Ampullary carcinoid is a rare tumor that can present with gastrointestinal bleeding, obstructive jaundice or pancreatitis. Some of these tumors are associated with Von Recklinghausen disease. The usual surgical options are a biliary-enteric anastomosis, Whipple procedure or rarely a local resection. The mean survival dqes not appear to be much different after a pancreaticoduodenectomy versus local surgical excision. We report a very rare case of a non-metastatic ampullary carcinoid causing upper gastrointestinal bleeding, which was managed by endoscopic ampullectomy.

The role played by serendipity in the origin of modern psychopharmacology has proven to be controversial in scientific *** its original meaning(Walpole),serendipity refers to discoveries made through a combination of accidents and *** have implemented an operational definition of serendipity based on finding something unexpected or unintended,regardless of the systematic process that led to the accidental observation,and we have established four different patterns of serendipitous *** this paper,we have analyzed the role of serendipity in the discovery and development of classical antidepressant drugs,tricyclic antidepressants and monoamine oxidase inhibitors as well as heterocyclic,“atypical”or“second generation”*** discovery of the antidepressant properties of imipramine and iproniazid,the prototypes of tricyclic antidepressants and monoamine oxidase inhibitors,respectively,fits the mixed type II pattern;initial serendipitous discoveries(imipramine was an antipsychotic and iproniazid was an antituberculosis agent)led secondarily to non-serendipitous *** the other components of these two families of drugs were developed specifically as antidepressants,modifying the chemical structure of the series leaders,thereby allowing all of them to be included in the type IV pattern,characterized by the complete absence of *** the heterocyclic drugs,mianserin(originally developed as an antihistamine)also falls into the type II pattern.

Models on climate change have predicted an increase of temperature over the earth’s surface with potential drastic effects on living *** analyzed the relationships between climatic conditions(temperature,rainfall,and wind speed)and the abundance of blood-sucking flying insects(biting midges and blackflies)and nest-dwelling ectoparasites(mites,fleas,and blowflies)collected from blue tit nests during bird breeding seasons for a period of 10 *** temperature,rainfall,and wind speed showed significant differences among *** and wind speed increased during the period of study while rainfall *** midge,blackfly,and blowfly abundances increased across years but not flea and mite *** date decreased and brood size increased across *** of year variation,parasites were related to climatic *** example,biting midge,blowfly,mite,and flea abundances were positive and significantly related to average *** also found a positive and significant relationship between abundances of Haemoproteus infections and biting midge abundances during the first year of life of birds out of ***,abundance and prevalence of Lankesterella infections in yearlings were positive and significantly related to mite abundances during the year of birth of *** and Lankesterella infections were also significantly related to climatic variables and Haemoproteus and Lankesterella infections increased across *** addition,body condition of adult females and males were negatively related to flea larvae and blowfly abundance *** body condition was also negatively related to biting midge *** in climatic conditions across years could therefore affect several parasites of birds but also to birds themselves.

Bacteria in the air present patterns in space and time produced by different sources and environmental *** studies have focused on the link between airborne pathogenic bacteria in densely populated cities,and the risk to the population’s *** associated with particulate matter(PM)were monitored from the air of Mexico city(Mexico).We employed a metagenomic approach to characterise bacteria using the 16S rRnA *** bacteria sampling was carried out in the north,centre,and south of Mexico city,with different urbanisation rates,during *** added to the particles were sampled using high-volume PM_(10) *** ascertain significant differences in bacterial diversity between zones and seasons,the Kruskal-Wallis,Wilcoxon tests were done on alpha diversity ***-three air samples were collected,and DnA was sequenced using next-generation *** results indicated that the bacterial phyla in the north and south of the city were Firmicutes,cyanobacteria,Proteobacteria,and Actinobacteria,while in the central zone there were more *** were no differences in the alpha diversity indices between the sampled *** to the OTUs,the richness of bacteria was higher in the central *** diversity was higher in the rainy season than in the dry season;the Shannon index and the OTUs observed were higher in the central zone in the dry *** bacteria such as Kocuria,Paracoccus,and Micrococcus predominated in both seasonal times,while Staphylococcus,corynebacterium,and nocardioides were found during the rainy season,with a presence in the central zone.

An in vitro gas production （GP） technique was used to investigate the effects of combining different doses of Salix babylonica extract （SB） with exogenous fibrolytic enzymes （EZ） based on xylanase （X） and cellulase （c）, or their mixture （Xc; 1：1 v/v） on in vitro fermentation characteristics of a total mixed ration of corn silage and concentrate mixture （50：50, w/w） as substrate. Four levels of SB （0, 0.6, 1.2 and 1.8 mL g-1 dry matter （DM）） and four supplemental styles of EZ （1 μL g-1 DM; control （no enzymes）, X, c and Xc （1：1, v/v） were used in a 4×4 factorial arrangement. In vitro GP （mL g-1 DM） were recorded at 2, 4, 6, 8, 10, 12, 24, 36, 48 and 72 h of incubation. After 72 h, the incubation process was stopped and supernatant pH was determined, and then filtered to determine dry matter degradability （DMD）. Fermentation parameters, such as the 24 h gas yield （GY24）, in vitro organic matter digestibility （OMD）, metabolizable energy （ME）, short chain fatty acid concentrations （ScFA）, and microbial crude protein production （McP） were also estimated. Results indicated that there was a SBxEZ interaction （P〈0.0001） for the asymptotic gas production （b）, the rate of gas production （c）, GP from 6 to 72 h, GP2 （P=0.0095）, and GP4 （P=0.02）. The SB and different combination of enzymes supplementation influenced （P〈0.001） in vitro GP parameters after 12 h of incubation; the highest doses of SB （i.e., 1.8 mL g-1 DM）, in the absence of any EZ, quadratically increased （P〈0.05） the initial delay before GP begins （L） and GP at different incubation times, with lowering b （quadratic effect, P〈0.0001 ） and c （quadratic effect, P〈0.0001 ; linear effect, P=0.0018）. The GP was the lowest （P〈0.05） when the highest SB level was combined with cellulose. There were SBxEZ interactions （P〈0.001） for OMD, ME, the partitioning factor at 72 h of incubation （PF72）, GY24, ScFA, McP （P=0.0143）, and pH （P=0.0008）. Hepatitis c virus(HcV) infection is associated with increased thrombotic risk. Several mechanisms are involved including direct endothelial damage by the HcV virus, with activation of tissue factor, altered fibrinolysis and increased platelet aggregation and activation. In advanced stages, chronic HcV infection may evolve to liver cirrhosis, a condition in which alterations in the portal microcirculation may also ultimately lead to thrombin activation, platelet aggregation, and clot formation. Therefore in advanced HcV liver disease there is an increased prevalence of thrombotic phenomena in portal vein radicles. Increased thrombin formation may activate hepatic stellate cells and promote liver fibrosis. In addition, ischemic changes derived from vascular occlusion by microthrombi favor the so called parenchymal extinction, a process that promotes collapse of hepatocytes and the formation of gross fibrous tracts. These reasons may explain why advanced HcV infection may evolve more rapidly to end-stage liver disease than other forms of cirrhosis.

AIM： To study the blood-brain barrier integrity in prehepatic portal hypertensive rats induced by partial portal vein ligation, at 14 and 40 dafer ligation when portal pressure is spontaneously normalized. METHODS： Adult male Wistar rats were divided into four groups： Group Ⅰ： Sham14d, sham operated; Group Ⅱ： PHil, portal vein stenosis, （both groups were used 14 days after surgery）; Group Ⅲ： Sham4od, Sham operated and Group Ⅳ： PH4od Portal vein stenosis （Groups Ⅱ and Ⅳ used 40 d afer surgery）. Plasma ammonia, plasma and cerebrospinal fluid protein and liver enzymes concentrations were determined. Trypan and Evans blue dyes, systemically injected, were investigated in hippocampus to study blood-brain barrier integrity. Portal pressure was periodically recorded. RESULTS： Forty days afer stricture, portal pressure was normalized, plasma ammonia was moderately high, and both dyes were absent in central nervous system parenchyma. All other parameters were reestablished. When portal pressure was normalized and ammonia level was lowered, but not normal, the altered integrity of blood-brain barrier becomes reestablished. cOncLUSIOn： The impairment of blood-brain barrier and subsequent normalization could be a mechanism involved in hepatic encephalopathy reversibility. Hemodynamic changes and ammonia could trigger blood-brain barrier alterations and its reestablishment.
