De-escalating therapy in gastric aggressive lymphoma

The treatment of.primary gastric dif.use large B-cell lymphoma(DLBCL) has changed radically over the last 10–15 years, with the abandonment of.routine gastrectomy in f.vor of.more conservative therapies. Low-level evidence suggests that consolidation radiotherapy could be avoided in patients with limited-stage DLBCL of.the stomach who achieve complete remission af.er rituximab-CHOP combination. Small, recent prospective trials suggest that selected patients with limited-stage Helicobacter pylori(H. pylori)-positive DLBCL of.the stomach and f.vorable prognostic f.ctors can be managed with antibiotics alone, with excellent disease control and cure rates, keeping chemo-radiotherapy f.r unresponsive patients. This recommendation should equally regard patients with mucosa-associated lymphoid tissue-related or de novo DLBCL. f.ture studies should be f.cused on the establishment of.reliable variables able to distinguish the best candidates f.r exclusive treatment with H. pylori eradication f.om those who need f.r conventional chemo-immunotherapy.

To investigate the f.tty acid-based f.nctional lipidomics of.patients on long-term home parenteral nutrition receiving dif.erent intravenous lipid emulsions. METHODSA cross-sectional comparative study was carried out on 3 groups of.adults on home parenteral nutrition (HPN), receiving an HPN admixture containing an olive-soybean oil-based intravenous lipid emulsion (IVLE) (OO-IVLE; n = 15), a soybean- medium-chain triacylglycerol-olive-f.sh oil-based IVLE (SMOf.IVLE; n = 8) or HPN without IVLE (No-IVLE; n = 8) and 42 healthy controls (HCs). The inclusion criteria were: duration of.HPN ≥ 3 mo, current HPN admixtures ≥ 2 mo and HPN inf.sions ≥ 2/wk. Blood samples were drawn 4-6 h af.er the discontinuation of.the overnight HPN inf.sion. The f.nctional lipidomics panel included: the red blood cell (RBC) f.tty acid (f.) prof.le, molecular biomarkers [membrane f.uidity: saturated/monounsaturated f. ratio = saturated f.tty acid (Sf.)/monounsaturated f.tty acid (MUf.) index; inf.ammatory risk: n-6/n-3 polyunsaturated f.tty acid (PUf.) ratio = n-6/n-3 index; cardiovascular risk: sum of.n-3 eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) = n-3 index; f.ee radical stress: sum of.f. trans isomers = %trans index] and f. pathway enzyme activity estimate (delta-9-desaturase = D9D; delta-6-desaturase = D6D; delta-5-desaturase = D5D; elongase = ELO). Statistics were carried out using nonparametric tests. The amount of.each f. was calculated as a percentage of.the total f. content (relative%). RESULTSIn the OO-IVLE group, the percentage of.oleic acid in the RBCs was positively correlated with the weekly load of.OO-IVLE (r = 0.540, P = 0.043). In the SMOf.IVLE cohort, the RBC membrane EPA and DHA were positively correlated with the daily amount of.SMOf.IVLE (r = 0.751, P = 0.044) and the number of.HPN inf.sions per week (r = 0.753; P = 0.046), respectively. Previous analytical results on f.ow splitting are generalized to consider multiple boiling channels systems. The analysis is consistent with the approximations usually adopted in the use of.systems codes (like RELAP5 and TRACE5, among others) commonly applied to perf.rm saf.ty analyses of.nuclear power plants. The problem is related to multiple, identical, parallel boiling channels, connected through common plena. A theoretical model limited in scope explains this f.ow splitting without reversal. The unif.ed analysis perf.rmed and the conf.rmatory computational results f.und are summarized in this paper. New maps showing the zones where this behavior is predicted are also shown considering again twin pipes. Multiple pipe systems have been f.und not easily amenable f.r analytical analysis when dealing with more than f.ur parallel pipes. However, the particular splitting f.und (f.ow along N pipes dividing in one standalone pipe f.ow plus N -1 identical pipe f.ows) has been verif.ed up to f.urteen pipes, involving calculations in systems with even and odd number of.pipes using the RELAP5 systems thermal-hydraulics code.

腹泻是一类发病原因多且不易预防的多种疾病的症状,犬腹泻常发生于幼犬,其它年龄的犬也有发生.患病犬排便次数增多,排出稀软或水泻样粪便,有时可出现出血以及伴发恶臭.腹泻犬常常出现营养不良、维生素缺乏、贫血、免疫力低下以及水、电解质失调、酸碱平衡紊乱、食欲不振、消瘦、结膜苍白、无尿、被毛粗乱、伤口不易愈合甚至心率失常等症状.本调查通过中国农业大学动物医院收集腹泻犬病例152例,对腹泻病例的相关主诉、问诊、化验结果以及临床诊断等相关资料进行整理与分析,从而得出引起犬腹泻的病因主要有细小病毒、冠状病毒、犬瘟病毒、细菌、蛔虫、球虫、营养性原因、应激、肠套叠以及胰腺炎.
