Objective: Analyze the maternal mortality in the two departments of CHU Point “G” in Bamako, because of high maternal mortality rate in our country. Material and Methods: This was an analytical cross-sectional study on maternal deaths from February 19, 2005 to November 19, 2019 for patients admitted in both departments and who died during the pregnancy-puerperal period at CHU Point “G”. All the patients who died outside this pregnancy-puerperal period were not retained. The data were entered and analyzed using sPss 12.0 software. The statistical test used was that of Chi2, the statistical significance threshold was fixed at 5%. Results: During our study, we recorded 389 maternal deaths out of 16,033 admissions in 15 years and 18,060 live births during the same period making a maternal mortality ratio of 2153.931 and a frequency of 2.426. At the end of our study, we noted that the frequency of maternal deaths was higher in 2014: 12.9% (50/389). The maternal death predominantly affected women aged of 20 - 24 with a frequency of 22.4% (87/389). The multiparity (166/389 making 42.7%), illiteracy (341/389 making 87.7%), the poor evacuation conditions (non-medicalized transport): 263/389 making 67.6%;the evacuation without any evacuation sheet: 259/389 making 66.6%), poor CPN (Prenatal consultation) quality (undone CPN: 191/389 making 49.1%) and the poor monitoring of delivery works (no use of partograph in 343/389 making 88.2%) were the factors favoring maternal deaths. The main causes of maternal deaths were direct in 231/389 making 59.4% with hemorrhage in first line: 21.1% (82/389), infection (61/389 cases making 15.68%), dystocia: 50 cases making 12.85% and high blood pressure and complications (38/389 making 9.76%);indirect in 158/389 cases making 40.6% (Figures 1-3). This study investigated bacteria that are associated with milt in cultured samples of African catfish-Clarias *** broodstocks of Clarias gariepinus were collected from private Fish Farm in Ogun state,Nigeria,while the fish’s milt was collected and analyzed for microbial load at laboratory of the Department of Microbiology,Federal University of Agriculture,Abeokuta,Ogun *** molecular characterization of the amplicon was done at International Institute of Tropical Agriculture,***-edit was used for importing and mining nucleotide sequences into gene *** revealed by the results,bacterial organisms that were present in the milt of Clarias gariepinus are Aeromonas caviae,Proteus mirabilis,serratia rubidaea,Pseudomonas mosselii,Acinetobacter soli and Klebsiella *** Basic Local Alignment search Tools revealed the percentage similarity ranging from 86%-97.04%and their accession *** bacteria indicated high levels of faecal contamination in the *** conclusion,bacteria were found in the milt of cultured catfish and are capable of being pathogenic to humans and may increase the vertical transfer to fry during breeding and rearing ***,fish farmers should maintain a hygiene and serene environment during breeding and culturing of catfish.

Digestive hemorrhage by rupture of esophageal Varices is common and has a pejorative prognosis in our context. strong>Purpose:strong> The main purpose of this work was to study the digestive bleeding by esophageal varices and prognosis value of blood transfusion in the Hospital of gabriel Touré. strong>Methodology:strong> It was a prospective study that took place in the service of Hepato-gastroenterology of gabriel Touré Hospital from June 2016 to May 2017 and from November 2017 to August 2018. strong>Results:strong> At the end of the study, 77 patients met the inclusion criteria out of 1396 patients hospitalized during the same period. Varices bleeding represented a prevalence of 5.5% among hospitalized patients during the same period. The average age of our patients was 46.58 ± 15.09 years. The male sex was more reported in our study with a prevalence of 67.5%. At admission, 63.2% had clinical anemia, 58.4% low arterial pressure and 50.6% hemoglobin rate less than 7 g/dL. Blood transfusion was indicated in 47 patients (61%). The mortality rate was 23.4% and was comparable in both groups (p = 0.0990). Early rebleeding was significantly observed in the case of transfusion (p = 0.0452). Hepatic encephalopathy was the leading cause of death of our patients with 72.2%. strong>Conclusion:strong> Digestive bleeding by esophageal varices is a worsen complication in cirrhosis in hospital setting. Transfusion has not significantly improved the prognosis of our patients.

Hypopharyngeal Cancer: Epidemiological, Clinical and Paraclinical Aspects at the University Hospital Center (CHU) gabriel Toure in Bamako

Introduction: Hypopharyngeal cancers are ubiquitous cancers. These are very lymphophilic cancers;cervical lymph node metastases are present in 60% to 80% of patients at the time of diagnosis. Alcohol and tobacco have been implicated in their genesis. Objective: To study the epidemiological, clinical and paraclinical aspects of hypopharyngeal cancer in the ENT department of the CHU gabriel Toure. Patients and Methods: It is a retrospective and prospective quantitative study covering a period of 4 years from June 2014 to July 2018;all patients who presented with hypopharyngeal cancer were included. Results: Thirty-five (35) patients were collected during the study period. The mean age of the patients was 44 ± 14 years with extremes of 16 and 78 years. The sex ratio was 0.47. Dysphagia was the reason for consultation at 100%. The average consultation time was 14 months, with extremes of 3 to 26 months. smoking intoxication was found in 13 patients. The piriform sinus was interested in 52%. More than half of the patients were received at the advanced stage (T3 - T4). The histology was dominated by the well-differentiated epidermal carcinoma (91%). Conclusion: Hypopharyngeal cancer is common and occurs at all ages. T3 - T4 tumors are bad factors, hence early diagnosis and appropriate management are important for improving the prognosis.

目的描述截至2013年5月27日通过中国国家流感样病例哨点监测系统发现的H7N9流感感染者的完整病例系列的特征。设计病例系列研究。研究场所中国大陆地区31个省份的554家哨点医院的门诊和急诊病例通过实验室确诊的H7N9感染病例和流感样病例哨点监测发现的病例间的相互对照来发现感染病例。流感样病例哨点监测系统发现的病例均符合世界卫生组织的流感样病例定义，每周每家监测医院采集10～15例流感样病例的鼻咽拭子用于病毒学检测。我们通过与地、省级和国家级疾病预防控制中心开展的公共卫生调查，获得相关的流行病学数据；通过病历回顾获得临床和实验室数据。主要研究指标病例系列的流行病学、临床和实验室特征结果截至2013年5月27日实验室确诊的130例H7N9感染病例中，5例（4％）是通过流感样病例哨点监测系统首先发现的。5例病例的平均年龄为13岁（2～26岁），均无基础性疾病。5例病例的暴露史、地理位置和发病时间与其他实验室确诊病例均类似。仅有2例病例需要入院治疗，所有5例病例均表现为轻度或中度感染且康复过程顺利。结论我们的发现支持H7N9流感病毒感染存在“临床冰山现象”，强调需要加强关注H7N9病毒感染多样化的临床表现。从公共卫生角度看，本研究为H7N9病毒感染存在相当大比例的轻症病例提供了间接证据。

Clinical outcomes of tricuspid valve repair accompanying left-sided heart disease

AIM To determine whether the need for additional tricuspid valve repair is an independent risk factor when surgery is required for a left-sided heart *** One hundred and eighty patients(68 ± 12 years,79 males) underwent tricuspid *** proportionalhazards regression model for multivariate analysis was performed for variables found significant in univariate *** Tricuspid regurgitation etiology was functional in 154 cases(86%),organic in 16 cases(9%),and mixed in10 cases(6%),*** mortality at 30 days was 11.7%.Mean follow-up was 51.7 mo with survival at 5 years of 73.5%.Risk factors for mortality were acute endocarditis [hazard ratio(HR) = 9.22(95%CI:2.87-29.62),P schemic heart disease requiring myocardial revascularization [HR = 2.79(1.26-6.20),P = 0.012],and aortic valve stenosis [HR = 2.6(1.15-5.85),P = 0.021].significant predictive factors from univariate analyses were double-valve replacement combined with tricuspid annuloplasty [HR = 2.21(1.11-4.39),P = 0.003] and preoperatively impaired ejection fraction [HR = 1.98(1.04-3.92),P = 0.044].However,successful mitral valve repair showed a protective effect [HR = 0.32(0.10-0.98),P = 0.046].Additionally,in instances where tricuspid regurgitation required the need for concomitant tricuspid valve repair,mortality predictor scores such as Euroscore 2 could be shortened to a simple Euroscore-tricuspid comprised of only 7 *** explanation may lie in the fact that significant tricuspid regurgitation following leftsided heart disease represents an independent risk factor encompassing several other factors such as pulmonary arterial hypertension and *** Tricuspid annuloplasty should be used more often as a concomitant procedure in the presence of relevant tricuspid regurgitation,although it usually reveals an overly delayed correction of a left-sided heart disease.

strong>Objective:strong> The purpose of this work was to determine the epidemiological, clinical and evolutionary aspects of the pulmonary embolism of the young person in the cardiology department of the University Hospital Center (CHU) Point G in Bamako-Mali. strong>Methodology:strong> This was an analytical study from January 01, 2018 to December 31, 2018 in the CHU Point G cardiology department, including all patients hospitalized during this period. strong>Results:strong> Of 1379 hospitalized patients, 19 patients were concerned by pulmonary embolism of the young person. The prevalence of pulmonary embolism of the young person was 1.37%. The most affected age group was 21 - 30 (47.4%) of patients. The predominance was female (89.47%) with a gender ratio of 0.11 in favour of women. The average age in the series was 29.79 years with extremes of 16 years and 40 years. Factors predisposing to pulmonary embolism were dominated by peri partum, cardiomyopathy and obesity with 47.3%, 31.57% and 21.1%, respectively. The dominant signs were chest pain and dyspnea in 94.7% and 89.5% of cases respectively. Pulmonary embolism was unlikely in 60.52% according to the Geneva and Wells score simplified. At the thoracic angioscanner, the embolism was bilateral in 52.6% of cases and distal in 36.8% of patients;in 10 patients who performed cardiac ultrasound, pulmonary arterial hypertension (70%), dilation of the right ventricle (20%) and left ventricular dilation (40%). Hyper leucocytosis (47.4%), anemia and low prothrombin rate (TP) (22.2%) were the most found biological abnormalities. More than 2/3 (68.5%) our patients had an intermediate mortality risk according to the PEsI (Pulmonary Embolism severity Index) score. The average hospital stay was 10 days. In this White Paper we present the potential of the Enhanced X-ray Timing and Polarimetry(eXTP) mission for determining the nature of dense matter; neutron star cores host an extreme density regime which cannot be replicated in a terrestrial laboratory. The tightest statistical constraints on the dense matter equation of state will come from pulse profile modelling of accretion-powered pulsars, burst oscillation sources, and rotation-powered pulsars. Additional constraints will derive from spin measurements, burst spectra, and properties of the accretion flows in the vicinity of the neutron star. Under development by an international Consortium led by the Institute of High Energy Physics of the Chinese Academy of sciences, the eXTP mission is expected to be launched in the mid 2020 s.
