Aripiprazole augmentation in treating comorbid bipolar disorder and obsessive-compulsive disorder

Obsessive-compulsive disorder(OCD) is one of the most difficult additional diagnoses to manage in patients with bipolar disorder(BD) since the gold standard treatment for one disease(antidepressants for OCD) can worsen the other. This case report describes the efficacy of aripiprazole augmentation as maintenance therapy in a young patient with comorbid BD-OCD. Our patient presented complete remission of affective and obsessivecompulsive symptoms with remarkable improvement in social and occupational functioning for 24 *** drug reactions were not severe enough to result in drug discontinuation. In consideration of the important nosological, clinical and therapeutic implications, future research efforts may lead to more grounded guidelines,which are greatly needed in patients with comorbid BDOCD.

Agriculture is an important cause of multiple pollutants in *** population growth and increasing food demand,more nutrients,plastics,pesticides,pathogens and antibiotics are expected to enter water systems in the 21st *** a result,water science has been shifting from singlepollutant to multi-pollutant perspectives for large-scale water quality *** perspective paper summarizes and discusses four main highlights related to water pollution and agriculture from the multi-pollutant *** highlights reveal the spatial and temporal distribution and main sources of multiple pollutants in *** on the highlights,a scientific agenda is proposed to prioritize solutions for sustainable agriculture(UN Sustainable Development Goal 2)and clean water(UN Sustainable Development Goals 6 and 14).This agenda points out that when formulating solutions for water pollution,it is essential to take into account multiple pollutants and their interactions beyond biogeochemistry.

This study aims to address the feasibility of planned islanding operation and to investigate the effect of unplanned islanding using the master-slave islanding method for controlling the distributed generation units during grid-connected and islanding operation. Neplan desktop power simulation tool was used for the modelling and simulation of a realistic MV network with four different distributed generation technologies (diesel, gas, hydro and wind) along with their excitation and governor control systems, while an exponential model was used to represent the loads in the network. The dynamic and steady state behavior of the four distributed generation technologies were investigated during grid-connected operation and two transition modes to the islanding situation, planned and unplanned. The obtained results that validated through various case studies have shown that a suitable planned islanding transition could provide support to critical loads at the event of electricity utility outages.

Current approaches for the treatment of chronic lymphocytic leukemia （CLL） have greatly improved the prognosis for survival, but some patients remain refractive to these therapeutic regimens. Hence, in addition to reducing the long-term side- effects of therapeutics for all leukemia patients, there is an urgent need for novel therapeutic strategies for difficult-to-treat leukemia cases. Due to the cytotoxicity of drugs, the major challenge currently is to deliver the therapeutic agents to neoplastic cells while preserving the viability of non-malignant ceils. In this study, we propose a therapeutic approach in which high doses of hydroxychloroquine and chlorambucil were loaded into biodegradable polymeric nanopartides coated with an anti-CD20 *** first demonstrated the ability of the nanoparticles to target and internalize in tumor B-cells. Moreover, these nanoparticles could kill not only p53-mutated]deleted leukemia cells expressing a low amount of CD20, but also circulating primary ceils isolated from chronic lymphocytic leukemia patients. The safety of these nanoparticles was also demonstrated in healthy mice, and their therapeutic effects were shown in a new model of aggressive leukemia. These results showed that anti-CD20 nanoparticles containing hydroxychloroquine and chlorambucil can be effective in controlling aggressive leukemia and provided a rationale for adopting this approach for the treatment of other B-cell disorders.

The authors focused on the current surgical treatment of resectable gastric cancer, and significance of peri- and post-operative chemo or chemoradiation. Gastric cancer is the 4th most commonly diagnosed cancer and the second leading cause of cancer death worldwide. Surgery remains the only curative therapy, while perioperative and adjuvant chemotherapy, as well as chemoradiation, can improve outcome of resectable gastric cancer with extended lymph node dissection. More than half of radically resected gastric cancer patients relapse locally or with distant metastases, or receive the diagnosis of gastric cancer when tumor is disseminated; therefore, median survival rarely exceeds 12 mo, and 5-years survival is less than 10%. Cisplatin and fluoropyrimidine-based chemotherapy, with addition of trastuzumab in human epidermal growth factor receptor 2 positive patients, is the widely used treatment in stage IV patients fit for chemotherapy. Recent evidence supports the use of second-line chemotherapy after progression in patients with good performance status

The incidence of esophagogastric junction adenocarcinoma(AeG) is increasing worldwide. Barrett's esophagus(Be) associated with dysplasia is the main risk factor for the development of cancer. Currently, screening programs to individuate and eradicate Be represent the best way to reduce AeG cancer. Several endoscopic approaches are here discussed. Surgicalstrategies for different types of AeG cancer are now fairly standardized, and multidisciplinary strategies u s i n g c h e m o t h e ra py o r c h e m o ra d i o t h e ra py m ay improve the outcome of these patients. Here we briefly discuss the keypoints, main topics, and critical issues, according to accumulating evidence and taking into account our own experience.

The Microflora of Gills, Gut and Skin of european eels （Anguilla anguilla） in Lakes of Latvia

The microbial contamination of fish is the most important factor in assurance of food safety. The microflora on the surface of skin, gills and gut of fish is constantly under the influence of the water environment and this may cause colonization of microorganisms on fish. The goal of this study was to evaluate the bacterial contamination level on the skin, gills and gut of the european eel （Anguilla anguilla）, i.e., detecting total bacteria count （TBC） and enterobacteriaceae counts, as well as Listeria spp, and Salmonella spp. in freshly caught fish. Among the three lakes, TBC on skin, gills and gut varied from 0.66 CFU/cm2 to 4.93 CFU/cm~, from 0.40 CFU/cm2 to 5.51 CFU/cm2 and from 0.30 CFU/cm2 to 6.37 CFU/cm2, respectively. enterobacteriaceae count on skin, gills and gut was from 0 CFU/cm2 to 4.30 CFU/cm2, from 0 CFU/cm2 to 2.47 CFU/cm2 and from 0 CFU/cm2 to 1.72 CFU/cm2, respectively. The highest mean count of TBC on gills, skin and gut was found in samples from Sivers lake, while the lowest was in samples from Aluksne lake. Values among the lakes were significanty （P 〈 0.05） different. Also the highest mean counts of enterobacteriaceae were found on gills and skin of eels from Sivers lake, but the highest count on gut was found in Usma lake. All tested samples were Listeria monocytogenes and Salmonella spp. negative. TBC and enterobacteriaceae counts of skin, gills and gut were typical for wild fish in fresh water. Foodborne pathogens, such as Salmonella spp. and L. monocytogenes, have not been found in the present study, indicating that fish are safe for human consumption.
