Ba.kground: Ea.ly explora.ion of the semi constra.ned “Floa.ing-Socket” tota. shoulder repla.ement (TSR) in 1974 led to a.prolifera.ion of va.ious unconstra.ned designs tha. a.lowed resection or retention of the humera. hea., depending upon the pa.hologica. process involved. Degenera.ive glenohumera. a.thritis with mild to modera.e involvement of subchondra. bone a.lowed for a.resurfa.ing option, while severe humera. hea. involvement required a.pa.tia. or full humera. hea. repla.ement a.ta.hed to a. intra.edulla.y stem for fixa.ion. a.l components evolved from cemented to cementless a.plica.ion by 1982. The purpose of this pa.er is to describe the progression of Buechel-Pa.pa. (B-P) shoulder repla.ement development from the ea.ly 1970’s in both cemented a.d cement less a.plica.ions. Methods: Clinica. eva.ua.ions of “Floa.ing-Socket” TSR, followed by B-P stem-type, resurfa.ing types, bipola.-type a.d revision components, a.l of which comprise the B-P Shoulder Repla.ement System, were performed over a.49-yea. period. Results: “Floa.ing-Socket” impla.ts improved the results of simple, constra.ned ba.l-in-socket designs, but genera.ly fa.led by glenoid component loosening in both chimpa.zee a.d huma. a.plica.ions. Unconstra.ned resurfa.ing-type components, both a.a.omica. humera. hea. a.d full proxima. humera. components, were quite successful, with minima. fa.lures observed in long-term studies. Bipola. sa.va.e impla.ts, used for severe proxima. deficiencies, revisions a.d ma.sive rota.or cuff a.thropa.hy, were a.so very successful;providing overhea. ra.ge of motion in ma.y pa.ients. Conclusions: Resurfa.ing hemia.thropla.ty, in pa.ients with inta.t or repa.ra.le rota.or cuff mecha.isms, ga.e the most sa.isfa.tory results a.d were the lea.t technica.ly complica.ed to perform, requiring minima. instrumenta.ion. Ba.kground: Globa.ly, a. estima.ed 80 million unintended pregna.cies comprising both mistimed a.d unwa.ted pregna.cies a.e recorded yea.ly. Yet only ha.f of the women a. risk of mistimed pregna.cy use contra.eptives. In developing countries, over 100 million fema.es ha.e unmet need, a.d na.iona. surveys in Gha.a.indica.e 23% unmet need ra.e. Methods: Using a.cross-sectiona. community-ba.ed a.proa.h, a.sa.ple size of 300 women of reproductive a.e were selected using multi-step cluster sa.pling techniques. The study wa. qua.tita.ive, using structured interviewer-a.ministered questionna.res. Results: Two-third (66%) of the women in reproductive a.e still ha. unmet need, 71% were currently pregna.t, a.d more tha. a.third (36%) confirmed ever ha.ing a.mistimed pregna.cy. Fifty-three percent (53%) of the women confirmed never communica.ing with their pa.tners on fa.ily pla.ning issues, a.little below ha.f (45%) took their own hea.th ca.e decisions. Seventy nine percent (79%) ever received fa.ily pla.ning services from a.hea.th professiona.. Fa.tors rela.ed to unmet needs included mistimed pregna.cy, level of educa.ion, preferred birth/pregna.cy interva., communica.ion between pa.tners a.d the a.tonomy to spend self-ea.nings. Conclusion: Considering tha. high ra.es of unmet need results in mistimed pregna.cy, improved policies a.ound the influence of unmet need on mistimed pregna.cies a.e needed.

[目的/意义]古籍整理与出版是古籍保护的重要内容和方法之一,目前学界对古籍整理成果的法律保护还存在争议。[方法/过程]通过可视化分析软件CiteSpa.e对2000-2022年CNKI数据库中收录的我国古籍整理成果法律保护相关的130篇核心期刊论文进行统计分析,从发文数量、研究机构、核心作者、研究热点与前沿等方面,对近二十年我国古籍整理的法律保护研究成果进行梳理和总结。[结果/结论]结果表明:我国古籍整理成果法律保护研究的发文量呈波浪式增长趋势;高产机构主要由国家图书馆、天津师范大学古籍保护研究院构成,高校图书馆是研究的主要力量,但机构间合作不密切;核心作者共现网络密度低,合作程度有待提高;研究态势主要趋向于多学科交叉融合发展。在我国司法实践中,大多数法院认可古籍整理成果的独创性,符合著作权中作品的构成要件,少数法官则偏向于从民事权益的角度对其进行保护。而古籍整理成果的独创性在理论界成为争议焦点,部分学者提出了反不正当竞争法保护模式,但邻接权保护已渐成主流。从立法、司法、行政介入、合作研究四个方面,对完善我国古籍整理成果法律保护提出建议:加快古籍整理成果保护立法;完善司法实践中古籍整理成果的法律保护体系;利用行政手段加强古籍整理行业的监管力度;加强古籍整理成果保护研究机构的合作。
