Background: Emergence agitation (EA) is a common phenomenon observed in pediatric patients following general anesthesia. This study aimed to assess the efficacy of propofol and fentanyl in preventing EA and to compare their associated complications or side effects. Methods: This prospective randomized observational comparative study was conducted at Dhaka Medical College Hospital from July 2013 to June 2014. The study aimed to evaluate the effects of propofol and fentanyl on EA in children aged 18 to 72 months undergoing circumcision, herniotomy, and polypectomy operations. Ninety children were included in the study, with 45 in each group. Patients with psychological or neurological disorders were excluded. Various parameters including age, sex, weight, American Society of Anesthesiologists (ASA) class, duration of anesthesia, Saturation of Peripheral Oxygen (SPO2), heart rate (HR), respiratory rate (RR), Pediatric Anesthesia Emergence Delirium (PAED) score, duration of post-anesthesia care unit (PACU) stay, incidence of laryngospasm, nausea, vomiting, and rescue drug requirement were compared between the two groups. Results: Age, sex, weight, ASA class, and duration of anesthesia were comparable between the two groups. Perioperative SpO2 and HR were similar in both groups. However, the PAED score was significantly higher in the fentanyl group during all follow-ups except at 30 minutes postoperatively. The mean duration of PACU stay was significantly longer in the fentanyl group. Although the incidence of laryngospasm was higher in the fentanyl group, it was not statistically significant. Conversely, nausea or vomiting was significantly higher in the fentanyl group. The requirement for rescue drugs was significantly higher in the fentanyl group compared to the propofol group. Conclusion: Both propofol and fentanyl were effective in preventing emergence agitation in pediatric patients undergoing various surgical procedures under sevoflurane anesthesia. However, propo

Rheumatoid Arthritis (RA) is a chronic autoimmune disorder that is usually manifested as inflammation in multiple joints and several extra-articular symptoms, involving the liver, kidney, eye, skin, blood, blood vessels, heart, lungs, nervous system, and other organs. Methotrexate (MTX) is the anchor drug that treats RA. As renal and liver abnormalities are more common during disease conditions as well as during the treatment period, we tried to find out if there is any impact of MTX in these organs during the treatment of RA patients. Once the disease complications are developed, it is quite difficult to reverse the disease, and treatment in this situation is not very effective. Consequently, patients suffer a lot. So, early evaluation of renal and liver function is essential for the treatment of RA patients and it might also help prevent different complications which are usually very frequently observed. This was a cross-sectional study. A total of 150 RA patients treated with MTX were evaluated for the study where female and male respondents were 115 and 35 respectively. In this study, we found that 82% of RA patients had creatinine levels ≤ 1.1 mg/dL although the normal range of serum creatinine is below 1.4 mg/dL. Usually, a 15% increase in Serum creatinine level from the baseline is considered renal impairment. We found 4% of such cases. Moreover, 2% of RA patients had creatinine levels above the normal range of 1.4 mg/dL and those patients were hypertensive as well. So, a total (4 + 2 = 6)% had renal impairments. Among them, 5% had diabetes mellitus. On the other hand, the ultrasonogram (USG) of RA patients with kidney disease showed signs of renal parenchymal disease and 3% of RA patients having renal problems whose serum creatinine level was within the normal range showed signs of chronic kidney disease (CKD). In irrigated agricultural systems,nitrogen(N)and water are the vital resources for sustainability of the crop production in the modern era of climate *** current study aimed to assess the impact of water and N management on the productivity of irrigated rice *** the context,a field observation was done at the research farm of Bangladesh Agricultural University,Mymensingh,during dry seasons in consecutive two years(2018–2019 and 2019–2020).The experiments were set up following split-plot design assigning water management in the main plots,nitrogen management in the sub-plots,and the cultivars were approved in the split-split plot with three *** two years observation,it was revealed that rice cultivar Binadhan-8 gave the maximum value of leaf area index,number effective tillers hill-1 and grains panicle-1 which lead to the higher grain yield(GY).Substantial relationships were observed among the concentration of N,growth,total dry matter(TDM)and N content,N uptake,N utilization effectiveness,and ***,with little exception,the Combined effect of water and N,cultivars and water management were varied significantly for all ***,the results of the current study concluded that application of irrigation at 8 days after the disappearance of ponded water and source of 105 kg N ha-1 from PU+Poultry manure are the best management approach for the excellent performance of rice cultivar Binadhan-8.

Background: Standard precautions (SPs) are the minimum infection prevention practices that aim to protect Health care workers (HCWs) including physicians and prevent them from transmitting the infections to their patients. Purpose: To assess the level of compliance of physicians with standard precautions of handling patients with infectious respiratory disease. Method: A cross sectional questionnaire-based study was conducted in two tertiary level hospitals named M abdur Rahim Medical College and Hospital, Dinajpur, Bangladesh and Rajshahi Medical College and Hospital, Rajshahi, Bangladesh to assess the level of compliance of physicians with standard precautions. Purposive sampling technique was applied as per inclusion criteria and data was collected by face to face interview from 285 physicians. Statistical analysis of the results was done by SPSS and a p value less than 0.05 was considered as significant. Result: The study revealed that maximum physicians (76.5%) handling patients with infectious respiratory disease had a moderate level of compliance with standard precautions. 78.6% of the physicians had moderate level of awareness about standard precautions. Only 20.35% of the physicians had training on infection control. 94.4% of the respondents didn’t know the component of standard precaution and most of the physicians were not aware of the sequence of wearing and removing the different components of PPEs. Conclusion: The study result implies that with increased awareness, the compliance of the physicians with standard precautions increased. The main reasons of non-compliance with standard precautions were found to be lack of resources, lack of regular training and excess workload.

Geochemistry of subsurface Late Quaternary ironstones in Rajshahi and Bogra Districts, Bangladesh: implications for genetic and depositional conditions

The present study deals with the geochemistry of Late Quaternary ironstones in the subsurface in Rajshahi and Bogra districts, Bangladesh with the lithological study of the boreholes sediments. Major lithofacies of the studied boreholes are clay, silty clay, sandy clay, fine to coarse grained sand, gravels and sands with(fragmentary) ironstones. The ironstones contain major oxides, Fe_2 O_3*(*total Fe)(avg. 66.6 wt%), SiO_2(avg. 15.3 wt%), Al_2 O_3(avg. 4.0 wt%), MnO(avg. 7.7 wt%), and CaO(avg. 3.4 wt%). These geochemical data imply that the higher percentage of Fe_2 O_3* along with Al_2 O_3 and MnO indicate the ironstone as goethite and siderite, which is also validated by XRD data. A comparatively higher percentage of SiO_2 indicates the presence of relative amounts of clastic quartz and manganese-rich silicate or clay in these rocks. These ironstones also have significant amounts of MnO(avg. 7.7 wt%) suggesting their depositional environments under oxygenated condition. Chemical data of these ironstones suggest that the source rock suffered deep chemical weathering and iron was mostly carried in association with the clay fraction and organic matter. Iron concretion was mostly formed by bacterial build up in swamps and marshes, and was subsequently embedded in clayey *** the coastal environments, the water table fluctuates and goethite and siderite with mud and quartz became dry and compacted to form ironstone.

This study mainly focuses on exploring the regional variation of the changing patterns of temperature and rainfall in Bangladesh. The analysis is based on the temperature and rainfall variation in Bangladesh over five regions as Dhaka, Cox’s Bazar, Rajshahi, Bogra and Sylhet. The duration of the study period was chosen as 1953-2012 for Dhaka, 1948-2012 for Cox’s Bazar, 1972-2012 for Rajshahi, 1958-2012 for Bogra and 1957-2012 for Sylhet. The findings of the non-parametric Mann-Kendal test reveals that significant increase of maximum temperature has been found in Cox’s Bazar and Sylhet. Significant decrease of maximum temperature has been found in Dhaka and Bogra. Significant increase of minimum temperature has been found in Dhaka and Cox’s Bazar whereas significant decrease has been found in Rajshahi. Significant decrease of rainfall has been found in Rajshahi among the study region. The maximum temperature increases significantly by 0.021 degree Celsius per year in Cox’s Bazar and Sylhet. In case of minimum temperature highest increase is found in Dhaka by 0.049 degree Celsius followed by Cox’s Bazar (0.038 degree Celsius per year) whereas significant decrease has been found in Rajshahi by 0.047 degree Celsius per year.

In the present paper, different Autoregressive Integrated Moving Average (ARIMA) models were developed to model the carbon dioxide emission by using time series data of forty-four years from 1972-2015. The performance of these developed models was assessed with the help of different selection measure criteria and the model having minimum value of these criteria considered as the best forecasting model. Based on findings, it has been observed that out of different ARIMA models, ARIMA (0, 2, 1) is the best fitted model in predicting the emission of carbon dioxide in Bangladesh. Using this best fitted model, the forecasted value of carbon dioxide emission in Bangladesh, for the year 2016, 2017 and 2018 as obtained from ARIMA (0, 2, 1) was obtained as 83.94657 Metric Tons, 89.90464 Metric Tons and 96.28557 Metric Tons respectively.

The COOLOD-N2 and PARET computer codes were used for a steady-state thermal hydraulic and safety analysis of the 3 MW TRIGA Mark-II research reactor located at Atomic Energy Research Establishment (AERE), Savar, Dhaka, Bangladesh. The objective of the present study is to ensure that all important safety related thermal hydraulic parameters uphold margins far below the safety limits by steady-state calculations at full power. We, therefore, have calculated the hot channel fuel centreline temperature, fuel surface temperature, cladding surface temperature, the departure from nucleate boiling (DNB) heat flux and DNB ratio, axial fuel centreline temperature and compared. The comparison indicates that the calculated values are in satisfactory agreement between the codes. The data obtained in this investigation are largely far to compromise safety of the reactor. The results can also be used to upgrade the current core configuration of the TRIGA reactor.
