In the issue of Gastroenterology published October 2018,the article by Moon et al.1 demonstrated that screening patients with cirrhosis for hepatocellular carcinoma(HCC)by ultrasound scan(USS),measurement of serumα-fetoprotein(AFP),each alone or in combination,was not associated with the decreased HCC-related *** this matched case control study,all the study patients were registered in detail in the *** of Veterans *** study included 238 cases who died of HCC with cirrhosis,and the same number of matched controls with cirrhosis who had been enrolled in *** of Veterans Affairs care for the 4 years before the index date and alive at the time of their matched case's *** study retrospectively collected each case's USS and AFP screening data for 4 years before the date of HCC diagnosis or the equivalent index date in *** found that there was no significant difference of frequency of routine screening between the cases and ***,the authors interpreted these results as indicating that the routine screening would not reduce cancer-related mortality.

Liver Stiffness Measurement Can Reflect the Active Liver Necroinflammation in Population with Chronic Liver Disease:A Real-world Evidence Study

Advanced liver disease,which includes cirrhosis and hepatocellular carcinoma(HCC),is a serious problem in *** order to elucidate the status of these diseases,the medical records of 317 HCC patients and 62 liver cirrhosis patients were studied and a comparative analysis was *** the HCC patients,89.0%of cases were due to hepatitis B virus(HBV),8.2%cryptogenic and only 2.8%of cases were hepatitis C virus(HCV)*** contrast,11.3%and 9.7%of cases of cirrhosis were cryptogenic or HCV-related respectively,while HBV-related cases accounted for 79.0%The majority of patients were in their *** HBV-related HCC patients were the youngest and had the highest percentage of *** HCC patients had the highest levels of platelets,ALT and AST,and the lowest rate of *** with patients with HCC having severe fibrosis in noncancerous tissues,those with HCC and milder fibrosis were younger and had a lower rate of HBeAb positivity,a higher rate of HBsAb positivity and higher platelet *** and AFP levels,male gender and HBcAb positivity were the four risk factors for development of HCC among HBV-related cirrhosis patients.

We demonstrated a scheme of phase-locked terahertz quantum cascade lasers(THz QCLs)array,with a single-mode pulse power of 108 mW at 13 *** device utilizes a Talbot cavity to achieve phase locking among five ridge lasers with first-order buried distributed feedback(DFB)grating,resulting in nearly five times amplification of the single-mode *** to the optimum length of Talbot cavity depends on wavelength,the combination of Talbot cavity with the DFB grating leads to better power amplification than the combination with multimode Fabry-Perot(F-P)*** Talbot cavity facet reflects light back to the ridge array direction and achieves self-imaging in the array,enabling phase-locked operation of *** set the spacing between adjacent elements to be 220μm,much larger than the free-space wavelength,ensuring the operation of the fundamental supermode throughout the laser's dynamic range and obtaining a high-brightness far-field *** scheme provides a new approach for enhancing the single-mode power of THz QCLs.

肝硬化是各种慢性肝脏疾病的终末期改变，早期诊断对患者的治疗及其预后极为重要。尽管肝活组织检查是诊断肝硬化的金标准，但因其有创性，临床应用受限。传统的无创诊断肝硬化指标如天冬氨酸氨基转移酶／血小板比值指数、FIB-4（Fibrosis-4）模型和肝脏弹性指数测定在临床应用上各有其局限性。因此，寻找新的肝硬化血清学指标成为研究热点。根据对既往研究的分析及近期的研究发现，提出了高尔基体糖蛋白73是最有潜力的肝硬化诊断血清标志物的建议。现简要介绍其在肝硬化中的潜在应用价值及其可能的干扰因素，并讨论其相关机制。

Chronic hepatitis B virus (HBV) infection is a major global public health problem. Approximately 887,000 people die of HBV infection-related diseases annually, with cirrhosis and hepatocellular carcinoma (HCC) being the principal causes of mortality.[1] Timely antiviral therapy greatly reduces the risks of cirrhosis and HCC. However, unfortunately, of those patients who are eligible for antiviral treatment, only 25% of patients in clinic settings and 12% of those in community settings obtain timely antiviral therapy.[2] Therefore, reliable means of identifying patients with chronic HBV infection that require antiviral therapy are necessary, particularly for use in the community.

在我国,HBV感染所致病毒性肝炎、肝硬化、肝细胞癌等仍严重威胁我国公民的健康。积极的抗病毒治疗能够明显延缓肝脏疾病进程、减少终末期肝病的发生,但现有药物很难使患者临床治愈,特别是核苷(酸)类药物往往需要长期用药。新的血清学指标可以进一步优化抗病毒治疗方案,其中血清HBVRNA由肝组织内cccDNA转录,主要是未经或部分逆转录的前基因组RNA及其剪切变异体。总结了血清HBVRNA在HBV感染者血清中的存在形式和来源、影响其水平的因素、与其他血清病毒学指标的相关性、及其与肝组织cccDNA的关系,并评价了其在监测抗病毒疗效和预测停药后病毒反弹发生风险等方面的临床意义。
