Current surgical management of pancreatic endocrine tumor liver metastases

BACKgROUND: The management of metastatic disease in pancreatic endocrine tumors (PETs) demands a multidisciplinary approach and the cooperation of several medical specialties. The role of surgery is critical, even when a radical excision cannot always be achieved. DATA SOURCES: A PubMed search of relevant articles published up to February 2011 was performed to identify current information about PET liver metastases regarding diagnosis and management, with an emphasis on surgery. RESULTS: The early diagnosis of metastases and their accurate localization, most commonly in the liver, is very important. Surgical options include radical excision, and palliative excision to relieve symptoms in case of failure of medical treatment. The goal of the radical excision is to remove the primary tumor bulk and all liver metastases at the same time, but unfortunately it is not feasible in most cases. Palliative excisions include aggressive tumor debulking surgeries in well-differentiated carcinomas, trying to remove at least 90% of the tumor mass, combined with other additional destructive techniques such as hepatic artery embolization or chemoembolization to treat metastases or chemoembolization to relieve symptoms in cases of rapidly growing tumors. The combination of chemoembolization and systemic chemotherapy results in better response and survival rates. Other local destructive techniques include ethanol injection, cryotherapy and radiofrequency ablation. CONCLUSION: It seems that the current management of PETs can achieve important improvements, even in advanced cases.

To describe a case of probable relapsing autoimmune hepatitis associated with vaccination against hepatitis A virus （HAV）. A case report and review of literature were written concerning autoimmune hepatitis in association with hepatitis A and other hepatotropic viruses. Soon a^er the administration of formalin-inactivated hepatitis A vaccine, a man who had recently recovered from an uncharacterized but self-limiting hepatitic illness, experienced a severe deterioration （AST 1687 U/L, INR 1.4）. Anti-nuclear antibodies were detectable, and liver biopsy was compatible with autoimmune hepatitis. The observation supports the role of HAV as a trigger of autoimmune hepatitis. Studies in helper T-cell activity and antibody expression against hepatic proteins in the context of hepatitis A infection are summarized, and the concept of molecular mimicry with regard to other forms of viral hepatitis and autoimmunity is briefly explored.

Numerous industrial applications of spouted beds involve a mixture of granular particles, where differences in particle size, density, and shape can cause particle segregation, affecting the quality of flow and decreasing product homogeneity. The aim of this study was to investigate the particle segregation and fluid dynamics of a conical spouted bed operating with a binary mixture of particles of different densities. Using glass beads and polyethylene particles with a diameter of 4 mm, we determined the effects of the mass fraction of denser particles and static bed height on the minimum spouting condition and mixture index. The pressure drop and airflow in the minimum spouting condition increased with an increase in bed height or jetsam concentration. Regarding particle segregation, the jetsam particles tended to concentrate near the bottom, at the spout–annulus interface because they have a shorter trajectory than the lighter particles in the fountain region. Jetsam-rich mixtures with high initial static bed heights exhibited more efficient particle mixing, excluding those at the bottom of the bed.

AIM: To evaluate the long-term treatment outcomes in patients who underwent revision of external dacryocysto- rhinostomy (DCR) and nasal intubation by bicanalicular silicone tubing (BSTI) under endonasal endoscopic guidance. · METHODS: Data from 28 patients with recurrent dacryocystitis were retrospectively reviewed. Revision external DCR and bicanalicular nasal intubation by silicone tubing under endonasal endoscopic guidance was performed in 28 eyes of 28 patients. The patients were evaluated with respect to the reason of recurrence, time to recurrence, time to revision, duration of follow-up and surgical success. · RESULTS: Endoscopic endonasal examination detected an osteotomy-side obstruction by the excessive granulation tissue in 24 patients (86%), nasal septal deviation in three patients (10%) and nasal polyp in one patient (4%). Recurrence occurred after a mean duration of 5.3±3.7 months following the first operation. The mean time between the first DCR operation and the revision DCR was 11.5 ± 9.3 months. After a mean follow-up of 14.9±7.8 months, the rate of anatomic success alone was 85% (24/28); the rate of subjective success was 78% (22/28). · CONCLUSION: Revision external DCR and bicanalicular nasal intubation by silicone tubing under endonasal endoscopic guidance can be recommended in patients with recurrent dacryocystitis as a surgical approach that achieves satisfactory objective and subjective success rates.

Dear Editor,Virus-induced gene silencing （VIgS） is a fast and powerful method to study gene function in plants （Burch-Smith et al., 2004）. It is based on plant defense mechanisms against viral gene replication and allows high-throughput silencing of genes of inter- est （SenthiI-Kumar and Mysore, 2014）. The molecular mechanisms involved in post-transcriptional gene silencing （PTgS） have been studied intensively, and its steps are well known. The silencing pro- cess begins with the recognition through Dicer-like ribonucleases （DCL） of double-stranded RNA （dsRNA） that is generated during viral replication. Upon recognition, the dsRNA is processed into 21-24 nucleotide fragments, termed small interfering RNA （siRNA）. Based on sequence homology, both viral siRNA and plant mRNA associate with an RNA-induced silencing complex （RISC） and are targeted for degradation （Llave, 2010）.

An 86-year-old male patient was admitted in our cardiology ward with signs of congestive biventricular heart failure. The patient presented with deteriorating dyspnea on mild exertion and at rest the last days, compatible with class NYHA Ⅲ-Ⅳ heart failure, bilateral peripheral oedema, increased NT-proBNP （9198 pg/mL）, mildly elevated Troponin （TnT 64 pg/mL）, interstitial pulmonary oedema and bilateral pleural effusions in chest X ray.
