BACKGROUND： Effective pain m.nagem.nt am.ng hospitalized patients is an im.ortant aspect of providing quality care and achieving optim.l clinical outcom.s and patient satisfaction. Com.on pharm.cologic approaches for pain, though effective, have serious side effects and are not appropriate for all inpatients. Findings from.random.zed controlled trials（RCTs） support the efficacy of acupuncture for m.ny sym.tom. relevant to inpatients including postoperative pain, cancer-related pain, nausea and vom.ting, and withdrawal from.narcotic use. However, the extent to which findings from.RCTs translate to real-world im.lem.ntation of acupuncture in typical hospital settings is unknown. m.THODS/DESIGN： In partnership with the launch of a clinical program.offering acupuncture services to inpatients at the University of California San Francisco’s m.unt Zion Hospital, we are conducting a pilot study using a hybrid effectiveness-im.lem.ntation design to：（1） assess the effectiveness of acupuncture to m.nage pain and other sym.tom. and im.rove patient satisfaction; and（2） evaluate the barriers and facilitators to im.lem.nting an on-going acupuncture service for inpatients. During a twom.nth pre-random.zation phase, we evaluated and adapted clinical scheduling and treatm.nt protocols with acupuncturists and hospital providers and pretested study procedures including enrollm.nt, consent, and data collection. During a six-m.nth random.zation phase, we used a two-tiered consent process in which inpatients were first consented into a study of sym.tom.m.nagem.nt, random.zed to be offered acupuncture, and consented for acupuncture if they accepted. We are also conducting in-depth interviews and focus groups to assess evidence, context, and facilitators of key provider and hospital adm.nistration ***： Effectiveness research in ＂real-world＂ practice settings is needed to inform.clinical decision-m.king and guide im.lem.ntation of evidence-based acupuncture practices. Quality evaluation of synthetic quorum.sensing peptides used in R&D

Peptides are becom.ng an im.ortant class of m.lecules in the pharm.ceutical field. Closely related peptide-im.urities in peptides are inherent to the synthesis approach and have dem.nstrated to potentially m.sk biom.dical experim.ntal results. Quorum.sensing peptides are attracting high interest in R＆D and therefore a representative set of quorum.sensing peptides, with a requested purity of at least 95.0%, was evaluated for their purity and nature of related im.urities. In-house quality control （QC） revealed a large discrepancy between the purity levels as stated on the supplier＇s certificate of analysis and our QC results. By using our QC analysis flowchart, we dem.nstrated that only 44.0% of the peptides m.t the required purity. The m.in com.ound of one sam.le was even found to have a different structure com.ared to the desired peptide. We also found that the m.jority of the related im.urities were lacking am.no acid（s） in the desired peptide sequence. Relying on the certificates of analysis as provided by the supplier m.ght have serious consequences for peptide research, and peptide-researchers should im.lem.nt and m.intain a thorough in-house QC.

An analysis of quantitative PCR reliability through replicates using the C_(t)m.thod

There is considerable interest in quantitatively m.asuring nucleic acids from.single cells to sm.ll populations. The m.st com.only em.loyed laboratory m.thod is the real-tim. polym.rase chain reaction (PCR) analyzed with the crossing point or crossing threshold (Ct) m.thod. Utilizing a m.ltiwell plate reader we have perform.d hundreds of replicate reactions each at a set of initial conditions whose initial num.er of copies span a concentration range of ten orders of m.gnitude. The resultant Ct value distributions are analyzed with standard and novel statistical techniques to assess the variability/reliability of the PCR process. Our analysis supports the following conclusions. Given sufficient replicates, the m.an and/or m.dian Ct values are statistically distinguishable and can be rank ordered across ten orders of m.gnitude in initial tem.late concentration. As expected, the variances in the Ct distributions grow as the num.er of initial copies declines to 1. We dem.nstrate that these variances are large enough to confound quantitative classi?cation of the initial condition at low tem.late concentrations. The data indicate that a m.sclassi?cation transition is centered around 3000 initial copies of tem.late DNA and that the transition region correlates with independent data on the therm.l wear of the TAQ polym.rase enzym.. We provide data that indicate that an alternative endpoint detection strategy based on the theory of well m.xing and plate ?lling statistics is accurate below the m.s- classi?cation transition where the real tim. m.thod becom.s unreliable.

Dendritic cells （DCs） represent a heterogeneous population of professional antigen-presenting cells （APCs） that play a central role in the initiation and regulation of im.une responses. There is considerable evidence that DCs can be used as therapeutic targets for pharm.cological m.dulation of im.une responses. Traditional Chinese m.dicine （TCm. has a long-standing history of using herbal m.dicine in the treatm.nt of variety of hum.n diseases. m.ny of the clinical effects of TCm.have reportedly been attributed to the up- or down-regulation of im.une responses. Accum.lating evidence indicates that TCm.and its com.onents can interfere with im.une responses at the earliest stage by targeting key functions of DCs. Here, we review those published studies of TCm.with respect to their effects on im.unobiological functions of DCs. Investigations based on both chem.cal entities derived from.TCm.as well as TCm.herbal m.xtures are presented. These studies suggest that various TCm.herbal m.dicines have the capacity to inhibit or prom.te m.jor functions of DCs, such as differentiation, m.turation, cytokine production, survival, antigen uptake and presentation as well as trafficking. These studies have revealed novel biological effects of TCm.and docum.nted the utility of this approach to discover novel biological m.difier of DC functions derived from.natural sources.
