Appropriateness of indication and diagnostic yield of colonoscopy: First report based on the 2000 guidelines of the American Society for Gastrointestinal Endoscopy

AIM: To assess the appropriateness of referrals and to determine the diagnostic yield of colonoscopy according to the 2000 guidelines of the American Society for Gastrointestinal Endoscopy (ASGE).METHODS: A total of 736 consecutive patients (415males, 321 females; mean age 43.6±16.6 years)undergoing colonoscopy during October 2001-March2002 Were prospectively enrolled in the study. The 2000ASGE guidelines were used to assess the appropriateness of the indications for the procedure. Diagnostic yield was defined as the ratio between significant findings detected on colonoscopy and the total number of procedures performed for that ***: The large majority (64%) of patients had colonoscopy for an indication that was considered'generally indicated'; it was 'generally not indicated' for20%, and it was 'not listed' for 16% in the *** diagnostic yield of colonoscopy was highest for the 'generally indicated' (38%) followed by 'not listed'(13%) and 'generally not indicated' (5%) *** the multivariable analysis, the diagnostic yield was independently associated with the appropriateness of indication that was 'generally indicated' (odds ratio=12.3) and referrals by gastroenterologist (odds ratio = 1.9).CONCLUSION: There is a high likelihood of inappropriate referrals for colonoscopy in an open-access endoscopy system. The diagnostic yield of the procedure is dependent on the appropriateness of indication and referring physician's specialty. Certain indications 'not listed' in the guidelines have an intermediate diagnostic yield and further studies are required to evaluate whether they should be included in future revisions of the ASGE guidelines.

The hepatitis C virus(HCV)is estimated to affect 71 million people *** 2016,the World Health Organization adopted the first global health sector strategy to eliminate viral hepatitis as a public health threat by *** December 2018,the European Association for the Study of the Liver,International Liver Foundation convened an expert panel to address the elimination of HCV in *** steps have already been taken to eliminate HCV in Kuwait,including free HCV treatment for Kuwait’s citizens,high blood safety standards,and the implementation of screening and awareness *** expert panel made several recommendations aimed at accelerating the elimination of HCV in Kuwait:The development of a national strategy and action plan to guide all HCV elimination activities;the formation of a coordination mechanism to support collaboration between hepatitis working committees;the prioritization of microelimination at primary,secondary or tertiary facilities,in prisons and rehabilitation centers;and ensuring the involvement of multiple stakeholders–including relevant civil society groups–in all *** screening and linkage to care should be prioritized in Kuwait,with the expansion of the prescriber base to primary healthcare providers and nurse practitioners to be *** awareness and educating people about HCV infection also remain essential to achieve the goal of HCV ***,a national HCV registry should be developed to help monitor the implementation of viral hepatitis plans and progress towards achieving national and international targets.

The prevalence of nonalcoholic fatty liver disease(NAFLD)in the Middle East is increasing in parallel to an increase in the prevalence of associated risk factors such as obesity,metabolic syndrome,and type 2 diabetes *** 20%to 30%of the patients progress to develop nonalcoholic steatohepatitis(NASH),a histological subtype of NAFLD,with features of hepatocyte injury such as hepatocyte *** can progress to fibrosis,cirrhosis,and even hepatocellular *** thus causes a substantial burden on healthcare systems and it is imperative that appropriate strategies are discussed at a regional level to facilitate effective management tailored to the needs of the *** fulfil this unmet need,expert gastroenterologists,hepatologists,and endocrinologists from the region came together in three advisory board meetings that were conducted in Saudi Arabia,United Arab Emirates,and Kuwait,to discuss current local challenges in NAFLD screening and diagnosis,and the different available management *** experts discussed the disease burden of NAFLD/NASH in the Middle East;screening,diagnosis,and referral patterns in NAFLD;and available treatment options for NAFLD and *** paper summarizes the discussions and opinion of the expert panel on the management of NAFLD/NASH and also presents an extensive literature review on the topic.

Combined impacts of bioaugmentation and vermiremediation on crude oil-contaminated soil: Mitigating strategies for prospective environmental management

This study aimed to address the remediation of crude oil-contaminated soil by employing four different factorial setups:I-used the earthworms(Eudrilus eugeniae)for vermiremediation,II–utilized the bacteria(Pseudomonas aeruginosa FA-7)for bioaugmentation,III–combined earthworms+bacteria to perform vermiremediation+bioaugmentation,and IV–served as abiotic ***,garden soil weighing 1 kg and 50 g of desiccated cow dung were introduced into the experimental groups and abiotic ***,different quantities of crude oil ranging from 5 to 20 mL were *** on the results,from 5 mL of oil-contaminated soil,the factorial setup-III removed the maximum proportion of total petroleum hydrocarbons(TPH)in V+B1(58.06%).The abiotic control treatments AC1(1.11%)exhibited the lowest TPH removal *** bioremediated soil had significantly(P<0.05)different physicochemical properties than the initial crude oil contaminated *** instance,earthworm and microbial action resulted in considerable reductions in pH(4.72%),total organic carbon(44.03%),total nitrogen(42.42%),total phosphorus(52.17%),and total potassium(22.64%)by the end of the *** the 60th day of factorial setup-III,the bacterial population(386.470±0.432 CFU×10^(6) g^(−1)),actinomycetes(188.043±0.246 CFU×10^(8) g^(−1)),and fungi(16.603±0.202 CFU×10^(5) g^(−1))were *** germination rates of Trigonella foenum-graecum seeds were increased by 86.6%in factorial setup III(V+B1)when compared to other factorial ***,the incorporation of vermiremediation and bioaugmentation provides a distinctive platform for effective oil-contaminated soil remediation.

Developing Lexicons for Enhanced Sentiment Analysis in Software Engineering:An Innovative Multilingual Approach for Social Media Reviews

Sentiment analysis is becoming increasingly important in today’s digital age, with social media being a significantsource of user-generated content. The development of sentiment lexicons that can support languages other thanEnglish is a challenging task, especially for analyzing sentiment analysis in social media reviews. Most existingsentiment analysis systems focus on English, leaving a significant research gap in other languages due to limitedresources and tools. This research aims to address this gap by building a sentiment lexicon for local languages,which is then used with a machine learning algorithm for efficient sentiment analysis. In the first step, a lexiconis developed that includes five languages: Urdu, Roman Urdu, Pashto, Roman Pashto, and English. The sentimentscores from SentiWordNet are associated with each word in the lexicon to produce an effective sentiment score. Inthe second step, a naive Bayesian algorithm is applied to the developed lexicon for efficient sentiment analysis ofRoman Pashto. Both the sentiment lexicon and sentiment analysis steps were evaluated using information retrievalmetrics, with an accuracy score of 0.89 for the sentiment lexicon and 0.83 for the sentiment analysis. The resultsshowcase the potential for improving software engineering tasks related to user feedback analysis and productdevelopment.

The rapid growth of Internet of Things(IoT)devices has brought numerous benefits to the interconnected ***,the ubiquitous nature of IoT networks exposes them to various security threats,including anomaly intrusion *** addition,IoT devices generate a high volume of unstructured *** intrusion detection systems often struggle to cope with the unique characteristics of IoT networks,such as resource constraints and heterogeneous data *** the unpredictable nature of network technologies and diverse intrusion methods,conventional machine-learning approaches seem to lack *** numerous research domains,deep learning techniques have demonstrated their capability to precisely detect *** study designs and enhances a novel anomaly-based intrusion detection system(AIDS)for IoT ***,a Sparse Autoencoder(SAE)is applied to reduce the high dimension and get a significant data representation by calculating the reconstructed ***,the Convolutional Neural Network(CNN)technique is employed to create a binary classification *** proposed SAE-CNN approach is validated using the Bot-IoT *** proposed models exceed the performance of the existing deep learning approach in the literature with an accuracy of 99.9%,precision of 99.9%,recall of 100%,F1 of 99.9%,False Positive Rate(FPR)of 0.0003,and True Positive Rate(TPR)of *** addition,alternative metrics,such as training and testing durations,indicated that SAE-CNN performs better.
