Outcome of non-variceal acute upper gastrointestinal bleeding in relation to the time of endoscopy and the experience of the endoscopist: A two-year survey

AIM: To prospectively assess the impact of time of endoscopy and endoscopist's experience on the outcome of non-variceal acute upper gastrointestinal (GI) bleeding patients in a large teaching ***: All patients admitted for non-variceal acute upper GI bleeding for over a 2-year period were potentially eligible for this study. They were managed by a team of seven endoscopists on 24-h call whose experience was categorized into two levels (high and low) according to the number of endoscopic hemostatic procedures undertaken before the study. Endoscopic treatment was standardized according to Forrest classification of lesions as well as the subsequent medical therapy. Time of endoscopy was subdivided into two time periods: routine (8 a.m.-5 p.m.) and on-call (5 p.m.-8 a.m.). For each category of experience and time periods rebleeding rate, transfusion requirement, need for surgery, length of hospital stay and mortality we compared. Multivariate analysis was used to discriminate the impact of different variables on the outcomes that were ***: Study population consisted of 272 patients (mean age 67.3 years) with endoscopic stigmata of hemorrhage. The patients were equally distributed among the endoscopists, whereas only 19% of procedures were done out of working hours. Rockall score and Forrest classification at admission did not differ between time periods and degree of *** analysis showed that higher endoscopist's experience was associated with significant reduction in rebleeding rate (14% vs 37%), transfusion requirements (1.8±0.6 vs 3.0±1.7 units) as well as surgery (4% vs 10%), but not associated with the length of hospital stay nor mortality. A novel distributed control architecture for unmanned aircraft system (UASs) based on thenew Robot Operating System (ROS) 2 middleware is proposed, endowed with industrialgradetools that establish a novel standard for high-reliability distributed systems. Thearchitecture has been developed for an autonomous quadcopter to design an inclusivesolution ranging from low-level sensor management and soft real-time operating systemsetup and tuning to perception, exploration, and navigation modules orchestrated by afinite-state machine. The architecture proposed in this study builds on ROS 2 with itsscalability and soft real-time communication functionalities, while including security andsafety features, optimised implementations of localisation algorithms, and integrating aninnovative and flexible path planner for UASs. Finally, experimental results have beencollected during tests carried out both in the laboratory and in a realistic environment,showing the effectiveness of the proposed architecture in terms of reliability, scalability, andflexibility.

In this study the authors compared the sequence types （STs） designed by sequence based typing （SBT） of 4 clinical and 12 environmental strains of Legionella pneumophila serogroup 1 （LP1） which were isolated from hospital facilities for the mentally disabled. The strains were selected after a retrospective surveillance of 565 clinical records （2002-2009） and investigations of water circuit. It was possible to correlate two clinical strains with the corresponding environment, which were collected from showers that had exposed the patients （ST685, ST16） and two clinical strains present in the same structure （STI）. The other environmental strains were isolated from water in the department with confirmed or suspected clinical cases. All the strains （seven） from the first structure had ST188; two from the second structure had ST34; and the last from the third structure, gave an ST694. The results were compared with the European Working Group for Legionella Infections （EWGLI） database： the ST 1, 16, 34 and 188 were already known in literature, among clinical and nosocomial cases, especially for ST 1, the most distributed worldwide. Two STs were new to the database. ST685 was isolated both from a patient and from the water; ST694, which was found exclusively in the environmental compartment of a control structure （no cases of legionellosis and low number of nosocomial pneumonia）, was unknown in the literature and the authors could only speculate on its possible minor virulence and/or distribution. The implementation of SBT and international comparisons may be useful to gain genotypic knowledge of circulating environmental strains, also verifying their presence in the clinical setting.

The laparoscopic approach for treatment of rectal cancer has been proven feasible and oncologically safe, and is able to offer better short-term outcomes than traditional open procedures, mainly in terms of reduced length of hospital stay and time to return to working activity. In spite of this, the laparoscopic technique is usually practised only in high-volume experienced centres, mainly because it requires a prolonged and demanding learning curve. It has been estimated that over 50 operations are required for an experienced colorectal surgeon to achieve proficiency with this technique. Robotic surgery enables the surgeon to perform minimally invasive operations with better vision and more intuitive and precise control of the operating instruments, thus promising to overcome some of the technical difficulties associated with standard laparoscopy. It has high-definition threedimensional vision, it translates the surgeon's hand movements into precise movements of the instruments inside the patient, the camera is held and moved by the first surgeon, and a fourth robotic arm is available as a fixed retractor. The aim of this review is to summarise the current data on clinical and oncologic outcomes of robot-assisted surgery in rectal cancer, focusing on short- and long-term results, and providing original data from the authors' centre.

AIM To assess the impact of short infliximab(IFX) infusion on hospital resource utilization and *** All inflammatory bowel diseases(IBD) patients who received IFX 1 h infusion from March 2007 to September 2014 in eight centers from Southern Italy were included in the analysis. Demographic, clinical and infusion related data were collected. The potential benefits related to the short infusion protocol were assessed both in terms of time saving and increased infusion unit capacity. In addition, indirect patient-related cost savings were *** One hundred and twenty-five patients were recruited(64 with ulcerative colitis and 61 with Crohn's disease). Median duration of disease was of 53 mo and mean age of pts at diagnosis was of 34 years(SD: ± 13). Adverse infusion reactions were reported in less than 4% both before and after short infusion. The total number of infusions across the selected centers was of 2501(30.5% short infusions). In the analyzed cohort, 1143 h were saved(762 in the infusion and 381 in observation phases) through the rapid IFX infusion protocol. This time saving(-15% compared to the standard protocol in infusion phase) represents, from the hospital perspective, an opportunity to optimize infusion unit capacity by allocating the saved time in alternative cost-effective treatments. This is the case of opportunity cost that represents the value of forgone benefit which could be obtained from a resource in its next-best alternative use. Hence, an extra hour of infusion in the case of standard 2-h IFX represents a loss in opportunity to provide other cost effective services. The analysis showed that the short infusion increased the infusion units capacity up to 50% on days when the IFX infusions were scheduled(infusion phase). Furthermore, the analysis showed that the short IFX infusion protocol leads to time savings also in the post-infusion phase(observation) leading to a time saving of 10% on average among the analyzed centers. In this paper we describe the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to accretion flows in the strong field gravity regime around both stellar-mass and supermassive black-holes. eXTP has the unique capability of using advanced "spectral-timing-polarimetry" techniques to analyze the rapid variations with three orthogonal diagnostics of the flow and its geometry, yielding unprecedented insight into the inner accreting regions, the effects of strong field gravity on the material within them and the powerful outflows which are driven by the accretion process.
