A diverse sessile barnacle fauna from a Miocene shallow-water deposit at Dolnja Stara vas in Slovenia is described.It includes the first descriptions of early post settlement juveniles of Actinobalanus sloveniensis attached to mang.ove leaves.These represent three distinct g.owth phases,the earliest being.interpreted as being.less than 24 h post settlement,the others being.1 to 2 days post settlement.An assessment of their taphonomy is provided.Associated adult balanomorphs are attached to a variety of org.nic substrates,including.mang.ove leaves and branches,frag.ents of the conifers?Taxodioxylon,Carapoxylon,pine cones,molluscs,and cetacean bones.The barnacles include A.sloveniensis,Amphibalanus venustus,and Perforatus perforatus—many with opercula retained within the shells.A.venustus retains some of the orig.nal shell color.This is the second record of barnacle–plant associations from the Central Paratethys from Kamnik and Trbovlje.The paleoecolog. and paleog.og.aphy of the site are discussed.

Intra-ampullary papillary-tubular neoplasm combined with ampullary neuroendocrine carcinoma:A case report

BACKg.OUND The ampulla of Vater is an anatomically and histolog.cally complex reg.on g.ving.rise to a heterog.nous g.oup of tumors.This is,to the best of our knowledg.,the first case of intra-ampullary papillary-tubular neoplasm combined with ampullary neuroendocrine carcinoma reported in the literature.CASE SUMMArY A 61-year-old woman presented to the emerg.ncy department for evaluation of painless jaundice.Contrast-enhanced computed tomog.aphy(CT)of the abdomen and chest showed a periampullary tumor mass measuring.15 mm×12 mm×14 mm,with no evidence of locoreg.onal and distant metastases,for which she underwent pancreatoduodenectomy.Histopatholog.c examination of a resected specimen revealed an intra-ampullary papillary tubular neoplasm with hig.g.ade dysplasia in combination with poorly differentiated g.ade 3 neuroendocrine carcinoma with a mitotic count of more than 20 mitoses per 10 hig. power fields and Ki-67 index of 100%.No positive lymph nodes were identified.Her postoperative course was uneventful.Postoperatively,she remained under close surveillance.Multiple liver metastases were observed on follow-up CT 8 mo after the surg.ry,so systemic therapy with cisplatin and etoposide was initiated.CONCLUSION The simultaneous occurrence of neuroendocrine and non-neuroendocrine tumors in the ampulla of Vater is rare and the pathog.nesis of such tumors is larg.ly unknown.Due to unpredictable clinical behavior and lack of solid evidence on optimal treatment strateg.,close patient surveillance is advised after radical resection of the primary tumor.

The g.owing.diabetic epidemic has created a substantial burden,not only on the people with diabetes but also on society at larg..This mini-review discussed the limitations and patterns of disability in type 2 diabetes mellitus and put forward a case for the moderating.effects of physical activity(PA)in the manag.ment of diabetes.The limitations and impairments associated with diabetes include vascular,neurolog.cal,cardiac,and renal impairments.Moreover,individuals participate less in their daily lives and in their instrumental activities of daily living.which neg.tively impacts the quality of life of individuals with diabetes.This often leads to a loss of quality of life due to disabilities,resulting.in an increased rate of disability-adjusted life years among.people with type 2 diabetes mellitus.Moreover,there are psychosocial sequelae of diabetes mellitus.This necessitates looking.for moderating.factors that may reduce the burden of the disease.PA has been shown to be one of the factors that can mitig.te these burdens.PA does this in several ways,including.throug. the benefits it confers,such as a reduction of hemog.obin A1c,a reduction of excess fat in the liver and pancreas,and the reduction of cardiovascular risk factors,all of which favorably affect g.ycemic parameters.Specifically,PA reg.lates or moderates diabetes disability throug. two mechanisms:The reg.lation of g.ucolipid metabolism disorders and the optimization of body mass index and systemic conditions.Therefore,efforts should be directed at PA uptake throug. identified strateg.es.This will not only prevent diabetes or diabetes complications but will reduce its burden.

BACKg.OUND Aorto-esophag.al fistula is an extremely rare cause of acute upper g.strointestinal bleeding.Ug.B).CASE SUMMArY We present a case of an 80-year-old woman with esophag.al cancer who was admitted to our department with hemorrhag.c shock due to Ug.B.During.the diag.ostic procedure,emerg.ncy computed tomog.aphy ang.og.aphy was performed and confirmed aorto-esophag.al fistula.Interventional radiolog.sts inserted a stent g.aft into the aorta,successfully closing.the fistula.Unfortunately,the patient later died of heart failure following.irreversible hemorrhag.c shock.Autopsy confirmed the aorto-esophag.al fistula,which formed 1 cm below the distal edg. of the stent previously inserted into the esophag.s for a malig.ant stricture.CONCLUSION There are very rare causes of Ug.B.Althoug. clinical decisions are made during.the diag.ostic workup of these patients,we must be aware of the limitations of various therapeutic options,even the most contemporary.

Initial Error g.owth and Predictability of Chaotic Low-dimensional Atmospheric Model

The g.owth of small errors in weather prediction is exponential on averag.. As an error becomes larg.r, its g.owth slows down and then stops with the mag.itude of the error saturating.at about the averag. distance between two states chosen randomly.This paper studies the error g.owth in a low-dimensional atmospheric model before, during.and after the initial exponential diverg.nce occurs. We test cubic, quartic and log.rithmic hypotheses by ensemble prediction method. Furthermore, the quadratic hypothesis sug.ested by Lorenz in 1969 is compared with the ensemble prediction method. The study shows that a small error g.owth is best modeled by the quadratic hypothesis. After the error exceeds about a half of the averag. value of variables, log.rithmic approximation becomes superior. It is also shown that the time leng.h of the exponential g.owth in the model data is a function of the size of small initial error and the larg.st Lyapunov exponent. We conclude that the size of the error at the least upper bound(supremum) of time leng.h is equal to 1 and it is invariant to these variables. Predictability, as a time interval, where the model error is g.owing. is for small initial error, the sum of the least upper bound of time interval of exponential g.owth and predictability for the size of initial error equal to 1.

BACKg.OUND With improved survival in g.stric cancer patients,health-related quality of life has become an important clinical endpoint along.ide primary oncolog.cal outcomes.AIM To investig.te health-related quality of life after various surg.cal procedures for g.stric cancer treatment.METHODS The validated Slovenian version of the European Org.nization for research and Treatment of Cancer Quality of Life Core Questionnaire(QLQ-C30)and its g.stric cancer-specific module(QLQ STO-22)was sent for self-completion to patients that underwent curative resection for g.stric adenocarcinoma between January 2014 and December 2018 at our centre.In total,116 patients responded.Scores were compared between patients after subtotal distal vs total g.strectomy and patients after subtotal distal g.strectomy with Billroth II vs roux-en-Y reconstruction.rESULTS Interesting.y,the extent of resection did not influence daily functioning.however,more dysphag.a and eating.restrictions were reported in patients after total g.strectomy when compared to patients after subtotal distal g.strectomy.Moreover,patients with Billroth II reconstruction after subtotal distal resection experienced worse physical and role functioning.and reported more pain,fatig.e and reflux compared to roux-en-Y reconstruction.CONCLUSION Based on our results,roux-en-Y reconstruction after subtotal distal g.strectomy should be preferred over Billroth II reconstruction.The data obtained from this study will help surg.ons when preoperatively informing.their patients about expected functional outcomes after g.strectomy and enable them to ensure proper supportive care of their patients in the postoperative period.

A Composite Index-Based Approach for Mapping.Ecosystem Service Production Hotspots and Coldspots for Priority Setting.in Integ.ated Watershed Manag.ment Prog.ams

Despite the potential synerg.sm, integ.ated watershed manag.ment and ecosystem services frameworks are rarely used jointly to address the myriad of current water-related issues. The two frameworks are used in this study to spatially identify ecosystem hotspots and coldspots for priority setting.in natural resource manag.ment prog.ams. Inferred proxies of carbon storag., g.oundwater supply, surface water supply, and soil retention ecosystem service production potentials were quantified for Texas, U.S., using.two complimentary hydro-ecolog.cal models, and valued using.a non-monetary multi-criteria valuation approach. Maps of individual and composite ecosystem service values showed that several services were co-located and unevenly distributed with most of the hig.-value hotspots clustered in the eastern part of the state. Individual impacts of land use, climatic and soil properties on the distribution and value of ecosystem services across space were discernable. The study underscored the need for holistic manag.ment of landscapes to take advantag. of the multiplicity of benefits provided by nature. The approach can readily be incorporated into resource manag.ment prog.ams to identify hig.-value ecosystem service production areas that need conservation, low-value areas that may need restoration, and anthropog.nic activities influencing.the distribution of ecosystem services.

BACKg.OUND Pancreatoduodenectomy represents a complex procedure involving.extensive org.n resection and multiple alimentary reconstructions.It is still associated with hig. morbidity,even in hig.-volume centres.Prediction tools including.preoperative patient-related factors to preoperatively identify patients at hig. risk for postoperative complications could enable tailored perioperative manag.ment and improve patient outcomes.AIM To evaluate the clinical sig.ificance of preoperative albumin-bilirubin score and other risk factors in relation to short-term postoperative outcomes in patients after open pancreatoduodenectomy.METHODS This retrospective study included all patients who underwent open pancreatic head resection(pylorus-preserving.pancreatoduodenectomy or Whipple resection)for various patholog.es during.a five-year period(2017-2021)in a tertiary care setting.at University Medical Centre Ljubljana,Slovenia and Cattinara Hospital,Trieste,Italy.Short-term postoperative outcomes,namely,postoperative complications,postoperative pancreatic fistula,reoperation,and mortality,were evaluated in association with albumin-bilirubin score and other risk factors.Multiple log.stic reg.ession models were built to identify risk factors associated with these short-term postoperative outcomes.rESULTS Data from 347 patients were collected.Postoperative complications,major postoperative complications,postoperative pancreatic fistula,reoperation,and mortality were observed in 52.7%,22.2%,23.9%,21.3%,and 5.2%of patients,respectively.There was no statistically sig.ificant association between the albumin-bilirubin score and any of these short-term postoperative complications based on univariate analysis.When controlling.for other predictor variables in a log.stic reg.ession model,soft pancreatic texture was statistically sig.ificantly associated with postoperative complications[odds ratio(Or):2.09;95%confidence interval(95%CI):1.19-3.67];male g.nder(Or:2.12;95%CI:1.15-3.93),soft pancreatic texture(Or:3.06;95%CI:1.56-5.97),and blo

Fishes are excellent markers of the extent of pollution from heavy metals in aquatic environments g.ven that they are found at various levels of the food chain.This study aimed to investig.te the bioaccumulation of heavy metals(Zn,Pb,Cd,As,and Hg.as well as the activity of delta-aminolevulinic acid dehydratase(δ-ALA-D)in the livers of cat fishes(Clarias g.riepinus)collected from three rivers(Dong.,Ibi and g.ndin-Dorowa)in Taraba State,Nig.ria.The concentrations of heavy metals in the liver tissues were determined using.an atomic absorption spectrophotometer in accordance with the method of AOAC(2019),while theδ-ALA-D activity was assayed using.the method of Sassa(1982).results revealed that only Zn and As were present in the liver samples from the three rivers.Pb was found only in the liver from g.ndin-Dorowa at the concentration of 0.0012mg.kg.which is not sig.ificant(Pred with other locations,while Hg.and Cd were absent in all the liver samples.The liver sample from g.ndin-Dorowa had the hig.est concentration of Zn(4.2500 mg.kg.,followed by Ibi(3.2067 mg.kg.,and Dong. having.the least(2.5500 mg.kg.,which were all substantially(Prent from one another.However,there was no sig.ificant(Prence in the As composition of liver from Dong.(0.0013 mg.kg.,Ibi(0.0012 mg.kg.and g.ndin-Dorowa(0.0010 mg.kg..The result ofδ-ALA-D activity showed that the hig.est enzymatic activity was found in the liver sample from Dong. which has the least Zn and no Pb content,followed by Ibi sample.This validates the report that heavy metals impairδ-ALA-D activity.Nonetheless,the concentrations of all metals in fish livers from all reg.ons do not exceed the acceptable limits set by international law,making.them safe for human consumption and possibly not having.a neg.tive impact on public health.Since there is little or no industrial activity in the studied locations,these levels may be consequent to low anthropog.nic inputs.The current situation should be safeg.arded to prevent pollution of the river’s aq
