sensitiv. technique that enables ev.luation of the health status of clams after exposure to pharmaceuticals under laboratory conditions, thus representing a robust Tier-1 screening biomarker.

Challenges in transplantation for alcoholic liv.r disease

Transplantation for the treatment of alcoholic cirrhosis is more controv.rsially discussed than it is for any other indication. The crucial aspect in this setting is abstinence before and after liv.r transplantation. We established pre-transplant selection criteria for potential transplant candidates. Prov.ded that the underlying disease can be treated, there is no reason to withhold liv.r transplantation in a patient suffering from alcoholic cirrhosis. Ev.luation of the patient by a multidisciplinary team, including an addiction specialist, is considered to be the gold standard. Howev.r, sev.ral centers demand a specified period of abstinence- usually 6 mo- irrespectiv. of the specialist's assessment. The 6-mo rule is v.ewed critically because liv.r transplantation was found to clearly benefit selected patients with acute alcoholic hepatitis; the benefit was similar to that achiev.d for other acute indications. Howev.r, the discussion may well be an academic one because the waiting time for liv.r transplantation exceeds six months at the majority of centers. The actual challenge in liv.r transplantation for alcoholic cirrhosis may well be the need for lifelong post-transplant follow-up rather than the patient's pretransplant ev.luation. A small number of recipients experience a relapse of alcoholism; these patients are at risk for organ damage and graft-related death. Posttransplant surv.illance protocols should demonstratealcohol relapse at an early stage, thus permitting the initiation of adequate treatment. Patients with alcoholic cirrhosis are at high risk of dev.loping head and neck, esophageal, or lung cancer. The higher risk of malignancies should be considered in the routine assessment of patients suffering from alcoholic cirrhosis. Tumor surv.illance protocols for liv.r transplant recipients, currently being dev.loped, should become a part of standard care; these will improv. surv.v.l by permitting diagnosis at an early stage. Yttria-stabilized zirconia(YSZ,ZrO_(2)-8%Y2O_(3))films were deposited onto lanthanum strontium manganite(LSM,La_(0.7)Sr_(0.3)MnO_(3))substrates using dip-coating process aiming for the application in solid oxide fuel cells(SOFCs).YSZ precursor was prepared by sol-gel method;three v.lues of the organic/inorganic concentration ratio(1,3 and 5)were utilized and the sol v.scosity was adjusted(60 mPa·s and 100 mPa·s)before deposition on the *** influence of these synthesis parameters on the structure and morphology of the deposited films was examined by X-ray diffraction(XRD)and scanning electron microscopy(SEM).The films showed characteristic peaks of LSM,YSZ(with cubic structure)and secondary phases of SrZrO_(3) and La_(2)O_(3).Depending on the synthesis conditions,crack-free,homogeneous and well adhered films were obtained,with thickness of 11-24μm.

Analysis of South Hemisphere Temperature Anomalies ov.r the Last Millennium up to the Beginning of the Contemporary Age: A Comparison between a Statistical Model and a Global Dynamic Model

We analyze the consistency of the coupled atmosphere-ocean GISS-ER climate model in reproducing South American temperature anomalies ov.r the last millennium. For that purpose, we compare the model results with the Neukom’s temperature anomaly reconstruction prov.ded by a statistical model that uses multiproxy data and climatological temperature anomalies. Specifically, we examine temperature anomalies for summer and winter of a 6-member ensemble of the GISS-ER model with 1× solar forcing and 2× solar forcing, calculated for the period 1000-1899. Using the Neukom’s reconstructions and the model outputs, we calculate the summer mean anomalies of the period 1001-1700 (a period that includes Mediev.l Climate Anomaly -MCA- and Little Ice Age -LIA-), and winter mean anomalies of the period 1706-1800 (a period that includes the pre-industrial period). These mean anomalies are subtracted from Neukom’s reconstructions defining the reconstructions wrt 1001-1700 (summer) and wrt 1706-1800 (summer and winter). In general, the model is not v.ry consistent with the reconstructions, since the model’s mean and spread show v.ry small interannual v.riability in contrast to what the reconstructions exhibit, and often with anomalies of opposite sign. In the period 1001-1700, the model presents, on av.rage, negativ. temperature anomalies for the 1× and 2× forcing. In the period 1706-1800, the model displays positiv. anomalies for summer and negativ. anomalies for winter for the 1× and 2× forcing. In particular, the anomaly that better reproduces the sign of the reconstructions is that wrt 1001-1700 with 1× forcing and for summer. The model has an error of only 33% in reproducing the sign of the anomalies, in comparison to the Neukom’s reconstruction for the same reference period. The model’s anomaly wrt 1001-1700 with 1× forcing for summer also reproduces the MCA and LIA’s sign of the reconstruction. Droughts in Homogeneous Areas of South America and Associated Processes during the Months of Austral Spring and Summer

Droughts that occurred in selected areas located in homogeneous regions of South America during the austral springs(SON)and summers(DJF)from 1982 to 2019 are identified using the Standard Precipitation Index(SPI).Four areas were analyzed for droughts in SON,and three areas were analyzed for droughts in *** areas in the Amazon suffered from the majority of their droughts during El Niño years,while most of the droughts in the areas of southern Brazil,Uruguay,and North Argentina occurred during La Niña *** southeastern and central-western parts of Brazil,droughts occurred during both phases of El Nino-Southern Oscillation(ENSO)and also during neutral ***,other processes besides ENSO are likely related to the observ.d *** droughts were inv.stigated for each area and month,and composites of atmospheric and oceanic v.riables during both seasons were analyzed for the selected *** and large-scale field composites were examined to identify the main processes associated with dry conditions in the different *** features were related to the influence of high pressure ov.r southern and southeastern areas and the div.rgence of moisture flux in all *** circulations contributed to subsidence ov.r the dry *** large-scale influential features include SST anomalies,wav.trains ov.r the South Pacific Ocean with centers of action ov.r South America that produced subsidence in the study areas,and conv.ction anomalies in the Maritime continent and surrounding ***,the droughts were associated with a combination of regional and large-scale features that produced subsidence ov.r the analyzed regions.

Electrocardiographic measures of repolarization dispersion and their relationships with echocardiographic indices of v.ntricular remodeling and premature v.ntricular beats in hypertension

Objectiv. To examine the relationship between Tpeak- Tend interv.l （Tpe） and Tpe/QT ratio with occurrence of v.ntricular premature beats （v.Bs） and left v.ntricular remodeling in hypertension. Methods A total of 52 patients with mild to moderate essential hypertension were included, undergoing echocardiography and 24-hours Holter monitoring. v.ntricular remodeling was assessed by left v.ntricular mass index （Lv.I） using the Dev.reux formula and diastolic fimction by transmitral E and A wav. v.locities and E/A ratio. Tpe was measured in the precordial leads. The end of the T wav. was set by the method of the tangent to the steepest descending slope of the T wav.. Results Tpe and Tpe/QT in leads v. （r = 0.33, P = 0.01; r = 0.27, P = 0.04 respectiv.ly） and v. （r = 0.40, P = 0.002; r = 0.40, P = 0.003, respectiv.ly） correlated significantly with Lv.I. A significant inv.rse relationship was observ.d between E/A ratio and QT （r = -0.33, P = 0.01）, Tpe in v. （r = -0.39, P = 0.003） and Tpe/QT in v. （r = -0.31, P = 0.02）. Tpe in v., v., mean Tpe and maximum Tpe with cut-offv.lues of 60 ms, 59 ms, 62 ms and 71 ms, respectiv.ly, associated with the occurrence of v.ntricular premature beats. Conclusions The repolarization parameters Tpe interv.l and Tpe/QT ratio correlate with Lv.I and indices of left v.ntricular diastolic function and show better predictiv. v.lues than traditional parameters such as QT interv.l and QT dispersion. Lead v. is the best lead for measuring Tpe and Tpe/QT. These ECG indices can therefore be used in clinical practice to monitor Lv.remodeling and predict occurrence of v.Bs.
