Large Conventional Osteosarcoma of the Proximal Humerus in a 13-Year-Old Child: Case Report

Introduction: Osteosarcoma is the most common primary malignant bone tumor in children. It is highly aggressive and has a poor prognosis. A late presentation modifies and makes difficult the management affecting the survival of children. We report the case of a large conventional osteosarcoma in a 13-year-old girl. Case Presentation: Adolescent girl admitted for painful swelling of the left shoulder with absolute functional impotence of the thoracic limb and severe anemia. the painful swelling was thought to have been caused by a minor trauma that had occurred six months previously. the patient’s general condition was poor, and she presented with a large, shiny, painful mass over the shoulder and upper 2/3 of the left arm, measuring 28 cm long by 28 cm wide and 57 cm in circumference, and a large fistulous axillary adenopathy. Ct scan showed a tumour lesion of the left humerus with liver and lung metastases, raising suspicion of osteogenic osteosarcoma. the tumor was classified according to tNM staging: t2N1M1(a + b). Management was modified when uncontrolled bleeding developed. It consisted of an extended amputation of the left thoracic limb. Pathological analysis showed a high-grade conventional osteosarcoma. Quality improvement was obtained for thirty days, followed by the onset of dyspnea. the evolution was towards death at forty days post-operatively. Conclusion: Osteosarcoma is a highly aggressive cancer. Delayed treatment leads to a fatal outcome. Early diagnosis is one of the challenges to be met in order to improve survival.

Metabolic reprogramming in inflammaging and aging in t cells

Aging represents an emerging challenge for public health due to the declined immune responses against pathogens, weakened vaccination efficacy, and disturbed tissue homeostasis. Metabolic alterations in cellular and systemic levels are also known to be cardinal features of aging. Moreover, cellular metabolism has emerged to provide regulations to guide immune cell behavior via modulations on signaling cascades and epigenetic landscape, and the aberrant aging process in immune cells can lead to inflammaging, a chronic and low-grade inflammation that facilitates aging by perturbing homeostasis in tissues and organs. Here, we review how the metabolic program in t cells is influenced by the aging process and how aged t cells modulate inflammaging. In addition, we discuss the potential approaches to reverse or ameliorate aging by rewiring the metabolic programming of immune cells.

Reflecting petawatt lasers off relativistic plasma mirrors:a realistic path to the Schwinger limit

the quantum vacuum plays a central role in physics. Quantum electrodynamics(QED) predicts that the properties of the fermionic quantum vacuum can be probed by extremely large electromagnetic fields. the typical field amplitudes required correspond to the onset of the ‘optical breakdown’ of this vacuum, expected at light intensities>4.7×10^(29) W/cm^(2). Approaching this ‘Schwinger limit’ would enable testing of major but still unverified predictions of power lasers. to close this considerable gap, a promising paradigm consists of reflecting these laser beams off a mirror in relativistic motion, to induce a Doppler effect that compresses the light pulse in time down to the attosecond range and converts it to shorter wavelengths, which can then be focused much more tightly than the initial laser light. However, this faces a major experimental hurdle: how to generate such relativistic mirrors? In this article, we explain how this challenge could nowadays be tackled by using so-called ‘relativistic plasma mirrors’. We argue that approaching the Schwinger limit in the coming years by applying this scheme to the latest generation of petawatt-class lasers is a challenging but realistic objective.

Color polymorphisms are widely studied to identify the mechanisms responsible for the origin and maintenance of phenotypic variability in *** of the mechanisms of balancing selection currently thought to explain the long-term persistence of polymorphisms are the evolution of alternative phenotypic optima through correlational selection on suites of traits including color and *** of these mechanisms can generate differences in offspring viability and fitness arising from different morph ***,we examined the effect of parental morph combination on fertilization success,embryonic viability,newborn quality,antipredator,and foraging behavior,as well as inter-annual survival by conducting controlled matings in a polymorphic lacertid Podarcis muralis,where color morphs are frequently assumed to reflect alternative phenotypic optima(e.g.,alternative reproductive strategies).Juveniles were kept in outdoor tubs for a year in order to study inter-annual growth,survival,and morph *** agreement with a previous genome-wide association analysis,morph frequencies in the year-old juveniles matched the frequencies expected if orange and yellow expressions depended on recessive homozygosity at 2 separate *** findings also agree with previous literature reporting higher reproductive output of heavy females and the higher overall viability of heavy newborn lizards,but we found no evidence for the existence of alternative breeding investment strategies in female morphs,or morphcombination effects on offspring viability and *** conclude that inter-morph breeding remains entirely viable and genetic incompatibilities are of little significance for the maintenance of discrete color morphs in *** from the Pyrenees.

We conducted a large-scale density-functional theory study on the influence of the exchange-correlation functional in the calculation of electronic band gaps of ***,we use the large materials data set that we have recently proposed to benchmark 21 different functionals,with a particular focus on approximations of the meta-generalized-gradient *** these data with the results for 12 functionals in our previous work,we can analyze in detail the characteristics of each approximation and identify its strong and/or weak *** confirming that mBJ,HLE16 and HSE06 are the most accurate functionals for band gap calculations,we reveal several other interesting functionals,chief among which are the local Slater potential approximation,the GGA AK13LDA,and the meta-GGAs HLE17 and *** also compare the computational efficiency of these different *** on these data,we investigate the potential for improvement of a promising subset of functionals by varying their internal *** identified optimal parameters yield a family of functionals fitted for the calculation of band ***,we demonstrate how to train machine learning models for accurate band gap prediction,using as input structural and composition data,as well as approximate band gaps obtained from density-functional theory.

Multidisciplinary management of Mirizzi syndrome with cholecystobiliary fistula: the value of minimally invasive endoscopic surgery

Mirizzi syndrome, a rare complication of gallstones, is defined by obstruction of the main bile duct. this obstruction may worsen and thus result in cholecystobiliary fistula. Surgical management of Mirizzi syndrome is complicated by the presence of inflamed tissue around the hepatic pedicle, making it impossible to distinguish between the main bile duct and the gallbladder. the surgeon＇s first task is to perform subtotal cholecystotomy（from the fundus of the gallbladder to the neck） without trying to locate the cystic duct. In a second step, the gallstones are extracted and the main bile duct is then repaired. In most cases, a t-tube is used to drain the main bile duct, and abdominal drainage is left in place（in case a bile fistula forms）. this study concluded that preoperative drainage of the main bile duct in the treatment of Mirizzi syndrome types II and III is feasible and might help to decrease the postoperative complication rate.

Polypeptide analysis for nanopore-based protein identification

Presently,proteins are identified by cleaving them with proteases,measuring the mass to charge ratio of the fragments with a mass spectrometer,and matching the fragments to segments within known proteins in *** earlier demonstrated that a nanometer-scale pore formed by aerolysin(AeL)can discriminate between,and therefore identify,three similar size proteins from their trypsin-cleaved polypeptide *** this nanopore-protease method,the protein’s identity is instead determined from characteristic ionic current blockade patterns caused by the polypeptide fragments that enter the *** results also suggested that not all of the theoretically expected cleavage products partition into the *** better understand the mechanism by which polypeptide fragments are captured,and how different polypeptides reduce the pore’s ionic current,we studied the effects of 11 identical length polypeptides with different net charges and charge *** show that under certain experimental conditions,negative,positive,and neutral polypeptides are driven into the AeL pore by the same applied voltage *** capture rate and dwell time of polypeptides in the pore depend strongly on the ionic strength,the magnitude of the applied voltage,and the net charge and charge distribution of the *** dwell time distribution depends nonmonotonically on the applied voltage(regardless of the polymer’s net charge),and its maximum value depends on the polypeptide net charge and charge *** maximum dwell time for different polypeptides does not occur at the same applied voltage amplitude,which conceivably might complicate the detection and discrimination of some polypeptide *** additional experiments,computer simulations,and artificial intelligence research are needed to better understand how to optimize the partitioning of enzymatically cleaved fragments into the AeL nanopore,the method is still capable of accurately identifying

Characterization of the Activity, Mineralogy and Correlations between the Properties of Clayey Soils

the activity is related to the mineralogy and geological history of clays. Soils with the same value of the liquidity limit or plasticity index can have very different characteristics depending on the amount and type of clay minerals. the methylene blue value characterizes the activity of the clays and reflects the surface activity. ten inactive soils contain minerals (kaolinite, illite), these soils absorb little water. two swelling soils have normal activity and are composed of minerals (kaolinite, illite, Montmorillonite). the relationships between clayey soils properties, their activities and between the activity and the liquidity limit are defined. the correlations obtained are linear fit and exponential and sigmoidal fits. the correlations obtained with a coefficient of determination of R2 (0.859 - 0.999) can be used to characterize and predict certain parameters of fine-grained soils as a function of clay content.

Memory CD8 t cells can provide long-term protection against tumors,which depends on their enhanced proliferative capacity,self-renewal and unique metabolic rewiring to sustain cellular ***,memory CD8 t cells engage oxidative phosphorylation and fatty acid oxidation to fulfill their metabolic *** contrast,tumor-infiltrating lymphocytes(tILs)display severe metabolic defects,which may underlie their functional ***,we show that overexpression of proliferator-activated receptor gamma coactivator 1-alpha(PGC-1α),the master regulator of mitochondrial biogenesis(MB),favors CD8 t cell central memory formation rather than resident memory ***-1α-overexpressing CD8 t cells persist and mediate more robust recall responses to bacterial infection or peptide ***,CD8 t cells with enhanced PGC-1αexpression provide stronger antitumor immunity in a mouse melanoma ***,tILs overexpressing PGC-1αmaintain higher mitochondrial activity and improved expansion when rechallenged in a tumor-free ***,our findings indicate that enforcing mitochondrial biogenesis promotes CD8 t cell memory formation,metabolic fitness,and antitumor immunity in vivo.
