Clinical management of advanced gastric cancer: The role of new molecular drugs

Gastric cancer is the fourth most common malignant neoplasm and the second leading cause of death for cancer in Western countries with more than 20000 new cases yearly diagnosed in the United States. Surgery represents the main approach for this disease but, notwithstanding the advances in surgical techniques, we observed a minimal improvement in terms of overall survival with a significant increasing of relapsing disease rates. despite the development of new drugs has significantly improved the effectiveness of chemotherapy, the prognosis of patients with unresectable or metastatic gastric adenocarcinoma remains poor. Recently, several molecular target agents have been investigated; in particular, trastuzumab represents the first target molecule showing improvements in overall survival in human epithelial growth factor 2-positive gastric cancer patients. New molecules targeting vascular epithelial growth factor, mammalian target of rapamycin, and anti hepatocyte growth factor-c-Met pathway are also under investigation, with interesting results. Anyway, it seems necessary to select more accurately the population to treat with new agents by the identification of new biomarkers in order to optimize the results. In this paper we review the actual “scenario” of targeted treatments, also focusing on the new agents in development for gastric cancer and gastro-esophageal carcinoma, discussing their efficacy and potential applications in clinical practice.

The incidence of esophagogastric junction adenocarcinoma(AEG) is increasing worldwide. Barrett's esophagus(BE) associated with dysplasia is the main risk factor for the development of cancer. Currently, screening programs to individuate and eradicate BE represent the best way to reduce AEG cancer. Several endoscopic approaches are here discussed. Surgicalstrategies for different types of AEG cancer are now fairly standardized, and multidisciplinary strategies u s i n g c h e m o t h e ra py o r c h e m o ra d i o t h e ra py m ay improve the outcome of these patients. Here we briefly discuss the keypoints, main topics, and critical issues, according to accumulating evidence and taking into account our own experience.

Giorgio Vasari’s educational background and association with Renaissance humanists engendered his familiarity with the texts and imagery of classical,emblematic,and mythographic ***’s composition of images as a compendium of iconography for a decorative program was in the vein of the literary practices of Andrea Alciato(1482-1550),Pierio Valeriano(1477-1558),and Vincenzo Cartari(1531-1590),and followed Paolo Giovio’s advice on how to depict an emblematic image or impresa1(Giovio,1559,p.9).For Giovio(1483-1552),an impresa or badge must contain a figure and motto,its meaning should be clear and precise,the imagery must be pleasant to look at,and the motto must be brief,inventive,and *** sometimes Vasari did not follow his advice,relying more on the Renaissance Neoplatonic notion of a concept postulated by the Florentine philosopher Marsilio Ficino(1433-1499).In de vita coelitus comparanda(How Life Should Be Arranged According to the Heavens)2(Ficino,1489;1561-1563;1996,pp.7-19;Kerrigan&Braden,1989,pp.101-115).Ficino discusses the use and the magic potency of images by deliberating on the virtue of imagery,what power pertains to the figure in the Heavens and on Earth,which of the heavenly configurations are impressed on images by the ancients,and how the images are employed in antiquity3(Gombrich,1972,p.172;Chastel,1996,pp.81-89;Moore,1990,p.20,137,181;Bull,2006,pp.7-36).Vasari assimilated these concepts visually in the fresco painting of the Bride with a Rake(1548),located in one of the rooms in his house in Arezzo,the Chamber of Fortune(Chamber of Virtue),where he composed a paradoxical iconographic image-the subject of this *** essay is composed of two parts:an introduction to the location of the painting in the Casa Vasari in Arezzo and an iconographical and iconological interpretation of the imagery.

Sorbents for CO2 capture have been prepared by wet impregnation of a commercial active carbon （Ketjen-black, Akzo Nobel） with two CO2-philic compounds, polyethylenimine （PEI） and tetraethylenepentamine （TEPA）, respectively. The effects of amine amount （from 10 to 70 wt.%）, CO2 concentration in the feed, sorption temperature and gas hourly space velocity on the CO2 capture performance have been investigated. The sorption capacity has been evaluated using the breakthrough method, with a fixed bed reactor equipped with on line gas chromatograph. The samples have been characterized by N2 adsorption-desorption, scanning electron microscopy and energy dispersive X-ray （SEM/EDX）. A promising CO2 sorption capacity of 6.90 mmo1/gsorbent has been obtained with 70 wt.% of supported TEPA at 70℃ under a stream containing 80 vol% of CO2. Sorption tests, carried out with simulated biogas compositions （CH4/CO2 mixtures）, have revealed an appreciable CO2 separation selectivity; stable performance was maintained for 20 adsorption-desorption cycles.

Two different diets characterized by the absence of cereals or by the presence of conventional cereals were evaluated on the nutrient digestibility and faecal characteristics and faecal fermentative endproduct concentrations of 8 neutered adult Labrador retrievers housed at the Regional Centre Helen Keller(Messina,Italy)during the training work for the service guide for the ***(age = 17 ± 1 months,initial body weight [BW]= 26.3 ± 1 *** body condition score [BCS]= 4.5 ± 0.11)were divided into 2 homogeneous groups for sex(half males and half females).Dogs in the grain free(GF)group were fed a commercial diet characterized by the absence of grain cereals,and dogs in the control(CTR)group were fed a super-premium pet food characterized by conventional grains as the carbohydrate *** trial lasted 84 *** by a 7-d of adaption *** examination,*** faecal characteristics were *** statistical model included the effects of diet(GF ***),time(from d 0 to *** of the trial)and the interaction(diet x time).The high-protein,lowcarbohydrate dry diet(GF)offered higher apparent nutrient digestibility of protein(+10%;P =0.002)and fat(+7%;Pders consequent to stress coming from work and life in kennels,which cause in the Labrador retrievers an unusual weight loss.
