Deep Vein Thrombosis in HIV and Tuberculosis Treatment in a Case of a 19-Year-Old Guinean Student

The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old male student. The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old student. Observation: It is Mr. S. C., aged 19, admitted for pain and swelling of the right lower limb, physical asthenia and notion of fever. Evolution 5 days, with no particular history of cardiovascular disease, anti-retroviral treatment for 1 year and anti-tuberculosis treatment for 3 months. On clinical examination heart sounds are irregular with a heart rate at 115 bpm, blood pressure at 110/70 mmhg. A febrile red painful swelling at right lower limb with positive Homans sign. 0°C (Temperature 37.7°C), Weight at 58 Kg. The rest of the clinical examination is without particularity. Venous Doppler echo of the lower limbs: shows the presence of acute deep vein thrombosis of the right lateral vein. Mr. S. C. to benefit the following Medical Treatment: lovenox 0.6 UI subcutaneously morning and evening, Sintrom 4 mg 1 comprimed at night, Tramadol 50 mg morning and evening;with a good clinical evolution. CONCLUSION: TB and HIV are chronic infections that result in widespread inflammation predisposing patients to a MTVE (Venous thromboembolic disease) table as well as rifampicin and anti-proteases.

Intussusception is a pathology of infants and young children. Its occurrence in adults is very unusual. In the vast majority of cases, it is secondary to a benign or malignant tumor. Small bowel malignancies are relatively rare, 1% - 5% of malignant tumors of the gastrointestinal tract. Acute intussusception, a rare pathology in adults, often presents with non-specific symptoms. Contrary to children, the organic origin remains preponderant in the elderly, where the origin of neoplasia must be suspected in first intention. We report the case of a 49-year-old patient admitted urgently from the Luxembourg “Mère Enfant” hospital center in Bamako, Mali, for an occlusive syndrome. Several abdomino-pelvic ultrasounds were performed in several clinics with mixed conclusions: Hydro-aeric distension and a cessation of matter and gas without a clearly visible mass (04/09/2022);Demonstration of the central hydro-aeric levels wider than high containing fine and regular folds corresponding to connivents whose aspect is in factor of left acute small intestine occlusion, to be completed by the surgical opinion (06/09/2022) and a third ultrasound showed a normal appearance of the liver, gallbladder, kidneys, pancreas, spleen, bladder and prostate;diffuse aero-colic distention without lesion of secondary appearance (21/11/2022). A frontal standing ASP dated 11/25/2022 showed hail-like hydro-aeric levels. Surgical management consisted of an open oncological resection of an obstructive bowel tumor with ileo-ileal invagination. The anatomopathological and immunohistochemical study of the surgical specimen concluded to diffuse small cell non-Hodgkin’s lymphoma, without vascular embolism. The resection limits pass into the healthy zone.

Introduction: Spontaneous pneumothorax is a rare but serious complication of tuberculosis. Miliary tuberculosis (MT) is a severe form of tuberculosis secondary to hematogenous spread of Mycobacterium tuberculosis. Objective: To report a case of MT complicated by pneumothorax. Methodology: This was a 25-year-old patient, farmer, followed up at the Pneumo-phtisiology department of the CHU du Point G for MT whose condition was improving after the introduction of anti-tuberculosis chemotherapy. He consulted again after experiencing chest pain. Clinical and imaging revealed a Spontaneous pneumothorax complicating MT. The treatment combined anti-tuberculosis chemotherapy, chest drainage and respiratory physiotherapy. Outcome was favorable with improvement of clinical and radiological signs. Conclusion: Pneumothorax complicating MT requires a reflective diagnostic approach and rapid management to improve its prognosis.

Introduction: Anemia is frequently associated with the natural course of people living with HIV (PLWHIV). The objective was to describe the evolution of anemia in PLWHIV during the first 6 months of ART and to identify the associated factors in the hematology service of the Ignace Deen national hospital of the Conakry University Hospital. Methods: This was a prospective, observational descriptive and analytical study lasting one year from August 1, 2019 to July 31, 2020. It focused on PLWHIV who were newly included in ART during the period of study in the Hematology Department of Ignace Deen Hospital. Results: Of 45 PLWHIV, 40 presented with anemia of 88.89%. The mean age was 40.16 years ± 12.29 years and extremes of 22 and 71 years. The female sex represented 65% of cases with a sex ratio of 0.54. Prolonged fever was the main reason for consultation, 97.5%. The HIV-1 serotype was represented in all anemic patients. At least one OI was found in 24 patients, 60%. MO anemia was severe (28.9%), moderate (44.4%) and mild (26.7%). At M6 it was moderate (5.9%) and light (94.1%). It was normochromic normocytic in 55%. At M0, statistical analysis was significant between anemia and OIs, WHO stage and CD4 count, but the link was not established at M6. Conclusion: Anemia is frequently associated with HIV infection linked to delayed treatment. Its development would be better with the establishment of good support. Specific actions should be taken to better identify the factors involved.

Every woman has the right to receive satisfactory quality prenatal care in reproductive health. Giving a life while remaining alive and without sequelae must be the slogan of any gynecologist - obstetrician and midwife, as well as any health worker. It was a descriptive cross-sectional study dealing with the evaluation of the quality of prenatal consultations at the Kadiolo referral health center (or RHC). Adevis Donabedian’s model for assessing the quality of care and services served as a benchmark. According to Donabedian quality means good technical care, with good interpersonal relationships, and adequate and comfortable premises. This study took place from April 24 to December 04, 2017 and aimed to assess the current level of the quality of prenatal consultations at the Kadiolo referral health center, to study the structures in place including infrastructure and personnel, to specify the procedures and to determine their results. This work has permitted us to classify the RHC of Kadiolo at level III with 85%, which means that the references evaluated were satisfactory, with the level of 75% to 94%. In terms of structure, a pricing system was deemed affordable by the opinion of pregnant women. The analysis of the level of the human dimension revealed that the health center was level III. The pregnant women were satisfied with the reception which was good in 76% of the cases, as well as with the quality of the respect of the privacy in 96% and confidentiality in 95%. Despite the satisfaction of pregnant women, gaps remain to be filled in concerning the quality of the services received at the Kadiolo referral health center so as to reach level IV.

This study on Menstrual Hygiene Management (MHM) was carried out in two secondary schools in the Bamako district, “Chaine Grise” and “Cheick Modibo Diarra” located respectively on the right bank and the left bank depending on the geographic position of the Niger river. This work took place over a period of 6 months from January 2019 to June 2019. The objective of this study was to study menstrual hygiene management in school?setting for girls aged 16 to 18 in two secondary schools in Bamako. It was a transversal and qualitative description. The study population consists of girls aged 16 to 18 years enrolled in one of the selected secondary schools.?At the end of this study, we arrived at the following results:?*50% of the girls in our study have poor knowledge about menstruation;?*10% of girls miss school at least one day a month during menstruation;?*90% of girls use hygienic cotton to absorb menstrual blood;?*90% of the sources of supply for hygienic products are?mothers.?The unsanitary conditions of the toilets, lack of light and the non-separation of the toilets according to gender guidelines were found in 99% of the cases: *99% of girls say that the poor state of health infrastructures was one of the causes of genital infections linked to poor management of menstrual hygiene;?*lack of water in the toilets (99%). Through these results, we conclude that,?in our context,?menstruation remains a taboo and shameful subject for girls. In addition, some of their menstrual hygiene practices are a real danger to their health.

Introduction: In sub-Saharan Africa and Mali, young women who have had heart surgery want to become pregnant. The occurrence of pregnancy in these women who have had heart surgery is becoming more and more frequent in our country because of the persistence of acute rheumatoid arthritis (RAA) and especially the increasingly easy access to heart surgery. General Objective: To study the evolution of pregnancy and the prognosis of childbirth in women who have undergone heart surgery. Methodology: This was a retrospective and descriptive study that took place over a period of five (5) years in the gynecology-obstetrics department of University Teaching Hospital (UTH) Gabriel Touré and the cardiology department of UTH Luxembourg. Was included in the study any pregnant woman admitted to the gynecology-obstetrics department of UTH Gabriel Touré and having a history of heart surgery. Introduction: Healthcare-associated infections (HAIs) are a public health issue. An infection is said to be associated with the care if it occurs during or after the care of a patient, and if it was neither present nor incubation at the beginning of the care. Objective: The purpose of this work was to study the bacteriology of infections associated with obstetric care in the gynecology-obstetrics department of CHU Gabriel Touré. Patients and Methods: This is an epidemiological, descriptive, analytical study conducted in the gynecology-obstetrics department of the CHU Gabriel Touré, from April 11th, 2016 to August 29th, 2016 (5 months). Data collection focused on the clinical and laboratory characteristics of healthcare-associated infections in patients during their hospitalization. Included in the study were any patients hospitalized in the Gynecology and Obstetrics Department who agreed to participate in the study. The criteria used to diagnose the associated infection were those of the Atlanta CDC. Operative wound monitoring was done up to the 30th postoperative day. Results: We have recorded 200 patients, out of whom 138 were operated on and 23 cases of bacterial infection associated with care (11.50%). The average age of the patients was 32.52 years ± 13.36 years against 29.36 years ± 10.28 years for the patients who did not present the infection. Seven point five percent of the evacuated patients had an infection associated with care. The most common types of infection were surgical site infection (60.86%), urinary tract infection (26.08%), endometritis and sepsis with 13.04% each. The isolated organisms were all resistant to Amoxicillin, to Amoxicillin + Clavulanic acid (88.88%) and to Ciprofloxacin (77.77%). The average duration of hospitalization for patients who developed the infection was 14.70 days. The lethality was 1.50%.
