BACKGROUND Hepatitis C is the leading cause of chronic liver disease worldwide and it significantly contributes to the burden of hepatocellular carcinoma(HCC).However,there are marked variations in the incidence and mortality rates of HCC across different geographical *** the advent of new widely available treatment modalities,such as direct-acting antivirals,it is becoming increasingly imperative to understand the temporal and geographical trends in HCC mortality associated with Hepatitis ***,gender disparities in HCC mortality related to Hepatitis C are a crucial,yet underexplored aspect that adds to the disease's global *** some studies shed light on gender-specific trends,there is a lack of comprehensive data on global and regional mortality rates,particularly those highlighting gender *** gap in knowledge hinders the development of targeted interventions and resource allocation *** The results of our study show an overall decline in the mortality rates of patients with hepatitis C-related HCC over the last two ***,females exhibited a remarkable decrease in mortality compared to ***,East Asia and the Pacific displayed a significant decline in mortality,while Europe and Central Asia witnessed an upward *** America and the Caribbean also experienced an increase in mortality ***,no significant difference was observed in the Middle East and North *** America exhibited a notable upward *** Asia and Sub-Saharan Africa significantly declined throughout the study *** raises the hope of identifying areas for implementing more targeted *** some progress,multiple challenges remain in meeting the WHO 2030 goal of eliminating viral hepatitis[24].

粮油加工副产物可替代家禽饲料中部分基础日粮,以提高生产性能。非常规饲料的发酵可以提高其养分的利用效率,粮油加工副产物中含有丰富的有机化合物如蛋白质、脂肪酸等。通过益生菌发酵副产物作为家禽饲料,有助于缓解饲料资源短缺,提高养殖企业的经济效益。为此,本试验研究了蛋鸡日粮中添加发酵饲料(Fermented-GPB)对蛋鸡生产性能、蛋品质、血液生化指标及肠道菌群的影响。将468只49周龄海兰褐蛋鸡随机分为对照组(基础日粮)、试验Ⅰ组(4%Fermented-GPB,w/w)、试验Ⅱ组(8%Fermented-GPB,w/w)和抗生素组(黄霉素,50mg/kg),每组三个重复,每个重复39只,试验持续42天,预饲期为5天。分为在试验第21天和第42天二次测定鸡蛋品质,试验结束后,取各组蛋鸡肠道内容物分析肠道菌群结构,同时计算各组蛋鸡的生产性能。结果表明:(1)与对照组相比,Fermented-GPB组的产蛋率和采食量均增加。试验Ⅰ组蛋重(EW)显著高于Ⅱ组和抗生素组(p<0.01),试验Ⅱ组蛋重(EW)显著低于对照组(p<0.05)。与对照组相比,Fermented-GPB组的料蛋比下降(p<0.05),Ⅱ组蛋壳强度(ESS)和蛋形指数(ESI)均显著降低(p<0.05)。与对照组和抗生素组比较,Fermented-GPB组的卵黄颜色(YC)、蛋白高度(AH)和HU均有明显改善(p<0.01)。(2)试验 Ⅰ、Ⅱ组SOD、GSH-Px等抗氧化指标显著升高,MDA含量降低。与Ⅰ组相比,Ⅱ组蛋黄中的甘油三酯(TG)水平显著降低。(3)通过16S rDNA高通量测序比较试验组空肠和盲肠的OTU数量,试验组Ⅱ各组有效序列数量从高到低依次为197、305,试验Ⅰ组为195、156,抗生素组193、1,对照组为192、99。在试验组Ⅱ中,盲肠的有效序列数目从高到低依次为试验Ⅰ组195、789,Ⅱ组195、329,抗生素组为193、140,对照组为191、710。试验组Ⅰ和Ⅱ的序列数据聚类数几乎相同,而对照组最低。在空肠中,对照组的厚壁菌门细菌Proteobacteria和Bacteroidota的丰度相对较高,而其它三组的厚壁菌门细菌、Proteobacteria和放线菌门细菌丰度相对较高。厚壁菌门、Bacteroidota、Proteobacteria和放线菌门在所有四个处理中均为优势菌群。对照组和试验Ⅰ组仅有疣状微生物占优势,而对照组和抗生素组弯曲杆菌门微生物占优势。脱硫杆菌属细菌在试验Ⅱ组和抗生素组为优势菌群。因此,Fermented-GPB对蛋鸡生产性能、蛋品质指标等均无不良影响,对蛋鸡生产性能有一定的促进作用。本试验结果为发酵粮油加工副产物在蛋鸡饲料中的应用提供了实际依据。

Diagnosis and Management of Childhood Tuberculosis: A Cross-Sectional Study of Practices in Gilgit Baltistan, Pakistan

Background: Tuberculosis (TB) is one of the major killer diseases among infectious diseases. The success of TB control depends on the capability of the health care system to detect and accurately manage TB cases. Tuberculosis in children remained a low public health priority with limited epidemiologic studies. Struggles for TB control in children need to be enhanced as children are providing the reservoir for future cases to develop. Objectives: The study evaluated diagnostic and treatment practices related to childhood pulmonary tuberculosis in Gilgit Baltistan (GB), Pakistan. Methods: A descriptive, cross-sectional study based on retrospective record review of childhood Pulmonary Tuberculosis patients registered in the year 2020 with self-administered questionnaire. Results: Data of 557 childhood cases were collected. Most childhood cases were in age group 1 - 4 years (54%) with male predominance. More than 90% were diagnosed and treated at public sector facilities. 99% of the cases were clinically diagnosed with passive case finding. Cough was considered as a symptom of childhood Tuberculosis (TB) by 94% of physicians. Other important features included failure to thrive (13%), contact with a family history of TB (15%), Malnutrition(24%) and respiratory signs (21%). 99% physicians advised chest X-ray, Complete blood count (CBC) (95%) and Erythrocytes sedimentation rate (ESR) (72%) for diagnosis and fewer physicians (2%) used sputum smear microscopy and induced sputum (0.1%). Combining data on dosage, frequency and duration for drugs, 99% of the cases were found receiving correct regimen. The treatment outcomes of the cases were cured 4 (0.8%), treatment completed 551 (99.3%) and died 2 (0.4%) with no lost to follow up. Conclusions: The study highlights inappropriate diagnostic and treatment practices for managing childhood pulmonary TB among physicians in public and private sectors of Gilgit Baltistan. Antioxidant and antihyperlipidemic activity of Costus speciosus against atherogenic diet-induced hyperlipidemia in rabbits

Objective: An extract of Costus speciosus(CSE), a herb widely used in folk medicine, was evaluated for its antioxidant, antihyperlipidemic and ameliorating effects on histopathological changes in atherogenic ***: Twenty-four male rabbits(Oryctolagus cuniculus) were divided into 4 groups. Three groups were fed a diet containing 3% saturated fat and 1.3% cholesterol for 40 d. One of these was sacrificed on the 40 th day and was called the pathogenic(P) group; the other two groups received treatment for another 30 d as follows: one received 0.8 g/(kgád) of CSE and the other was given 0.01 g/(kgád) of simvastatin. The normal group was sacrificed on the 70 th day and used as a ***: CSE showed radical-scavenging ability. Administration of CSE for a 30-day period resulted in a significant decrease in total cholesterol, triacylglycerol, low-density lipoprotein and aspartate aminotransferase compared to the P group, while levels of hemoglobin, packed corpuscular volume and red blood cells were elevated. With respect to studies performed on the heart, a decrease in malondialdehyde and an increase in reduced glutathione were noted. Total protein increased in the liver, heart and aorta after treatment with CSE and also a marked improvement in histopathological parameters was ***: The present findings indicate that the C. speciosus rhizome possesses antiatherogenic and antioxidant properties which may provide protective effects against oxidative stress in atherosclerotic rabbits.

Pharmacological basis for the medicinal use of Michelia champaca in gut, airways and cardiovascular disorders

Objective: To discover the mechanism behind ameliorative effects of Michelia champaca(M. champaca) in gastrointestinal, respiratory and cardiovascular disorders. Methods: Antispasmodic potential was evaluated by trying the M. champaca extract(aqueous:ethanolic) on rabbit aorta, trachea and jejunum in vitro. Isotonic and isometric transducers coupled with Power Lab data acquisition system was used to record the responses of isolated tissues. Results: M. champaca extract relaxed the spontaneous and high K^+(80 mmol/L)-induced contractions of isolated jejunum preparation of rabbit showing a Ca^(2+) channel blocking mechanism. Moreover, extract shifted calcium concentration response curves towards right like standard calcium channel blocker verapamil. In rabbit tracheal preparation, M. champaca relaxed both carbachol(1 μmol/L) and high K^+-induced contractions, likewise verapamil. In rabbit aorta preparation, M. champaca relaxed phenylephrine(1 μmol/L) and high K^+-induced contractions similar to verapamil. Conclusion: M. champaca possesses spasmolytic, airways relaxant and vasodilator actions mediated perhaps due to blocking of Ca^(2+) channels, hence validating its therapeutic usage in diarrhea, asthma and hypertension.

Endometriosis is defined as the presence of endometrial tissue that is located outside the uterine cavity, associated with fibrosis and inflammatory reaction. The most common atypical locations are the gastrointestinal tract, urinary tract, lung, as well as abdominal surgical scars. Its diagnosis is still very difficult, especially when it manifests itself as an acute large bowel obstruction. We report the rare case of a 41-year-old patient diagnosed with acute colonic obstruction following a rectal tumor. She had undergone colonostomy even though colonoscopy biopsies were nonspecific, and a computed tomography (CT) scan was requested. It showed an adnexal heterogenous mass, therefore the patient had a laparotomy and a subtotal hysterectomy with bilateral salpingo-oophorectomy and a biopsy of the recto-colonic hinge. Histologic examination revealed a diagnosis of rectal endometriosis. The purpose of this work is to report a rare case of colonic endometriosis responsible for an acute large bowel obstruction.

Superior and Inferior Gastric Pouch Dilatation Post-Sleeve Surgery: Does Revision Work? A Systematic Review and Case Reports

Longitudinal sleeve gastrectomy (LSG) has become an acceptable option in the management of morbidly obese patients. Complications in the form of pouch dilatation may occur in post-sleeve surgery. However, revision sleeve gastrectomy procedures are effective in correcting such complications. The aim of this study was to systematically review all published cases that reported revision surgeries following the initial sleeve gastrectomy complicated by pouch dilatation. In addition, we are presenting two cases reported from our military/teaching hospital. A systematic literature search was conducted from English-language studies published from 2000 to 2014 from the following databases: PubMed, CINAHL, MEDLINE, EMBASE, Clinical evidence, Dara, Scopus, Web of Sciences, TRIP, Health Technology Database, Cochrane library, and Psych INFO. A total of 5340 articles were identified in the initial search and 5339 articles were excluded based on the exclusion criteria. Only one case study met the inclusion criteria for this systematic review, involving two patients. Also, we are reporting two cases with revision surgery performed for pouch dilatation post-sleeve surgery from our teaching hospital.?There was only one study that reported revision surgery as a management of gastric pouch dilatation post-sleeve surgery. Authors believe that the incidence of revision surgery for the management of pouch dilatation post-sleeve surgery is higher but underreported. This assumption may be valuable, especially because some surgeons consider pouch dilatation post-sleeve surgery a complication that arises due to surgical technique rather than the procedure itself;therefore, it is unreported.
