Spinal Analgesia with Intrathecal Morphine versus Conventional Analgesia after Laparoscopic Colectomy: A Retrospective Cohort Study

Objective: Postoperative pain (POP) following abdominal surgery can vary from a few hours to several days. This acute, unrelieved pain can become chronic, requiring patients to take analgesics on an almost daily basis for comfort. Analgesia using general opioids has many side effects and intrathecal morphine is a good alternative. This study was conducted to evaluate the efficacy of intrathecal morphine (ITM) versus conventional analgesia in the management of postoperative pain in colectomy performed by laparoscopic surgery. Methods: Cohort study conducted at the Hôpital Nord in Marseille, from 01 January to 31 July 2021 in patients aged at least 18 years undergoing anaesthesia for scheduled colectomy by laparoscopic surgery. The primary endpoint was postoperative pain intensity and the secondary endpoints were morphine consumption, treatment side effects and length of hospital stay. Statistical analysis was performed using XLSTAT software. Results: We included 193 patients: 131 in the control group (conventional analgesia) and 62 in the ITM group. We observed: a significant decrease in pain (assessed by numerical scale) in favour of the ITM group in the post-anaesthetic care room, i.e. 3 (±4) vs 1 (±2), p 0 and H2: 2 (±2) vs. 1 (±2);p Conclusion: These results suggest that intrathecal morphine (ITM) in laparoscopic colectomy provides effective postoperative analgesia with low morphine consumption, and a reduction in morphine side-effects compared with conventional analgesia.

Severe Acute Hepatitis of Unknown Origin in Children: What Do We Know Today?

In May 2022, the UK International Health Regulations National Focal Point notified World Health Organization of 176 cases of severe acute hepatitis of unknown etiology in children under 10 years of age. From that moment on, cases of severe acute hepatitis of unknown origin in children began to be reported in several countries. As of June 17, 2022, a total of 991 cases had been reported in 35 countries worldwide, 50 children needed a liver transplant and 28 patients died. According to information published by eCDC, 449 cases have been detected in 21 eU countries. The children were between 1 month and 16 years of age. Adenovirus was detected in 62.2% of the analyzed samples. So far, the cause of these cases is unknown and many hypotheses remain open, but hepatitis A-e viruses and COVID-19 vaccines have been ruled out. A possible hypothesis has been published to explain the cause of these cases of severe hepatitis, according to which it could be a consequence of adenovirus infection in the intestine in healthy children previously infected with SARS-CoV-2. No other clear epidemiological risk factors have been identified to date. Thus, at this time, the etiology of the current cases of hepatitis remains under active investigation.

The conversion of methane to olefins,aromatics,and hydrogen(MTOAH)can be used to stably obtain hydrocarbons when the effect of the catalytic surface is optimized from the reaction engineering *** this study,Fe/Si C catalysts were packed into a quartz tube *** catalytic surfaces of Si C and the impregnated Fe species decreased the apparent activation energies(e_a)of methane consumption in the blank reactor between 965 and 1020℃.Consequently,the hydrocarbon yield increased by 2.4times at 1020℃.Based on the model reactions of ethane,ethylene,and acetylene mixed with hydrogen in the range of 500-1020℃,an excess amount of Fe in the reactor favored the C-C coupling reaction over the selective hydrogenation of acetylene;consequently,coke formation was favored over the hydrogenation *** gas-phase reactions and catalyst properties were optimized to increase hydrocarbon yields while reducing coke *** 0.2Fe catalyst-packed reactor(0.26 wt%Fe)resulted in a hydrocarbon yield of 7.1%and a coke selectivity ofen the ratio of the void space of the postcatalyst zone to the catalyst space was adjusted to be≥*** on these findings,the facile approach of decoupling the reaction zone between the catalyst surface and the gas-phase reaction can provide insights into catalytic reactor design,thereby facilitating the scale-up from the laboratory to the commercial scale.

Although only a single serotype of hepatitis e virus (HeV),the causative agent of hepatitis e,has been identified,there is great genetic variation among the different HeV isolates *** are at least four major recognized genotypes of HeV:genotypes 1 and 2 are mainly restricted to humans and linked to epidemic outbreaks in nonindustrialized countries,whereas genotypes 3 and 4 are zoonotic in both developing and industrialized *** human strains,genotype 3 and 4 strains of HeV have been genetically characterized from swine,sika deer,mongooses,sheep,and ***,there are approximately 11,000 human and animal sequences of HeV available at the International Nucleotide Sequence Database *** is the major cause of waterborne outbreaks of hepatitis in areas of poor ***,it is responsible for sporadic cases of viral hepatitis in not only endemic but industrialized countries as *** of HeV occurs predominantly by the fecal-oral route,although parenteral and perinatal routes have been *** infection develops in most individuals as a self-limiting,acute,icteric hepatitis;with mortality rates around 1%.However,some affected individuals will develop fulminant hepatic failure,a serious condition that is frequently fatal without a liver *** complication is particularly common when the infection occurs in pregnant women,where mortality rates rise dramatically to up to 25%.Among the preventive measures available to avoid HeV infection,two separate subunit vaccines containing recombinant truncated capsid proteins of HeV have been shown to be highly effective in the prevention of *** of them,HeV 239,was approved in China,and its commercialization by Innovax began in November 2012 under the name Hecolin(R).

Wheat grain quality characteristics have experienced increasing attention as a central factor affecting wheat end-use products quality and human ***,in the last decades a reduction in grain quality has been ***,it is central to develop efficient quality-related phenotyping *** this sense,one of the most relevant wheat features related to grain quality traits is grain nitrogen content,which is directly linked to grain protein content and monitorable with remote sensing ***,the relation between nitrogen fertilization and grain nitrogen content(protein)plays a central role in the sustainability of *** aiming to develop efficient phenotyping tools using remote sensing instruments and to advance towards a field-level efficient and sustainable monitoring of grain nitrogen status,this paper studies the efficacy of various sensors,multispectral and visible red-greenblue(RgB),at different scales,ground and unmanned aerial vehicle(UAV),and phenological stages(anthesis and grain filling)to estimate grain nitrogen *** models were calculated using vegetation indices at each sensing level,sensor type and phenological ***,this study explores the up-scalability of the best performing model to satellite level Sentinel-2 equivalent *** found that models built at the phenological stage of anthesis with UAV-level multispectral cameras using red-edge bands outperformed grain nitrogen content estimation(R2=0.42,RMSe=0.18%)in comparison with those models built with RgB imagery at ground and aerial level,as well as with those built with widely used ground-level multispectral *** also demonstrated the possibility to use UAV-built multispectral linear models at the satellite scale to determine grain nitrogen content effectively(R2=0.40,RMSe=0.29%)at actual wheat fields.

Plant isoprenoids are formed from precursors synthesized by the mevalonate （MVA） pathway in the cytosol or by the methyl-D-erythritol 4-phosphate （MeP） pathway in plastids. Although some exchange of precursors occurs, cytosolic sesquiterpenes are assumed to derive mainly from MVA, while plastidial monoterpenes are produced preferentially from MeP precursors. Additional complexity arises in the first step of the MeP pathway, which is typically catalyzed by two divergent 1-deoxy-D-xylulose 5-phosphate synthase isoforms （DXS1, DXS2）. In tomato （Solanum lycopersicum）, the SIDXS1 gene is ubiquitously expressed with highest levels during fruit ripening, whereas SIDXS2 transcripts are abundant in only few tissues, including young leaves, petals, and isolated trichomes. Specific down-regulation of SIDXS2 expression was performed by RNA interference in transgenic plants to investigate feedback mechanisms. SIDXS2 down-regulation led to a decrease in the monoterpene β-phellandrene and an increase in two sesquiterpenes in trichomes. Moreover, incorporation of MVA-derived precursors into residual monoterpenes and into sesquiterpenes was elevated as determined by comparison of ^13C to ^12C natural isotope ratios. A compensatory up-regulation of SIDXS1 was not observed. Down-regulated lines also exhibited increased trichome density and showed less damage by leaf-feeding Spodoptera littoralis caterpillars. The results reveal novel, non-redundant roles of DXS2 in modulating isoprenoid metabolism and a pronounced plasticity in isoprenoid precursor allocation.
