Hepatic encephalopathy(HE) is one of the worst complications of liver disease and can be greatly influenced by nutritional status. Ammonia metabolism, inflammation and muscle wasting are relevant processes in HE pathophysiology. Malnutrition worsens the prognosis in HE, requiring early assessment of nutritional status of these patients. Body composition changes induced by liver disease and limitations superimposed by HE hamper the proper accomplishment of exams in this population, but evidence is growing that assessment of muscle mass and muscle function is mandatory due to the role of skeletal muscles in ammonia metabolism. In this review, we present the pathophysiological aspects involved in HE to support further discussion about advantages and drawbacks of some methods for evaluating the nutritional status of cirrhotic patients with HE, focusing on body composition.

Small wastewater treatment plants (WWTP) are frequently located, by necessity, in remote and isolated sites, which increases the difficulty of its energy supply. Some of them are located near the seaside, in environmental sensible zones, and due to tourism activity of these sites, seasonal effects related with population size fluctuation can occur, which can originate certain inefficiencies concerning WWTP design and energy supply. The objective of this paper is to describe a step by step procedure for evaluation of the wind potential of sites that are dependent of in-situ energy generation, as well as, a case study on the utilization of the potential wind energy in Magoito WWTP (5000 p.e.). The employed methodology comprised the collection of one year of in-situ wind data and its validation by comparison with historical data of about 10 years of a nearby anemometric station. The data provided by the two anemometric stations was statistically treated. It allowed the analysis of the results from the two stations are promissory in terms of wind availability and velocity. Finally, it comprised the simulation of the local wind conditions for a considerable larger area in order to find the best site for locating a wind turbine.

Ventriculoperitoneal shunting(VPS) is a widely accepted technique for the treatment of hydrocephalus. The probability of shunt dysfunction is pretty high throughout life. Laparoscopy has become a valuable tool to perform VPS and treat abdominal complications. An electronic literature search was performed to reveal the published data relating laparoscopy and ventriculoperitoneal shunt in Medline, Embase, Scielo and Lilacs databases. The keywords employed were "laparoscopy" OR "laparoscopic surgery" AND "ventriculoperitoneal shunt" OR "shunt" AND "surgery" OR "implantation" OR "revision" OR "complication". No high quality trials were developed comparing conventional laparotomic incision vs laparoscopic approach. Both approaches have evolved and currently there are less invasive options for laparotomy, like periumbilical small incisions; and for laparoscopy, like smaller and less incisions. Operating room time, blood loss and hospital stay may be potentially smaller in laparoscopic surgery and complications are probably the same as laparotomy. In revision surgery for abdominal complications after VPS,visualization of whole abdominal cavity is fundamental to address properly the problem and laparoscopic approach is valuable once it is safe, fast and much less invasive than laparotomy. Ventriculoperitoneal shunting is a widely accepted technique for the treatment of hydrocephalus. Laparoscopy assisted shunt surgery in selected cases might be a less invasive and more effective option for intrabdominal manipulation. The laparoscopic approach allows a better catheter positioning, lysis of fibrotic bundles and peritoneal inspection as well, without any additional complication.

In integrated factories of pulp and paper, the bleaching control is made by kappa number and/or brightness. However, the product paper is evaluated and valued for its whiteness. The level of final brightness of pulp certainly affects the demand for optical brighteners in paper manufacturing, nevertheless, there is not a direct correlation between consumption of optical brightener and final brightness, being the method of bleaching pulp, especially from the final stage of bleaching, also important in relation to this demand. Therefore, this study aimed to evaluate the potential of using the parameter whiteness as a tool to control bleaching pulp, instead of the traditional brightness and evaluate the impacts of different methods of bleaching and the pulp final brightness level in action of optical brighteners, to improve their performance. It was observed that the whiteness and brightness of the pulp are highly correlated and that the control of the bleaching plant can be done by any one of these two parameters. It was also concluded that, considering current prices of bleaching reagents and of optical brighteners, it is better to bleach the pulp up to lower brightness values (e.g. 88% ISO) and use larger amounts of optical brighteners to brighten the mass until the whiteness of 130 CIE. This conclusion is valid for whiteness measured before the size press. Further studies are needed to validate this conclusion considering the finished product.

Hepatic encephalopathy(HE)is a cognitive disturbance characterized by neuropsychiatric *** occurs in acute and chronic hepatic disease and also in patients with portosystemic *** presence of these portosystemic shunts allows the passage of nitrogenous substances from the intestines through systemic veins without liver ***,the embolization of these shunts has been performed tocontrol HE manifestations,but the presence of portal vein thrombosis is considered a *** this presentation we show a cirrhotic patient with severe HE and portal vein thrombosis who was submitted to embolization of a large portosystemic *** report:a 57 years-old cirrhotic patient who had been hospitalized many times for persistent HE and hepatic coma,even without precipitant *** had a wide portosystemic shunt and also portal vein *** abdominal angiography confirmed the splenorenal shunt and showed other *** larger shunt was embolized through placement of microcoils,and the patient had no recurrence of overt *** was a little increase of esophageal and gastric varices,but no endoscopic treatment was *** portosystemic shunts are frequent causes of recurrent HE in cirrhotic patients,portal vein thrombosis should be considered a relative contraindication to perform a shunt ***,in particular cases with many shunts and severe HE,we found that one of these shunts can be safely embolized and this procedure can be sufficient to obtain a good HE *** conclusion,we reported a case of persistent HE due to a wide portosystemic shunt associated with portal vein *** the patient had other shunts,she was successfully treated by embolization of the larger shunt.

Brazil is the world largest producer of sugarcane (Saccharum officinarum) leading to a great generation of residues such as bagasse and straw, which represent two thirds of sugarcane energetic potential. Regarding these residues energetic potential, it is interesting to study their application in biorefinery processes. Thus, this work aimed at performing a chemical characterization of sugarcane straw and bagasse from RB867515 cultivar grown in Rio Grande do Sul-Brazil aiming at their use as feedstock in biorefinery processes. The obtained results were compared to data from other states and it was possible to conclude that edaphoclimatic conditions of Rio Grande do Sul have little influence in sugarcane residues chemical composition. Sugarcane bagasse presents larger potential for energetic use because of its volatiles content (74.82%) slightly higher than straw (68.90%), besides its high lignin content (21.85%) and higher calorific value (18.70 MJ/kg). Both sugarcane residues produced in Rio Grande do Sul have potential use as substrate in the obtainment of high value-added products from their cellulosic fractions (41.30% and 37.25%, respectively). The evaluation of energetic and chemical potential of sugarcane bagasse and straw produced in Rio Grande do Sul leads to the conclusion that these materials have high potential for use as feedstock in biorefineries.

Paracoccidioidomycosis is a systemic granulomatous disease caused by fungus,and must be considered in the differential diagnosis of intra-abdominal tumors in endemic *** report a rare case of paracoccidioidomycosis in the pancreas.A 45-year-old man was referred to our institution with a 2-mo history of epigastric abdominal pain that was not diet-related,with night sweating,inappetence,weight loss,jaundice,pruritus,choluria,and acholic feces,without signs of sepsis or palpable *** ultrasonography(US)showed a solid mass of approximately 7 cm×5.5 cm on the pancreas *** computerized tomography showed dilation of the biliary tract,an enlarged pancreas(up to 4.5 in the head region),with dilation of the major pancreatic *** patient underwent exploratory laparotomy,and the surgical description consisted of a tumor,measuring 7 to 8 cm with a poorly-defined margin,adhering to posterior planes and mesenteric vessels,showing an enlarged bile *** drainage of the biliary tract,Rouxen-Y gastroenteroanastomosis,lymph node excision,and biopsies were performed,but malignant neoplasia was not *** analysis showed chronic pancreatitis and a granulomatous chronic inflammatory process in the choledochal lymph ***-alcohol resistant bacillus and fungus screening were ***-needle aspiration of the pancreas was performed under US *** smear was compatible with infection by Paracoccidioides *** report a rare case of paracoccidioidomycosis simulating a malignant neoplasia in the pancreas head.

Assessment of LLLT systemic effects on thyroid hormones function after dental titanium implant installation:An experimental rabbit model

This study aimed to assess the systemic effect of LLLT on thyroid gland functioning and consequently on calcium regulation through Triiodothyronine (T3) and Thyroxine (T4) measurements in rabbits’ serum. A total of thirty two New Zealand male rabbits were randomly distributed in four groups with eight animals each: control group C (nonirradiated animals), group EI (5 J/cm2 per session), group EII (10 J/cm2 per session) and group EIII (20 J/cm2 per session). All animals underwent lower left incisor extraction followed by immediate insertion of an osseintegrated implant, providing an equality of initial clinical condition between the groups. The experimental groups were irradiated with aluminium gallium arsenide diode laser (GaAlAs, λ = 830 nm, 50 mW, CW), during 13 days at each 48 hours, totalizing 7 sessions. Laboratorial T3 and T4 measurements were done in four distinct moments (before surgical procedure, immediately after surgical procedure, after the first LLLT session and after the last LLLT session) in all animals. The results obtained showed statistically significant differences in Triiodothyronine values between the groups throughout the experiment. It was concluded that the LLLT, in the protocol of irradiation used in this study, promoted a significantly alteration on rabbits’ serum hormonal levels.
