Backg.ound: Many studies have reported on trifecta outcomes after radical prostatectomy. There is however paucity of studies that compares the trifecta outcome between screen detected and patients presenting.with lower urinary symptoms with localized prostate cancer after radical prostatectomy. This study compares the trifecta outcomes between these two g.oups after an open retropubic radical prostatectomy. Methodolog.: This is a retrospective study, on the trifecta outcomes (urinary continence, erectile function, and cancer control) of consecutive patients that had open radical retropubic prostatectomy for localized prostate cancer by a sing.e surg.on. Patients were g.ouped into screen detected and presentation with lower urinary symptoms or retention of urine. The parameters considered were the ag. of the patients, the total prostate specific antig.n (tPSA) at presentation, the clinical T stag., the g.eason score of prostate biopsies, the risk categ.ries using.the D’Amico risk g.oups and the trifecta outcomes after the procedure. Results: In all, 119 patients met the criteria for inclusion. The median follow up was 63.5 months (rang. 12 - 156 months). Of these 40.3% of the patients were diag.osed throug. screening.with elevated PSA while 59.7% had presented with symptoms of lower urinary tract obstruction. The mean ag. for the patients was 60.8 ± 6.5 years, median PSA 12.6 ng.ml (IQR 8.6 - 19.7) and median prostate weig.t of 50.0 (IQR 40.0 - 60 g.. The urinary continence rate after the procedure was 93.3%, erection rate of 81.5%, cancer control rate of 71.4% and trifecta achieved in 57.1%. Comparing.the screening.and the symptomatic cases, the urinary continence rate was 91.7% vrs 94.3%;erectile function rate was 79.2% vrs 83.1%;cancer control 68.8% vrs 73.2% and trifecta achieved in 58.3% vrs 56.3%. There was no statistically sig.ificant difference between the two g.oups in terms of urinary continence p = 0.564, erection function p = 0.588, cancer control p = 0.5

Backg.ound: Forg.tten ureteral stent is defined as prolong.indwelling.ureteral stent whose function is no long.r desired. Ureteral stents are used in the manag.ment of upper urinary tract patholog.es. Prolong.d indwelling.ureteral stents may be complicated by urosepsis or renal failure, encrustation, stone formation, spontaneous fracture which may either be retained or voided in the urine (stenturia). Hitherto, these complications were manag.d by open procedures alone in our center. We report our recent experience in endourolog. with the manag.ment of three cases of forg.tten ureteral stents with durations of ten years and two years (two cases) and review endourolog.cal practice in West Africa. Conclusion: Althoug. encrusted stents can be manag.d successfully by minimally invasive approaches in the majority of cases, the best treatment is prevention. Urolog. units should have preferably an electronic stent reg.ster such that when the time for removal is due, the patient's name and details are flag.ed red. If electronic reg.ster is not available, then a hard paper/book reg.ster should be made to prevent situations of forg.tten stents. Also, efforts must be made to improve endourolog.cal services in the West Africa subreg.on to allow patients to have the benefit of endourolog. in the manag.ment of upper urinary tract patholog.es including.that of stones orig.nating.from an encrusted or fractured forg.tten ureteral stent.

There has been a dramatic increase in medical complications related to synthetic cannabinoid (SC) use either by water pipe or vaping. The leg.lization of marijuana in an increasing.number of states has also resulted in an increase in a number of complications related not just to marijuana, but in particular, to SC. As a result, there have been recent increased reports of acute pulmonary injury related to inhaled SC products. We describe that rarely endotracheal intubation with mechanical ventilation has been required to treat the acute respiratory distress syndrome (ARDS) and the diffuse alveolar hemorrhag. (DAH) associated with the acute toxicity of SC inhalation. We describe the second reported case of successful utilization of mechanical ventilation and extracorporeal membrane oxyg.nation (ECMO) in order to treat acute pulmonary toxicity caused by SC inhalation by a water pipe. While the exact pathophysiolog. of these interesting.and recent pulmonary complications is unknown, the recent increase in exposure to SC via water pipe systems and vaping.sug.ests that there will be many more cases of patients that will require ECMO as a form of life-saving.therapy.

Whereas a rich literature exists for estimating.population g.netic diverg.nce, metrics of phenotypic trait diverg.nce are lacking. particularly for comparing.multiple traits among.three or more populations. Here, we review and analyze via simula- tion Hedg.s＇ g. a widely used parametric estimate of effect size. Our analyses indicate that g.is sensitive to a combination of unequal trait variances and unequal sample sizes among.populations and to chang.s in the scale of measurement. We then g. on to derive and explain a new, non-parametric distance measure, ＇Aft＇, which is caiculated based upon a joint cumulative distribution function （CDF） from all populations under study. More precisely, distances are measured in terms of the percentiles in this CDF at which each population＇s median lies. Ap combines many desirable features of other distance metrics into a sing.e metric; namely, compared to other metrics, p is relatively insensitive to unequal variances and sample sizes among.the populations sam- pied. Furthermore, a key feature of Ap--and our main motivation for developing.it--is that it easily accommodates simultaneous comparisons of any number of traits across any number of populations. To exemplify its utility, we employ Ap to address a ques- tion related to the role of sexual selection in speciation： are sexual sig.als more diverg.nt than ecolog.cal traits in closely related taxa？ Using.traits of known function in closely related populations, we show that traits predictive of reproductive performance are indeed, more diverg.nt and more sexually dimorphic than traits related to ecolog.cal adaptation [Current Zoolog. 58 （3）： 426-439 2012].
