Potential role of an athlete-focused echocardiogram in sports eligibility

Sudden cardiac death(SCD)of an athlete is a rare but tragic event and sport activity might play a trigger role in athletes with underlying structural or electrical heart diseases.Preparticipation screenings(PPs)have been conceived for the potential to prevent SCD in young athletes by early identification of cardiac diseases.The European Society of Cardiology protocol for PPs includes history collection,physical examination and baseline electrocardiogram,while further examinations are reserved to individuals with abnormalities at first-line evaluation.Nevertheless,transthoracic echocardiography has been hypothesized to have a primary role in the PPs.This review aims to describe how to approach an athlete-focused echocardiogram,highlighting what is crucial to focus on for the different diseases(cardiomyopathies,valvulopathies,congenital heart disease,myocarditis and pericarditis)and when is needed to pay attention to overlap diagnostic zone(“grey zone”)with the athlete's heart.Once properly tested,focused echocardiography by sports medicine physicians may become standard practice in larger screening practices,potentially available during first-line evaluation.

The quick evaluation of venous thromboembolism is a key point of modern medicine since the delayed diagnosis is associated with a worse prognosis.Venous ultrasound(VU)is a sensitive and rapidly performed test in cases of suspected deep venous thrombosis.Various protocols have been proposed for its execution,such as the study of the whole deep venous circulation of the lower limb or the analysis of the femoral-popliteal area.The aim is to detect a vessel thrombus and the most sensitive element is the non-compressibility with the probe.Initially,the thrombus is hypoechogenic and adherent to the vessel;later,it tends to organize and recanalize.Usually,in the early stages,the risk of embolism is higher.The role of studying the iliac axis and calf veins is still uncertain.VU is not useful for assessing response to anticoagulation therapy and it is unclear whether the persistence of thrombotic abnormalities can guide on a possible prolongation of therapy.

Applying a Context-based Method to Build a Knowledge Graph for the Blue Amazon

Knowledge graphs are employed in several tasks,such as question answering and recommendation systems,due to their ability to represent relationships between concepts.Automatically constructing such a graphs,however,remains an unresolved challenge within knowledge representation.To tackle this challenge,we propose CtxKG,a method specifically aimed at extracting knowledge graphs in a context of limited resources in which the only input is a set of unstructured text documents.CtxKG is based on OpenIE(a relationship triple extraction method)and BERT(a language model)and contains four stages:the extraction of relationship triples directly from text;the identification of synonyms across triples;the merging of similar entities;and the building of bridges between knowledge graphs of different documents.Our method distinguishes itself from those in the current literature(i)through its use of the parse tree to avoid the overlapping entities produced by base implementations of OpenIE;and(ii)through its bridges,which create a connected network of graphs,overcoming a limitation similar methods have of one isolated graph per document.We compare our method to two others by generating graphs for movie articles from Wikipedia and contrasting them with benchmark graphs built from the OMDb movie database.Our results suggest that our method is able to improve multiple aspects of knowledge graph construction.They also highlight the critical role that triple identification and named-entity recognition have in improving the quality of automatically generated graphs,suggesting future paths for investigation.Finally,we apply CtxKG to build BlabKG,a knowledge graph for the Blue Amazon,and discuss possible improvements.

DNA Tandem Repeats as Iterable Objects to Count Cell Divisions: A Computational Model

Cell lineages of nematodes are completely known: the adult male of Caenorhabditis elegans contains 1031 somatic cells, the hermaphrodite 959, not one more, not one less;cell divisions are strictly deterministic (as in the great majority of invertebrates) but so far nothing is known about the mechanism used by cells to count precise numbers of divisions. In vertebrates, each species has its invariable deterministic numbers of somites, vertebrae, fingers, and teeth: counting the number of iterations is a widespread process in living beings;nonetheless, it remains an unanswered question and a great challenge in cell biology. This paper introduces a computational model to investigate the possible role of satellite DNA in counting cell divisions, showing how cells may operate under Boolean logic algebra. Satellite DNA, made up of repeated monomers and subject to high epigenetic methylation rates, is very similar to iterable sequences used in programming: just like in the “iteration protocol” of algorithms, the epigenetic machinery may run over linear tandem repeats (that hold cell-fate data), read and orderly mark one monomer per cell-cycle (cytosine methylation), keep track and transmit marks to descendant cells, sending information to cell-cycle regulators.

Amyotrophic lateral sclerosis(ALS)is a neuromuscular condition resulting from the progressive degeneration of motor neurons in the cortex,brainstem,and spinal cord.While the typical clinical phenotype of ALS involves both upper and lower motor neurons,human and animal studies over the years have highlighted the potential spread to other motor and non-motor regions,expanding the phenotype of ALS.Although superoxide dismutase 1(SOD1)mutations represent a minority of ALS cases,the SOD1 gene remains a milestone in ALS research as it represents the first genetic target for personalized therapies.Despite numerous single case reports or case series exhibiting extramotor symptoms in patients with ALS mutations in SOD1(SOD1-ALS),no studies have comprehensively explored the full spectrum of extramotor neurological manifestations in this subpopulation.In this narrative review,we analyze and discuss the available literature on extrapyramidal and non-motor features during SOD1-ALS.The multifaceted expression of SOD1 could deepen our understanding of the pathogenic mechanisms,pointing towards a multidisciplinary approach for affected patients in light of new therapeutic strategies for SOD1-ALS.

14-bit SAR ADC with On-Chip Digital Bubble Sorting Calibration Technology

This article designs a 14-bit successive approximation register analog-to-digital converter(SAR ADC).A novel digital bubble sorting calibration method is proposed and applied to eliminate the effect of capacitor mismatch on the linearity of the SAR ADC. To reduce the number of capacitors, a hybrid architecture of a high 8-bit binary-weighted capacitor array and a low 6-bit resistor array is adopted by the digital-to-analog(DAC). The common-mode voltage VCM-based switching scheme is chosen to reduce the switching energy and area of the DAC. The time-domain comparator is employed to obtain lower power consumption. Sampling is performed through a gate voltage bootstrapped switch to reduce the nonlinear errors introduced when sampling the input signal. Moreover, the SAR logic and the whole calibration is totally implemented on-chip through digital integrated circuit(IC) tools such as Design Compiler, IC compiler, etc. Finally, a prototype is designed and implemented using 0.18 μm Bipolar-complementary metal oxide semiconductor(CMOS)-double-diffused MOS 1.8 V CMOS technology. The measurement results show that the SAR ADC with on-chip bubble sorting calibration method achieves the signal-to-noise-and-distortion ratio of 69.75 dB and the spurious-free dynamic range of 83.77 dB.

Patellar reconstruction in primary total knee arthroplasty using bone chips from routine cuts:A case report and review of literature

BACKGROUND Total patellectomy is currently reserved for exceptional cases,such as recalcitrant patellofemoral instability and comminuted fractures,due to its demonstrated negative impact on knee biomechanics.Therefore,managing patellectomy is crucial to mitigate its inherent deleterious effects.Various techniques have been described,including autologous or allogeneic bone grafts for reconstruction and soft tissue realignment to enhance the extensor mechanism.CASE SUMMARY A 73-year-old male underwent a patellectomy due to a comminuted fracture,subsequently developing osteoarthritis and experiencing a decline in functional status.Concurrent with total knee replacement,we conducted a patellar reconstruction,incorporating routine bone cuts and utilizing bone chips to fashion a new patella.This intervention resulted in the restoration of full extension and improvement of knee function.CONCLUSION Patellar reconstruction demonstrates benefits on knee mechanics and stabilization,contributing to enhanced outcomes and satisfaction following knee replacement.We present an affordable technique for managing patellectomized patients undergoing total knee replacement.

The Early-Middle Triassic Supervolcano in the Yangtze Block,South China and associated obstacles to biotic recovery

The end-Permian mass extinction was one of the major global crises spanning the entire Early Triassic or longer.Eruptions of volcanos were one of the factors that delayed the biotic recovery after this event.Supervolcano eruptions can cause catastrophic effects on global environment,climate,and life.Here we investigate the tuff layers from Early-Middle Triassic boundary in the Yangtze Block and identify a supervolcano eruption event.The zircon U-Pb ages of the section-Langdai,section-Daijiagou and section-Longmendong tuff samples are 247.1±1.9 Ma,247.6±2.0 Ma and 247.7±1.7 Ma,respectively.These ages mark the Olenekian-Anisian boundary.The zircon grains from the tuff layers have negativeεHf(t)(-15.3 to-0.8),two-stage Hf model(TDM2)ages(1.7 to 2.2 Ga)and display high-δ18O values(mostly>10‰).Clay minerals and quartz dominate the rock composition.The whole rock compositions show that the tuff layers were derived from magma of intermediate to felsic composition,which formed by the remelting of Paleoproterozoic materials of continental crust.The volcanic eruption site is located in the Jinshajiang-Ailaoshan-Song Ma suture zone in the southwestern margin of the Yangtze Block.A combination of the closure of the Paleo-Tethys Ocean Basin and the collision of the Indochina Block and South China contributed to the eruption,which was a supervolcano eruption under the active continental margin arc settings.We speculate that this supervolcano eruption might have contributed to the delayed biotic recovery after the end-Permian mass extinction.

Abnormal ACT in a Patient with Prekallikrein Deficiency Undergoing Cardiopulmonary Bypass

Prekallikrein deficiency is a disorder that often remains undiagnosed. Prekallikrein activates factor XII, which initiates the intrinsic coagulation pathway. Prekallikrein deficiency results in prolonged Partial Thromboplastin Time and Activated Clotting Time in absence of anticoagulants or active bleeding. This case report describes the anesthesia management of a patient with Prekallikrein deficiency who underwent cardiac surgery with Cardiopulmonary Bypass for correction of a congenital cardiac malformation. We highlight the importance of understanding the different tests available for the diagnosis of coagulation factors deficiency during administration of heparin in the setting of cardiovascular procedures under general anesthesia.

Expanding the Boundaries of the Chemical Space of DNA Methyltransferase 1 Modulators

DNA methyltransferase 1 (DNMT1), one of the main epigenetic targets, is involved in the duplication of the DNA methylation pattern during replication, and it is essential for proper mammalian development. Small molecule DNMT1 modulators are attractive for biochemical epigenetic studies and have the potential to become drugs. So far, more than five hundred small molecules have been reported as DNMT1 inhibitors. However, only a limited number of DNMT1 activators have been disclosed because, at least in part, DNMT1 activators are typically regarded as negative data in virtual screening campaigns or optimization projects. This manuscript aims to report the chemical structures and biological activity of small molecules that increase the enzymatic activity of DNMT1. Results of the biochemical experimental assays are discussed. It was found that small molecule activators have a large variety of chemical scaffolds but share pharmacophore features. Visual analysis of the chemical space and multiverse based on molecular fingertips supported that activators are structurally diverse. This is the first report of eight small molecules that increase the enzymatic activity of DNMT1 by more than 400% in an enzymatic-based assay. The outcome warrants further investigation of the epigenetic activity of the compounds in a counter-screen assay, e.g., cell-based and in vivo context.
