AIM：gastro-esophageal reflux disease (gERD) is becoming increasingly common in Asia. Data on the efficacy of proton pump inhibitors in patients with non-erosive gERD (NERD)in Asia is lacking. This double-blind study compared the efficacy and safety of rabeprazole with esomeprazole in relief of symptoms in patients with ***: One hundred and thirty-four patients with reflux symptoms of NERD and normal endoscopy were randomized to receive rabeprazole 10 mg or esomeprazole 20 mg once daily for 4 wk. Symptoms were recorded in a diary and changes in severity of symptoms ***: At 4 wk of treatment, rabeprazole 10 mg and esomeprazole 20 mg were comparable with regards to the primary endpoint of time to achieve 24-h symptomfree interval for heartburn 8.5 d vs9 d and regurgitation 6 d vs 7.5 d. Rabeprazole and esomeprazole were also similarly efficacious in term of patient's global evaluation with 96% of patients on rabeprazole and 87.9% of patients on esomeprazole, reporting that symptoms improved(P=NS). Satisfactory relief of day- and night-time symptoms was achieved in 98% of patients receiving rabeprazole and 81.4% of patients receiving esomeprazole. Adverse events were comparable in both groups (P = NS).CONCLUSION: Rabeprazole 10 mg has a similar efficacy and safety profile in Asians with NERD as esomeprazole 20 mg. Further study is necessary to investigate whether the small differences between the two drugs seen in this study are related to the improved pharmacodynamicpro perties of rabeprazole. Both drugs were well tolerated.

BACKgROUND Acute diverticulitis is one of the most prevalent complications of diverticular disease and may result in abscess formation, perforation, fistula formation, obstruction, or bleeding. Diverticular abscesses may be initially treated with antibiotics and/or percutaneous drainage and/or surgery. Endoscopic ultrasound(EUS)-guided drainage techniques are increasingly used as a minimally invasive alternative to percutaneous or surgical approaches, as they are associated with better treatment outcomes, shorter recovery time and duration of *** SUMMARY A 57-year-old female presented to the emergency department on account of abdominal pain and fever. Clinical examination revealed tenderness in the left lower abdominal quadrant, with elevated inflammatory markers in laboratory tests. Abdominal computed tomography(CT) revealed an 8 cm × 8 cm × 5 cm well-encapsulated abscess of the sigmoid colon, surrounded by numerous diverticula. A diagnosis of Hinchey Ⅱ diverticular abscess was made, and the patient was admitted and commenced on appropriate antibiotic treatment. A transrectal EUS showed a fluid collection in direct contact with the sigmoid colon. Transluminal drainage was performed, and a lumen-apposing metal stent was inserted into the abscess collection. A follow-up CT scan showed a regression of the collection. The patient’s general condition improved, and the stent was removed during a follow-up transrectal EUS that revealed no visible *** We report the first successful management of a pelvic abscess in patient with Hinchey Ⅱ acute diverticulitis using EUS-guided transluminal drainage in Slovenia. The technique appears effective for well-encapsulated intra-abdominal abscesses larger than 4 cm in direct contact with the intestinal wall of left colon.

We report a case of acute chylous peritonitis mimicking acute appendicitis in a man with acute on chronic pancreatitis. Pancreatitis, both acute and chronic, causing the development of acute chylous ascites and peritonitis has rarely been reported in the English literature. This is the fourth published case of acute chylous ascites mimicking acute appendicitis in the literature.

After synthesis of an asymmetric tetradentate ONN＇O＇ Schiff base ligand （H2L） followed by reaction of the synthesized H2L with an equimolar mixture of methanolic solutions of the VO（acac）2, a new oxidovanadium（IV） Schiff base complex （VOL） was synthesized. The Schiff base ligand and its complex were characterized by FT-IR and UV-vis spectra and C, H, N analysis, The crystal structure of VOL was also determined by single crystal X-ray analysis. The VOL complex crystallizes in monoclinic space group Cc. The Schiffbase ligand acts as a tetradentate ligand through its two＇iminic nitrogens and two phenolic and acetylacetonate oxygens. Thermogravimetric analysis of the VOL showed that it decomposes in two steps and converts to mixed vanadium oxides at 477℃. In addition, thermal decomposition of the VOL complex in air at 660 ℃ leads to formation of V2O5 nanoparticles with the average size estimated from XRD 49 nm. The catalytic activity of the VOL complex was investigated in the epoxidation reaction and different reaction parameters were optimized. The results showed that the cyclic alkenes were efficiently converted to the corresponding epoxides, whereas the VOL did not appreciably convert the linear alkenes.

The distributions of two ubiquitous fouling cheilostome bryozoans,Cryptosula pallasiana(Moll,1803)and W atersipora sp.,on a ship moored for almost six years in Qingdao Bay show diff erences with respect to illumination,C ryptosula being dominant on the side of the ship which was exposed to the sun and Watersipora dominating on the other side which was in shadow for most of the *** interactions for substrate space were nearly always won by Watersipora,which succeeded in overgrowing the edges of C ryptosula colonies regardless of the side of the *** for the superiority of W atersipora in spatial competition with Cryptosula could include faster growth rate and the stronger feeding currents created by the larger lophophores of Watersipora.

Detection of H pylori infection by ELISA and Western blot techniques and evaluation of anti CagA seropositivity in adult Turkish dyspeptic patients

AIM: To detect H pylori infection and to evaluate the anti CagA seropositivity in adult Turkish dyspeptic patients. METHODS: We evaluated anti-H pylori IgA, Igg and anti-CagA antibodies using commercial enzyme-linked immunoassay (ELISA) and Western blot in dyspeptic Turkish patients. H pylori status was determined by histology and rapid urease testing. RESULTS: Fifty-six patients were entered. Forty-eight (85.7%) out of the 56 patients were positive for H pylori. H pylori Igg seropositivity was 82.1%, IgA seropositivity 48.2%. CagA ELISA showed that Igg was positive in 50% and IgA in 30.4% of those with H pylori infections. Western blot showed that Igg seropositivity was 80.4% and IgA seropositivity 33.9%. Western blot detected Igg antibodies with reactivity to CagA in 50%, VacA in 62.5%, UreB in 87.5%, UreA in 80.4%, and OMP in 57.1%. None of the tests had a sensitivity and specifi city above 80%. CONCLUSION: None of these commercial tests seems clinically useful for H pylori detection in adult dyspeptic patients, while Western blot can give seropositivity and determine anti-CagA, VacA virulence factor status of Turkish dyspeptic patients in the Izmir region.

Tensile and fracture properties of an Mg-RE-Zn alloy at elevated temperatures

Magnesium alloy EZ10 （Mg-RE-Zn） was deformed in tension at temperatures from 20 up to 520℃. A rapid decrease of the yield and tensile strength with temperature was observed at temperatures higher than 300 ℃. On the other hand, ductility of sam-ples rapidly increased in the same temperature range. Light microscopy and scanning electron microscopy was used to reveal the rea-son for these behaviours. Intermetallic particles in grain boundaries are responsible for excellent mechanical properties at lower tem-peratures. Diffusional processes occurring at temperatures higher than 300 ℃ significantly influenced the deformation mechanism as well as the fracture character.
