Effects of the combination of mask preconditioning.with midazolam pretreatment on anxiety and mask acceptance during.pediatric inhalational induction and postoperative mask fear in children

Backg.ound Anxiety and fear frequently causes an aversion to applying.a face mask and increases difficulty during.pediatric induction. There is at present little study of this problem. Therefore, the aim of this study was to investig.te the effect of the combination of mask preconditioning.and midazolam pretreatment on mask acceptance during.pediatric induction and on postoperative mask fear. Methods One hundred and sixty children were randomly assig.ed into four g.oups： the mask preconditioning.g.oup （Mag., the midazolam pretreatment g.oup （Mig., the mask/midazolam combination g.oup （Ma/Mig., and the saline g.oup （SAg.. The Modified Yale Preoperative Anxiety Scale （m-YPAS） was employed to assess the anxiety in the operation room （OR）. A Mask Acceptance Score （MAS） was measured during.inhalational induction and the incidence of mask fear （MAS 〈2） was evaluated postoperatively. Results The Mag.and Ma/Mig.g.oups had the hig.est mask acceptance scores but there were no differences between these two g.oups （P 〈0.05）. The averag. anxiety level of children entering.the OR was much lower in the Mag.and Ma/Mig.g.oups than in the Sag.g.oup （P 〈0.05）. During.induction, the anxiety level increased in the Sag.and Mag.g.oups but decreased in the Mig.and Ma/Mig.g.oups （P 〈0.05）. At the postoperative third day, the incidence of mask fears was as hig. as 23% in the Sag.g.oup, 15% in the Mig.g.oup, but only 2.5% in the Mag.and Ma/Mig.g.oups. Conclusions The sing.e use of mask preconditioning.has a better influence than midazolam for increasing.mask acceptance during.inhalational induction and reducing.postoperative mask fear, reducing.the anxiety level during.induction, improving.induction compliance and shortening.the total mask time. A mask preconditioning.and midazolam combination did not increase mask acceptance during.inhalational induction, reduce mask fears postoperatively, improve induction compliance, nor shorten the total mask time. Backg.ound: There are six Anesthesiolog. training.centers in Thailand that are approved to operate the training.prog.am. An evidence of residents’ knowledg. about pediatric postoperative pain manag.ment is needed for improving.the prog.am. Objective: To assess the third year anesthesiolog. residents’ knowledg. about pediatric postoperative pain manag.ment. Materials and Methods: The questionnaire was adapted from previous studies. The questionnaire has 35 questions consisted of 17 multiple choice questions and 18 true or false questions to cover 2 domains: 1) use of ag.-appropriate pediatric pain assessment (10 questions) and 2) pediatric pain treatment (25 questions). Minimal passing.level of the questionnaire rated by three young.anesthesiolog. staffs was 76.2%. All 62 participants were the 3rd year anesthesiolog. residents from 6 training.centers. Data were analyzed by descriptive statistics. Results: The response rate was 95.2%. Seventy-one percent of participants reported that they had learned about pediatric pain treatment. Of those, 55.9% rated their remaining.knowledg. at median level. The proportion of the correct score was 67.7% (mean 23.7 ± 2.9 SD) which was lower than the minimal passing.level. The hig.est score was 29 and the lowest score was 16. For pain assessment domain;the mean proportion of correct score was 65% (rang. 90%-40%). For pain treatment domain;the mean proportion of correct score was 68.8% (rang. 88%-44%). Conclusion: Anesthesiolog. residents’ knowledg. about pediatric postoperative pain manag.ment needs to be improved.
