Thoracoscopy in Cameroon: Indications, Technics and Short Term Results

Introduction: Thoracoscopy is a video-assisted surgical approach that mirrors the techniques used in thoracotomy. Our aim is to map the current state of thoracoscopy practice in Cameroon’s hospitals. Methods: This was a descriptive study that collected both retrospective and prospective data over 57 months across four hospitals in Cameroon. It included 13 patients and focused on variables such as socio-demographic factors, clinical profiles, surgical procedures, and postoperative follow-up. Results: Thirteen patients, predominantly male (84.6%, n = 11), with a mean age of 37.5 ± 16 years, were enrolled. Alcohol use (61.5%) and smoking (38.5%) were the most common past histories. The major complaints were dyspnea (84.6%) and chest pain (58.3%). The primary surgical procedure was clot-free thoracoscopy in 30.8% of cases, mainly for persistent hemothorax (41.8%). Most surgeries were elective (76.9%) and performed under general anesthesia with selective intubation (61.5%). The most common approach was single-port thoracoscopy (U-VATS) (76.9%), with no reported difficulties;however, one intraoperative incident occurred and was successfully treated. Drainage was performed systematically in all patients, and one case required conversion to open surgery. Postoperative complications were minor (Clavien-Dindo grade I) and mainly consisted of pain, with a mortality rate of 15.4%. No significant association was found between risk factors and the occurrence of complications or postoperative mortality. Conclusion: Thoracoscopy, a novel approach in our context, primarily focuses on minor thoracic surgeries.

Background: Costal fracture surgical is still a debate, therefore we shall select between early and delay surgical management. Case Report: We are reporting two cases of post road traffic clash delay ribs fractures osteosynthesis involving a 63-year-old man with multistage fractures on the left and pulmonary pinning of one of the costal arches, complicated by a homolateral haemothorax and a 41-year-old man with a bilateral flail chest. Conclusion: The simple postoperative course and the immediate postoperative improvement in the patient’s clinical respiratory condition enabled us to discuss the time frame for management, in this case the indication for early or later surgery.

Abiotic Factors Associated with Abundance Dynamics and Antibiotic Multidrug Resistance of Escherichia coli and Enterococcus faecalis Isolated from Some Ombessa Aquatic Systems (Central Cameroon Region)

The present study conducted in the town of Ombessa aims to assess the influence of abiotic factors on the abundance dynamics and antibiotic susceptibility of Escherichia coli and Enterococcus faecalis isolated from some aquatic systems from February to July 2022, monthly samples were taken at 10 water points used by the population (8 groundwater points and 2 surface water points). Samples were analyzed for physico-chemical parameters such as temperature, pH, electrical conductivity. Bacteriological variables such as BHAM, E. coli and E. faecalis abundances were also assessed. Antibiotic susceptibility of E. coli and E. faecalis was assessed using 06 antibiotics using the Kirby-Bauer diffusion disk method. The results show that bacterial abundances were the highest in surface waters. Bacterial densities were the highest in May and the lowest in February. The average densities recorded were 3845 CFU/100mL for BHAM, 380 CFU/100mL for E. coli and 14 CFU/100mL for E. faecalis in groundwater;and 8583 CFU/100mL for BHAM, 6878 CFU/100mL for E. coli and 812 CFU/100mL for E. faecalis in surface water. Antibiotic susceptibility tests showed that these bacterial species are sensitive to Gentamicin, Chloramphenicol, Azithromycin and Ciprofloxacin. They are all resistant to Trimethoprim/Sulfamethoxazole, E. coli is resistant to Doxycycline and E. faecalis has an intermediate sensitivity to Gentamicin. Overall, the Multiresistance to Antibiotics (MRA) indices obtained were above 0.2, indicating the presence of multidrug resistance in bacterial communities. The physico-chemical properties of the water varied over time and space, but on the whole remained below the threshold values of WHO guidelines. The degree of linkage between abiotic water variables and bacteriological parameters has shown that bacterial densities are more abundant in rainy seasons and increased O2 levels favor bacterial growth, while TSS, CO2 and dissolved nitrate levels affect the sensitivity of these bacteria

Dissemination of Resistance Integrons and Genes Coding for Blse and Cabapenemases in the Urban Drainage Network in Cote d’Ivoire

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DNA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEM was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKPC 17.94%, blaNDM 30.76% and blaOXA48 38.46% were detected in the samples.

Chromium-incorporated mesoporous silica materials （MCrS） have been successfully prepared in strong acidic media by assembly of preformed CrSI precursors with triblock copolymer of the Pluronic type （P123） by a two steps procedure. Various techniques including XRD, N2 adsorption, FT-IR spectroscopy, diffuse reflectance UV-vis （DRUV-vis） and EPR were employed for the materials characterization. MCrS materials have a hexagonal structure with uniform mesopores and contain Cr（V） and Cr（VI） under low and high Cr content and that the mesoporous walls contain the MFI structure building units.

Two novel Schiff bases, 3-[1-(2-(phthalazin-1-yl)hydrazono)ethyl)-1,3-oxa- zinane (PHEO) and 2-[(2-(phthalazin-1-yl)hydrazono)methyl]phenol (PHMP), derived from hydralazine hydrochloride, an effective drug against hypertension, were synthesized and characterized by spectroscopic methods, Infrared (IR), Proton Nuclear Magnetic Resonance (1H NMR) and Carbon-13 Nuclear Magnetic Resonance (13C NMR). PHEO showed low antimicrobial activity against one bacterial strain with MIC value of 250 μg/ml while PHMP showed interesting activity against 4 bacterial strains with MIC of 31.25 - 250 μg/ml compared to the standard drug, amoxicillin. PHEO and PHMP showed higher reducing activity on ferric ions compared to Vitamin C. On lipid peroxidation, PHEO showed higher inhibition while PHMP showed lower inhibition compared to Vitamin C. Both compounds presented lower stimulating effect and lower catalase activity compared to the standard Vitamin C. PHEO and PHMP showed less than 80% inhibition in the preliminary antiplasmodial assay and so were not considered for the dose-response studies.
