Pulmonary In Situ Adenocarcinoma with Mosaic Paving Pattern

CONTEXT: Adenocarcinoma already comprises half the cases of lung cancer. Its insidious clinical evolution contributes to the fact that, in absolute numbers, lung tumor is the cancer with the highest mortality in the world. When still in situ, the adenocarcinoma is even quieter, making its typical presentation on the computerized tomography of an irregular semisolid nodule smaller than 3.0 cm. It is often diagnosed in a finding of examination in an asymptomatic patient. The prevalence of in situ adenocarcinoma (ISA) is less than 5% of pulmonary malignancies and its radiological presentation with a diffuse mosaic paving pattern is even more unusual, mimicking other conditions more frequent to this finding. CASE REPORT: We describe the case of a 44-year-old male patient with a history of chronic smoking admitted to the emergency room at a referral hospital in S?o Paulo on 12/16/2016 with a complaint of progressive dyspnea associated with dry cough for 3 months, intermittent fever and weight loss of 8 kg in 2 months. A chest X-ray and computed tomography showed discrete focal points of peribroncovascular consolidation, predominantly central, areas with frosted glass attenuation associated with smooth thickening of the interlobular septa, sometimes interspersed with areas of preserved parenchyma, giving an aspect of “crazing paving” with diffuse distribution by the pulmonary parenchyma. The patient underwent a biopsy with the anatomicopathological diagnosis of primary Adenocarcinoma in situ of the lung. CONCLUSION: We emphasize that the “crazing paving” of adenocarcinoma in situ pulmonary should be considered and known by the radiologist, because although isolated it is a rare condition, its early distrust in cases of atypical evolution of the most common injuries can avoid a diagnosis in phases more advanced and higher mortality.

Context: Bronchopulmonary sequestration is defined as a congenital malformation in which a portion of lung tissue does not communicate with the normal bronchial tree and the pulmonary arteries. Case Report: We present a case of a 35-year-old male complaining of left lower back pain and with a mass in the left thoracoabdominal transition. Imaging studies have suggested a pulmonary sequestration and its diagnosis was confirmed by histopathological study. Conclusion: Diaphragmatic extralobar pulmonary sequestration is a rare anomaly whose radiological diagnosis is difficult. This case is illustrative and demonstrates the importance of imaging methods to the correct diagnosis and effective treatment.

Background:Hemodialysis(HD) per se is a risk factor for *** the growing body of evidence on blood-flow restriction(BFR) exercise in HD patients,identification of possible risk factors related to the prothrombotic agent D-dimer is required for the safety and feasibility of this training *** aim of the present study was to identify risk factors associated with higher D-dimer levels and to determine the acute effect of resistance exercise(RE) with BFR on this ***:Two hundred and six HD patients volunteered for this study(all with a glomerular filtration rate of ded concentric and eccentric lifting phases(each lasting 2 s) and were supervised by a strength and conditioning ***:Several variables were associated with elevated levels of D-dimer,including higher blood glucose,citrate use,recent cardiovascular events,recent intercurrents,higher inflammatory status,catheter as vascular access,older patients(>70 years old),and HD ***,RE+BFR significantly increases D-dimer after 4 *** with borderline baseline D-dimer levels(400-490 ng/mL) displayed increased risk of elevating D-dimer over the normal range(≥500 ng/mL).Conclusion:These results identified factors associated with a heightened prothrombotic state and may assist in the screening process for HD patients who wish to undergo RE+BFR.D-dimer and/or other fibrinolysis factors should be assessed at baseline and throughout the protocol as a precautionary measure to maximize safety during RE+BFR.

Spices are defined as any aromatic condiment of plant origin used to alter the flavor and aroma of foods. Besides flavor and aroma, many spices have antioxidant activity, mainly related to the presence in cloves of phenolic compounds, such as flavonoids, terpenoids and eugenol. In turn, the most common uses of gum arabic are in the form of powder for addition to soft drink syrups, cuisine and baked goods, specifically to stabilize the texture of products, increase the viscosity of liquids and promote the leavening of baked products (e.g., cakes). Both eugenol, extracted from cloves, and gum arabic, extracted from the hardened sap of two species of the Acacia tree, are dietary constituents routinely consumed virtually throughout the world. Both of them are also widely used medicinally to inhibit oxidative stress and genotoxicity. The prevention arm of the study included groups: Ia, IIa, IIIa, Iva, V, VI, VII, VIII. Once a week for 20 weeks, the controls received saline s.c. while the experimental groups received DMH at 20 mg/kg s.c. During the same period and for an additional 9 weeks, the animals received either water, 10% GA, EUG, or 10% GA + EUG by gavage. The treatment arm of the study included groups Ib, IIb, IIIb e IVb, IX, X, XI, XII). Once a week for 20 weeks, the controls received saline s.c. while the experimental groups received DMH at 20 mg/kg s.c. During the subsequent 9 weeks, the animals received either water, 10% GA, EUG or 10% GA + EUG by gavage. The novelty of this study is the investigation of their use alone and together for the prevention and treatment of experimental colorectal carcinogenesis induced by dimethylhydrazine. Our results show that the combined use of 10% gum arabic and eugenol was effective, with antioxidant action in the colon, as well as reducing oxidative stress in all colon segments and preventing and treating genotoxicity in all colon segments. Proton exchange membrane(PEM)water electrolysis represents a promising technology for green hydrogen production,but its widespread deployment is greatly hindered by the indispensable usage of platinum group metal catalysts,especially iridium(Ir)based materials for the energy-demanding oxygen evolution reaction(OER).Herein,we report a new sequential precipitation approach to the synthesis of mixed Ir-nickel(Ni)oxy-hydroxide supported on antimony-doped tin oxide(ATO)nanoparticles(IrNiyO_(x)/ATO,20 wt.%(Ir+Ni),y=0,1,2,and 3),aiming to reduce the utilisation of scarce and precious Ir while maintaining its good acidic OER *** tested in strongly acidic electrolyte(0.1 M HClO_(4)),the optimised IrNi1Ox/ATO shows a mass activity of 1.0 mAµgIr^(−1) and a large turnover frequency of 123 s^(−1) at an overpotential of 350 mV,as well as a comparatively small Tafel slope of 50 mV dec^(−1),better than the IrOx/ATO control,particularly with a markedly reduced Ir loading of only 19.7µgIr cm^(−2).Importantly,IrNi1O_(x)/ATO also exhibits substantially better catalytic stability than other reference catalysts,able to continuously catalyse acidic OER at 10 mA cm^(−2) for 15 h without obvious *** in-situ synchrotron-based x-ray absorption spectroscopy confirmed that the Ir^(3+)/Ir^(4+)species are the active sites for the acidic ***,the performance of IrNi1Ox/ATO was also preliminarily evaluated in a membrane electrode assembly,which shows better activity and stability than other reference *** IrNi1Ox/ATO reported in this work is a promising alternative to commercial IrO_(2) based catalysts for PEM electrolysis.

The southern São francisco Paleocontinent(SFP)comprises Archean nuclei and Paleoproterozoic complexes encompassing magmatic arcs juxtaposed during a Rhyacian to Orosirian orogenic *** Juiz de Fora Complex(JFC)represents an imbricated thrust system that comprises orthogranulites with a wide compositional range formed in an intra-oceanic setting during the Siderian to the Orosirian and later accreted to the southeastern margin of the *** we report new petrological,geochemical,whole-rock Nd and Sr data,as well as zircon U–Pb ages from felsic and mafic orthogranulites from the *** new data is combined with a regional compilation that enables an evaluation of the interaction between magmatism and orogenetic episodes in the context of the consolidation of São francisco Paleocontinent during the Rhyacian–*** collisional Island Arc tholeiites(IAT),Tonalites-Tron dhjemites-Granodiorites(TTGs)and sanukitoid magmatism occurred from 2200 Ma to 2085 *** was followed by post-collisional magmatism,which is represented by hybrid granitoids coeval with the emplacement of E-MORB basic *** signatures for the Rhyacian to Orosirian evolution are highlighted by the dominance of negativeεNd(t)associated with Meso-to Neoarchean Nd TDMmodel ages as well as inherited zircon grains from the hybrid *** JFC is extensively highlighted in the literature as a primitive intra-oceanic arc,but here we propose the reworking or recycling of ancient crustal segments within the mature arc stage of the JFC,suggesting a Mesoarchean crustal source involved in the JFC evolution.

Sarcopenia and Anemia Are Predictors of Poor Prognostic in Cervical Cancer Patients

Objective: Evaluate pretreatment sarcopenia and anemia as prognostic factors in women undergoing treatment for cervical cancer (CC) with concurrent chemoradiotherapy (CCRT). Methods: 151 women with CC were analysed in this cohort study. Pretreatment computed tomography (CT) images were analysed to assess skeletal muscle index (SMI). Hazard ratios (HR) and multivariate Cox proportional HR were used to analyse association between low SMI, age, body mass index (BMI), haemoglobin levels, histological type, and International Federation of Gynaecology and Obstetrics (FIGO) stage with PFS and OS. Results: A total of 151 patients were included, 53 (35.1%) presented pretreatment sarcopenia;51 (34%) stage I/II and 100 (66%) stage III/IV. Among those patients in advanced stage (III/IV) 37 (70%) (p = 0.28) were sarcopenic at the beginning of treatment. Sarcopenia was associated with worse progression-free survival (PFS) and overall survival (OS) in our cohort [HR 0.97 (p = 0.01)] [HR 0.73 (p = 0.001)], as well as anemia [HR 0.73 (p = 0.001)] [HR 0.78 (p = 0.001)]. Linear regression models indicated that despite showing no association with age, neutrophil or platelet counts, sarcopenia was associated with pretreatment anemia levels (p = 0.01). After a multivariate analysis, only haemoglobin (anemia) and complete CCRT remained associated with PFS and OS. Sarcopenia and anemia were associated with worse PFS and OS in FIGO stage I/II. Conclusion: Pretreatment sarcopenia was significantly associated with low haemoglobin levels. Anemia and incomplete CCRT were independently associated with poor prognosis in women with CC. Pretreatment sarcopenia, as low SMI, was a predictor of poor prognostic in early stages of CC.
