Obesity and its influence on liver dysfunction,morbidity and mortality after liver resection

Background:Obesity and associated steatosis is an increasing health problem *** influence on post-hepatectomy liver failure(PHLF)and after liver resection(LR)is still ***:Patients who underwent LR were investigated and divided into three groups[normal weight:body mass index(BMI)18.5-24.9 kg/m2,overweight:BMI 25.0-29.9 kg/m2,obese:BMI≥30 kg/m2]in this retrospective *** aim of this study was to assess the influence of BMI and nonalcoholic steatohepatitis(NASH)on PHLF and ***:Of 888 included patients,361(40.7%)had normal weight,360(40.5%)were overweight,167(18.8%)were *** age was 62.5 years(IQR,54-69 years).The primary indication for LR was colorectal liver metastases(CLM)(n=366,41.2%).NASH was present in 58(16.1%)of normal weight,84(23.3%)of overweight and 69(41.3%)of obese patients(P<0.001).PHLF occurred in 16.3%in normal weight,15.3%in overweight and 11.4%in obese patients(P=0.32).NASH was not associated with *** was no association between patients’weight and the occurrence of postoperative complications(P=0.45).At multivariable analysis,solely major LR[odds ratio(OR):2.7,95%confidence interval(CI):1.83-4.04;P<0.001]remained a significant predictor for ***:Postoperative complications and PHLF are comparable in normal weight,overweight and obese patients and LRs using modern techniques can be safely performed in these patients.

The interplay between glucose metabolism and that of the two other primary nutrient classes, amino acids and fatty acids is critical for regulated insulin secretion. Mitochondrial metabolism of glucose, amino acid and fatty acids generates metabolic coupling factors(such as ATP, NADPH, glutamate, long chain acyl-CoA and diacylglycerol) which trigger insulin secretion. The observation of protein induced hypoglycaemia in patients with mutations in GLUD1 gene, encoding the enzyme glutamate dehydrogenase(GDH) and HADH gene, encoding for the enzyme short-chain 3-hydroxyacyl-CoA dehydrogenase has provided new mechanistic insights into the regulation of insulin secretion by amino acid and fatty acid metabolism. Metabolic signals arising from amino acid and fatty acid metabolism converge on the enzyme GDH which integrates both signals from both pathways and controls insulin secretion. Hence GDH seems to play a pivotal role in regulating both amino acid and fatty acid metabolism.

Dear editor,Dysregulated inflammatory responses primarily account for damage to the central nervous system,however,neurodegenerative processes likely contribute to the pathophysiology of progressive forms of multiple sclerosis(MS).These pathophysiological aspects include axonal energy depletion and impaired remyelination capacity(Levin et al.,2014).The clinical efficacy of most disease-modifying therapies in MS to date has been achieved by targeting functions of the adaptive immune system(Cree et al.,2019;Szepanowski et al.,2021).

Delphi-Based Planning Approach in the Maritime Transport Chain

The objective of this paper is to illustrate behavioural aspects and a collaborative Delphi-based planning approach in the maritime transport chain. Therefore, this paper analyses changes in behavioural routines and governance structures in the maritime transport chain using a case study that depicts mutual projections of expected container transhipment moderated by a neutral third for reducing the uncertainty in planning within the whole maritime transport chain. The actors＇ different views on container transport are collected and directly confronted to an expert panel showing bounded rationality and thus asymmetric information can be circumvented The planning approach shows how actors＇ different internal planning can be improved and evident to the market. The whole maritime transport chain expects the future development in container transport which can be given by using collaborative Delphi-based planning. As a result, an overall picture for planning issues can be constituted, improving both individual planning of the actors and expected market growth.

The Presence of an Exercise Instructor or Group Members Affects Physical Self-Concept and Physical Self-Esteem: An Experimental Study in the Elderly

Background: Physical self-concept and physical self-esteem are important sources of general self-esteem and overall health. Evidence indicates that exercise leads to an enhancement in physical self-concept and physical self-esteem. Until now however, it remained unclear whether the beneficial effects of exercise were impacted by different types of exercise environments. Thus, the purpose of this study was to experimentally investigate, whether the presence or absence of an instructor or other group members influenced elder adults’ physical self-concept and esteem. Method: Participants were 46 women and 20 men (Mean age 65.4, SD = 4.36) who were randomly assigned to one of four experimental groups differing across two factors—group versus no-group and instructor versus no-instructor. Participants took part in an 8-week-walking program with a comparable number of training sessions across the four experimental groups. Physical self-concept and physical self-esteem were assessed both pre and post exercise intervention. Results: Descriptively, the highest improvement in physical self-concept and esteem were observed in the instructor/no-group and the no-instructor/group condition. Between these two conditions, no significant difference of change was observed. Moreover, no improvement was observed when both instructor and group were present. Conclusion: Effects of either instructor presence or group presence may be explained by mechanisms of social integration and social support. An unexpected absence of effects on self-concept and esteem under the instructor/group condition are explained with reference to an individual’s cognitive dissonance caused by the need to adapt to different exercise oriented goals or attitudes of the instructor and the group members which might hinder the positive development of the self.

Affective exercise experience as an emerging theoretical concept has great potential to provide a more nuanced understanding of individual factors that influence exercise ***,concerning the Affective Exercise Experiences(AFFEXX)questionnaire,it has not been examined yet whether the structural score of the AFFEXX is a useful index to predict physical activity(refers to any bodily movement produced by skeletal muscles that requires energy expenditure).Furthermore,there is currently a gap in knowledge regarding the psychological mechanisms that can explain the relationship between affective exercise experiences and the level of physical activity(PA).In order to adress these gaps in the literature,we conducted two studies among Chinese collge students that aimed(i)to investigated whether the total score of the three AFFEXX-C constructs(antecedent appraisals,core affective exercise experiences,and attraction-antipathy towards exercise)is a relaible indicator that can be utilized in research and pratical settings and(ii)to evalute the specific psychological mechanisms that can explain the relationship between affective exercise experience and *** Study 1,we recruited 801 voluntary Chinese college students for bifactor and correlational *** Study 2,875 Chinese college students were enrolled to verify our findings from Study 1 and to explore the aforementioned *** from the bifactor analyses supported our hypothesis that the total scores of the three AFFEXX-C constructs can be used among Chinese college students to establish a link with ***,our results suggested that core affective exercise experiences and attraction-antipathy mediated the relationship between antecedent appraisals and the level of moderate-to-vigorous intensity ***,measuring affective exercise experiences using the AFFEXX-C,specifically the total scores of each individual construct may be a useful approach to predict future PA levels.

pH值是评价青贮饲料品质的重要指标之一。设计了青贮饲料pH值无线监测装置,实现了对青贮饲料在发酵过程中pH值变化的同步实时获取。在对仪器进行标定试验后,选用玉米秸秆和青草饲料样本对实际测量结果进行验证,结果表明:饲料pH值测量值与准确值呈严格线性相关关系,玉米秸秆和青草饲料样本,r分别为0.997 3和0.995 7,pH值无线监测装置可准确测量青贮饲料pH值。分别对玉米秸秆和青草饲料青贮发酵过程中pH值的变化过程进行监测,结果表明:对于玉米秸秆,其pH值初始值为5.4,从发酵的第0.5天到第2天pH值从5.4快速下降到4.0,之后pH值下降趋缓,最终稳定于3.8左右;对于青草饲料样本,其pH值初始值为5.9,从发酵的第0.8天到第3天pH值从6.0下降到5.2,发酵7 d后,pH值稳定于5.1。

1)Background:The common factors which potentially contribute to the development of eating disorders and exercise dependence during early adulthood are still relatively *** present study aimed to examine the role of BMI,body image inflexibility,and generalized anxiety in these two behavioral problems in a sample of college students.2)Methods:In total,878 habitual exercisers(58.1%male with BMI=22.12±2.39;41.9%female with BMI=20.55±2.21)with age of 20.09±1.76 years participated in this *** main outcomes of interest are exercise dependence symptoms,eating disorders symptoms,body image inflexibility,and symptoms of generalized anxiety(as measured by Exercise Dependence Scale-Revised,Eating Disorder Examination-Questionnaire-Short Form,Body Image Acceptance and Action Questionnaire,and Generalized Anxiety Disorder-7,respectively).Pearson correlation,path analysis,and model fit information were tested.3)Results:After controlling for age,gender,and field of study,lower BMI was linked to more exercise dependence symptoms but this association was not statistically significant,while a greater BMI was significantly associated with a higher risk of developing eating disorders(β=−0.08,p<0.001).Moreover,higher body image inflexibility significantly and positively contributed to severe exercise dependence(β=0.26,p<0.001)as well as abnormal eating attitudes and behaviors(β=0.74,p<0.001).Furthermore,generalized anxiety is a significant contributor to exercise dependence symptoms(β=0.14,p<0.001)but not eating disorders symptoms.4)Conclusion:Based on our finding that body image inflexibility is a common risk factor for the development of exercise dependence and eating disorders,the prevention and treatment of these two disorders should involve the improvement of psychological *** addition,the individual with a higher BMI is more vulnerable to developing eating disorders,while those who have severer generalized anxiety symptoms should be given more attention whe
