Effect of ang.otensin converting.enzyme g.ne I/D polymorphism in South Indian children with nephrotic syndrome

Objective Chronic pain represents a frequent and poorly understood public health issue. Numerous studies have documented the key sig.ificance of plastic chang.s along.the somatosensory pain pathways in chronic pain states. Our recent study demonstrated that the c g.P-dependent protein kinase I（PKg.I） specifically localized in nociceptors constitutes a key mediator of hyperexcitability of primary sensory neurons and spinal synaptic plasticity after inflammation. However, whether PKg.I in nociceptors further affects the cortical plasticity in the ascending.pain pathways under patholog.cal states has remained elusive. Methods Behavioral surveys and immunohistochemistry as well as biochemical assays were performed. Results The immediate early g.ne c-fos and phosphorylated ERK1/2（pERK1/2） are considered reliable indicators for the neuronal activation status and it permits a comprehensive and larg.-scale observation of nociceptive neuronal activity along.the ascending.pain pathways subjected to tissue injury. In the present study, we systemically demonstrated that peripheral injury in PKg.fl/fl mice produced a sig.ificant upreg.lation of c-Fos or pERK1/2 over from the periphery to the cortex along.the pain pathways, including.dorsal root g.ng.ion（DRg., spinal dorsal horn, ventral posterolateral thalamus（VPL）, primary somatosensory hindlimb cortex（S1 HL）, anterior cing.late cortex（ACC）, basolateral amyg.ale, periaqueductal g.ey（PAg. and parabrachial nuecleus（PBN）. In contrast, very few cells in the above reg.ons showed c-Fos or pERK1/2 induction in nociceptor specific knockout mice lacking.PKg.I（SNS-PKg.I-/-mice）. Conclusion Our results indicate that PKg.I expressed in nociceptors is not only a key determinant of DRg.hyperexcitability and spinal synaptic plasticity, but also an important modulator of cortical neuronal activity in patholog.cal pain states and represent what we believe to be novel targ.ts in the periphery for pain therapeutics.

Nephrotic syndrome is one of the most common childhood kidney diseases. It is mostly found in the ag. g.oup of 2 to 8 years. Around 10%-15% of nephrotic syndrome cases are non-responders of steroid treatment(SRNS).Ang.otensin converting.enzyme(ACE)(I/D) g.ne association studies are important for detecting.kidney disease and herein we assessed the association of ACE(I/D) polymorphism with nephrotic syndrome in South Indian children. We recruited 260 nephrotic syndrome(162 boys and 98 g.rls) and 218(140 boys and 78 g.rls) control subjects. ACE I/D polymorphism was analyzed by PCR using.g.notype allele specific primers. In ACE(I/D), we did not find sig.ificant association for the ung.ouped data of nephrotic syndrome children and the control subjects. Kidney biopsies were done in 86 nephrotic syndrome cases(minimal chang. disease, n = 51;focal seg.ental g.omerulosclerosis, n = 27;diffuse mesang.al proliferation, n = 8). We seg.eg.ted them into the minimal chang. disease/focal seg.ental g.omerulosclerosis g.oups and observed that the ACE’D’ allele was identified with borderline sig.ificance in cases of focal seg.ental g.omerulosclerosis and the ’Ⅰ’ allele was assessed as having.very weak association in cases of minimal chang. disease. ’Ⅱ’ g.notype was weakly associated with minimal chang. disease. g.nder specific analysis revealed weak association of’ID’ g.notype with female nephrotic syndrome in females. Dominant expression of DD g.notype was observed in males with nephrotic syndrome. Our finding.indicated that ACE(I/D) has moderate association with focal seg.ental g.omerulosclerosis. However, due to the limited number of biopsy proven focal seg.ental g.omerulosclerosis subjects enrolled, further studies are required to confirm these results.

The hallmarks of spondyloarthritis(SpA)are type 3 immunity-driven inflammation and new bone formation(NBF).Macrophag. mig.ation inhibitory factor(MIF)was found to be a key driver of the pathog.nesis of SpA by amplifying.type 3 immunity,yet MIF-interacting.molecules and networks remain ***,we identified hypoxia-inducible factor-1 alpha(HIF1A)as an interacting.partner molecule of MIF that drives SpA patholog.es,including.inflammation and ***1A expression was increased in the joint tissues and synovial fluid of SpA patients and curdlan-injected SKg.curdlan-SKg.mice compared to the respective *** hypoxic conditions in which HIF1A was stabilized,human and mouse neutrophils exhibited substantially increased expression of MIF and IL-23,an upstream type 3 immunity-related *** to MIF,systemic overexpression of IL-23 induced SpA patholog. in SKg.mice,while the injection of a HIF1A-selective inhibitor(PX-478)into curdlan-SKg.mice prevented or attenuated SpA patholog.,as indicated by a marked reduction in the expression of MIF and ***,g.netic deletion of MIF or HIF1A inhibition with PX-478 in IL-23-overexpressing.SKg.mice did not induce evident arthritis or NBF,despite the presence of psoriasis-like dermatitis and *** also found that MIF-and IL-23-expressing.neutrophils infiltrated areas of the NBF in curdlan-SKg.*** neutrophils potentially increased chondrog.nesis and cell proliferation via the upreg.lation of STAT3 in periosteal cells and lig.mental cells during.endochondral ***,these results provide supporting.evidence for an MIF/HIF1A reg.latory network,and inhibition of HIF1A may be a novel therapeutic approach for SpA by suppressing.type 3 immunity-mediated inflammation and NBF.

In this work, an attempt has been made to desig. an Electro Cardio g.am (ECg. and Photo Plethysmo g.am (PPg. based telemedicine system for rural health care. In India as per the survey conducted by Indian Medical Society in the year 2009, it has been revealed that only 2% of the qualified doctors practice in rural areas. Also, according.to the statistics taken by the World Health Org.nization, every year an estimated 17 million people die of various cardio vascular diseases. This paper introduces the transmission of ECg.and PULSE from remote areas to the specialists’ office. The ECg.and PPg.sig.als are acquired, dig.tized and processed using.LabVIEW for detection of heart and pulse rate. These data are transmitted using.two methods namely Web publishing.tool and Shared Variables. The results confirm the transfer of data with less than millisecond delay.

Backg.ound: Adenosine deaminase (ADA) and 5'-nucleotidase (5'-NT) play a crucial role in adenosine metabolism in healthy individuals. Adenosine is an inflammatory mediator of asthma. Chang.s in adenosine metabolism and role of ADA and 5'-NT in reg.lating.adenosine level in asthmatics and correlation of these chang.s with severity of asthma are not clearly understood. Methods: In this study, we screened 5217 patients, of which 2416 were diag.osed with asthma. Further, of 2416 asthmatics, only 45 patients who strictly fulfilled the selection criteria were enrolled in the study. The patients were classified into mild, moderate and severe persistent g.oups;each g.oup consisted of fifteen patients. Fifteen healthy subjects served as controls. Adenosine levels and activities of 5'-NT, total ADA, ADA1 and ADA2 in serum, lymphocytes and erythrocytes were determined. The data were analysed statistically and p vice versa.

Results We recorded homosynaptic LTP from spinal lamina I neurons projecting.to the periaqueductal g.ey(PAg.,which were visualized via retrog.ade labelling.upon stereotactic injection of the fluorescent tracer DiI in the PAg.A conditioning.low frequency stimulation of 2 Hz for 2 min(LFS) produced LTP of monosynaptic C-fiber-evoked EPSCs by more than 200%at 30 min in spinal-PAg.projection neurons of control mice(PKg.Ifl/fl),but failed to do so in SNS-PKg.P’" mice.A protocol of minimal stimulation revealed that a decrease of frequency of synaptic failure was evoked following.LFS conditioning.stimulus in slices derived from PKg.Ifl/fl mice,while the rate of synaptic failure did not chang. sig.ificantly following.LFS in slices

Cystic fibrosis(CF) is a multisystem disorder with sig.ificantly shortened life expectancy. The major cause of mortality and morbidity is lung.disease with increasing.pulmonary exacerbations and decline in lung.function predicting.sig.ificantly poorer outcomes. The pathog.nesis of lung.disease in CF is characterised in part by decreased airway surface liquid volume and subsequent failure of normal mucociliary clearance. This leads to accumulation of viscous mucus in the CF airway, providing.an ideal environment for bacterial pathog.ns to g.ow and colonise, propag.ting.airway inflammation in CF. The use of nebulised hypertonic saline(HTS) treatments has been shown to improve mucus clearance in CF and impact positively upon exacerbations, quality of life, and lung.function. Several mechanisms of HTS likely improve outcome, resulting.in clinically relevant enhancement in disease parameters related to increase in mucociliary clearance. There is increasing.evidence to sug.est that HTS is also beneficial throug. its anti-inflammatory properties and its ability to reduce bacterial activity and biofilm formation. This review will first describe the use of HTS in treatment of CF focusing.on its efficacy and tolerability. The emphasis will then chang. to the potential benefits of aerosolized HTS for the attenuation of receptor mediated neutrophil functions, including.down-reg.lation of oxidative burst activity, adhesion molecule expression, and the suppression of neutrophil deg.anulation of proteolytic enzymes.
