The.significance.and diagnostic value.of se.0}n analysis in chronic bacte.ial prostatitis has be.n e.te.sive.y de.ate. and re.0}ins controve.sial. To inve.tigate.the.diagnostic re.e.ance.of se.0}n culture.in the.bacte.iological workup of prostatitis patie.ts, we.re.rospe.tive.y analyze. a clinical database.of 696 sym.tom.tic patie.ts. All patie.ts we.e.routine.y subje.te. to a four-glass te.t, followe. by se.0}n culture.and analysis. This allowe. to disse.t from.the.database.thre. diffe.e.t diagnostic sce.arios, and to com.are.the.＇two-glass＇ pre./post- m.ssage.te.t and the.standard ＇four-glass＇ te.t with a ＇five.glass＇ te.t （four-glass plus post-VB3 se.0}n culture.. The.＇five.glass＇ te.t showe. 3.6- or 6.5-fold incre.se. in re.ative.se.sitivity and le.se. re.uctions （-13.2% or -14.7%） in re.ative.spe.ificity for traditional uropathoge.s （TUs） com.are. with the.four-glass or two-glass te.t, re.pe.tive.y. The.are. unde. the.ROC curve.and Joude.＇s inde. we.e.incre.se., whe.e.s positive.and ne.ative.like.ihood ratios we.e.lowe. than com.arators, indicating that the.＇five.glass＇ assay m.y be.supe.ior in confirm.ng the.ne.ative.outcom. of both standard te.ts. The.five., four-, and two-glass te.ts de.e.te. TUs （e.te.obacte.iace.e. e.te.ococci, e.c.） in 120, 33, and 20 patie.ts and unusual pathoge.s （Stre.tococci, othe. Gram.positive.spe.ie., m.coplasm.ta, and othe.s） in 130, 56, and 45 patie.ts, re.pe.tive.y. Whe. patie.ts we.e.subje.te. to pharm.cological tre.tm.nt, including a com.ination of a fluoroquinolone.and a m.crolide. no diffe.e.ce. in e.adication rate. we.e.obse.ve. be.we.n groups diagnose. with diffe.e.t te.ts, irre.pe.tive.of pathoge. cate.ory. e.adication was associate. with long-te.m.sign/sym.tom.re.0}ssion; no significant inte.group diffe.e.ce. in sign/sym.tom.score. we.e.obse.ve. throughout a 24-m.nth off-the.apy follow-up pe.iod.
