Current antiretroviral therapy has reduced morbidity and mortality of HIV patients. However, their induced hepatotoxicity constitutes a risk. In this issue, we report a clinical case of fulminant hepatitis, observed in the HIV unit of the hepatog.stroenterolog. department of the g.neral hospital of Loandjili in Pointe-Noire. The patient is a 36-year-old female HIV treated with triple-dug.combination antiretroviral therapies (ART) including.one antiprotease (ritonavir) and two non-nucleoside reverse transcriptase inhibitors (nevirapine and efavirenz). He developed fulminant hepatitis five years after treatment initiation. He succumbed to the side effects. Althoug. antiretroviral combination therapies are the standard of care for HIV infection, increased vig.lance is warranted to early identify this side effect and adjust treatment in order to prevent fatal consequences.

Prevalence of hepatitis B in Pointe-Noire is 23%. The objective of this study was to evaluate the level of therapeutic manag.ment of patients with hepatitis B in Pointe-Noire. Methods: A retrospective, cross-sectional, descriptive and analytical study. Results: 302 patients included: 217 (72%) men and 85 (28%) women of median ag. 40 years. The viral load was measured in 140 (46%) patients and in 162 (54%), the viral load was not measured. Their distribution according.to the viremia was as follows: 0 - 1 log.= 1 (0.7%) patients;1.5 to 2 log.= 23 (16%) patients;2.5 to 3 log.= 49 (35%) patients;>3log.= 55 (39%) patients. Of the 302 patients included, 36 (11%) patients were treated with antivirals and 253 (83%) patients were not. Conclusion: The number of patients treated for chronic viral hepatitis B was low. Our results show interest for improved preventive and curative manag.ment of hepatitis B in Cong. in order to reduce the existing.hig. prevalence.

本研究通过实证分析Econet-Leo Burundi公司,探讨布隆迪电信业服务质量与满意度之间的关系。众所周知,服务质量和顾客满意的密切关系是企业在激烈市场竞争中开展活动必须承认的重要理念。当今竞争形势下,提供高标准的服务是保持竞争优势的主要途径。满意的顾客将成为企业利润的源泉和基础,因为顾客的满意会使他重复购买,并带来积极的口碑和品牌忠诚。由此可见,顾客满意度对公司成功有着积极的影响。针对本研究问题,230人作为研究样本被选中。在确立了研究问题和目标后,通过问卷收集数据对研究假设进行了验证。详细的统计分析既包括简单的百分比分析和表格分析,主要用于数据的呈现和初步分析。同时,本研究使用回归分析和相关系数分析来验证研究假设。研究结果显示:顾客感知价值对服务质量有显著影响;顾客感知价值对顾客满意度有显著影响;服务质量与顾客满意度显著正相关。针对以上研究,文章建议企业必须密切关注为顾客提供的服务质量,因为这会影响顾客的满意度。为了确保顾客满意度达到一个非常高的标准,公司必须首先承认客户的期望,以及思考公司如何操作使顾客的期望获得满足。文章同时指出,顾客满意度有助于提高顾客忠诚度,企业应该欢迎来自顾客的想法,并且通过各种有效手段来准确衡量服务质量和客户满意度。

Summary: There is no evidence for comorbidity diabetes and hepatitis C virus infection in the Cong.. The aim of this work was to determine the seroprevalence and molecular biodiversity of HCV in order to contribute to improving.the manag.ment of Cong.lese diabetics. Patients and methods: It was a cross-sectional study that took place from 1 February to 30 September 2018 at the Brazzaville University Hospital, the Diabcare Health Center and the Adolphe Cissé Hospital in Pointe-Noire. It concerned diabetic patients followed in Brazzaville and Pointe-Noire ag.eeing.to the study, after obtaining.the opinion of the ethics committee of the research in health science. All samples collected were screened for the presence of anti HCV Ab using.a rapid ALERE HCV test and the Monolisa HCV Ag.Ab ultra test for confirmation in Cong.. Detection of the viral RNA was done by PCR retrotranscription and g.notyping.was performed according.to the reverse hybridization technique in France. Data analysis was done with EpiInfo 6.0 software (2016);the proportions were compared using.the Chi-square test or the Fisher test at the sig.ificance level of 5%. Results: Of 447 patients with diabetes mellitus, 49 had HCV positive serolog. and the sex ratio was 0.63. Seroprevalence of AC anti HCV was 11% (49/447);HCV RNA was detectable in 71.4% (n = 35) patients. The averag. ag. of the population was 62 ± 10 years with extremes rang.ng.from 26 to 82 years. The circulating.g.notypes were 4 (97.1%) and 1 (2.8%). Subtyping.was defined in 17.64% (n = 6) of g.notype 4 patients;undefined in 82.36% (n = 28) of g.notype 4 patients, and in one of g.notype 1 patients. The subtypes identified were subtype 4e (60%), subtype 4e (8.8%), subtype 4a/4c/4d (5.8%), and subtype 4h (2.9%). Conclusion: The prevalence of HCV is hig. in our study. These are important data for the improvement of the manag.ment of diabetics.

Ing.stion of caustics by children is serious because of the increase in their frequency and the difficulties due to their manag.ment. Objective: To improve the manag.ment of caustic lesions in Pediatric Department at the University Hospital of Brazzaville. Patients and Methods: Prospective study was conducted from January 2014 to December 2015 in the Pediatric Departments of the UHB and centers of dig.stive endoscopy in Brazzaville. All children who ing.sted a caustic product and hospitalized were included after obtaining.parental’s consent. The studied parameters were: Ag., sex, nature and quantity of the caustic, the family’s attitude, clinical sig.s, endoscopic results according.to Zag.r’s classification, therapeutic modalities and evolution. Results: 8292 children were hospitalized and 68 because of the ing.stion of caustic, a frequency of 0.8%. The averag. ag. was 23.6 months [2 months - 16 years]. The male sex was predominant (57.4%). Ing.stion was accidental in 88.2% and voluntary in 11.8%. Factors favoring.ing.stion were storag. methods (81.5%) and parental’s inattention or neg.ig.nce (10.8%). Bleach was the most incriminated caustic (54.4%) followed by caustic soda (29.4%). The estimated quantity ing.sted was assumed to be minimal in 61.7%. Unsuitable acts were practiced by the family before the medical consultation in 66.1%. The dominant symptoms were dig.stive (47%) and neurolog.cal (25%). The averag. time to perform endoscopy was 48 hours in 88.2%. Endoscopy revealed lesions in 51.7%, the different stag.s were I (12 cases), II (16 cases) and III (3 cases). The mains factors of g.avity were inappropriate g.stures (85%) and the non-respect of the fasting.(9%). Proton pump inhibitors were used in 50%. The evolution was favorable for in 73.5%. Conclusion: Ing.stion of caustics by children is most often accidental favored by the inadequate packag.ng.and storag.. Prevention is about educating.people in order to reduce their frequency.

Introduction: Viral hepatitis B is a communicable disease that is preventable by vaccination. Knowledg. of transmission routes and prevention methods could play a key role in limiting.and reducing.the spread of the disease. Aim: This was a prospective study that was carried out from July 1 to September 30, 2018 in the department of g.stroenterolog. and internal medicine in people carrying.the hepatitis B virus. All patients were asked about their knowledg., attitudes and practices reg.rding.viral hepatitis B. Materials and Methods: This was a cross-sectional study, collecting.prospective data, which was carried out from July 1 to September 30, 2018 in the g.stroenterolog. department of CHUB (University Hospital of Brazzaville). Elig.ble for the study were all patients who consulted or were followed in the ward for HBV infection during.the study period. All patients were asked about their knowledg., attitudes and practices reg.rding.viral hepatitis B. After obtaining.informed consent, we asked patients about knowledg., attitudes and practices towards the hepatitis B virus. The data was collected on a pre-established survey sheet respecting.anonymity and confidentiality. CSPro 7 software and SPSS 17 software were used for data entry and analysis. Chi-square was used to compare the percentag.s. These were expressed with their 95% confidence intervals [95% CI]. A value of p 0.005 was considered sig.ificant. Results: Forty-two patients were included in the study, these were 66.7% (n = 28) men and 33.3% (n = 14) women. The averag. ag. was 38.76 ± 10.9 years. 45.2% of them (n = 19) were sing.e and 59.5% (n = 25) had the next hig.est level of education. Thirty-nine patients had no knowledg. of the source of their contamination;the sexual route was the most cited (61.9%). Alcohol consumption was rated badly by 38 patients (90.5%);of these 22 (52.4%) thoug.t it worsened the disease. Lack of knowledg. of an HBV carrier in the family was noted in 33 (78.6%) patients;20 (47.6%)

Diffuse varioliform g.stritis is a rare attack of the g.stric mucosa occurring.mostly on atopic g.ound. This report is based on the observation of a 16-year-old g.rl with diffuse varioliform g.stritis discovered after an internal medical examination via endoscope after attempting.a suicidal act by overdosing.herself with Ibuprofen. After the medical examination of the patient. Despite the quantity of ing.sted ibuprofen and presence of Helicobacter pylori on the biopsie, the particularity of this study is that there is no exudative lesion of the g.stric mucosa. With hélicobacter pylori eradication evolution was favorable.

The involvement of general practitioners in the early detection of viral hepatitis B and C must be paramount. The objectives of this work were to assess GPs’ knowledge, attitude and practice with regard to screening for viral hepatitis B and C. We carried out a cross-sectional analytical study among GPs recruited by simple random sampling from the list of GPs in the city of Brazzaville. The judgement criteria were knowledge and practice of screening for viral hepatitis B and C, assessed according to a Likert scale divided into 3 categories: very good, good and bad. One hundred and twenty-one general practitioners were included, including 48 women and 73 men, with a sex ratio of 1.52;the average age was 33 years;52.89% of them worked in the private sector with an average period of activity of 4 years. The majority of the doctors had a good knowledge (69.42%) and a bad practice (56.20%) of screening for viral hepatitis B and C. The duration of activity was the factor linked to knowledge of viral hepatitis B and C (p = 0.006) while age was the factor linked to the practice of screening for viral hepatitis B and C (p = 0.0366). In conclusion, in Brazzaville, general practitioners’ knowledge of screening for viral hepatitis B and C is good in general, but the practices are poor and require targeted in-service training.

Plummer-Vinson syndrome (PVS) is a rare condition characterized by classical triad;hig. dysphag.a, iron-deficiency anemia and oesophag.al ring.formation. We report a case of PVS in a 62-year-old patient admitted to the department of g.stroenterolog. and internal medicine for hig. dysphag.a. The clinical examination showed anemic syndrome, g.neral impairment and polyarthritis. Biolog.cal examinations concluded to iron deficiency anemia. The endoscopy had observed a circular ring.immediately above Killian’s mouth ruptured during.the examination. The patient was treated with iron and proton pump inhibitors. The oesophag.al symptomatolog. reg.essed completely from the second day of treatment.
