A multifaceted approach towards understanding the peculiar behavior of(α)-hydroxyiminophosphonates

The peculiar isomer-selective reduction of(α)-hydroxyiminophosphonates(oxime isomers)into(α)-hydroxyaminophosphonate(hydroxylamine)derivatives is presented.A library of 16(α)-hydroxyiminophosphonates is prepared and studied via a unique multifaceted approach involving the interplay of NMR,XRD,MS,IM-MS and computational chemistry *** combination of NMR,XRD and HPLC enables the seamless separation,identification and quantification of the oxime isomers(E/Z).Tandem MS(MS/MS)enables the determination of the fragmentation patterns for both *** energy breakdown curves highlight the order of apparition of the fragments as well as their related energy of fragmentation,demonstrating that the strength of the C–P bond in the Z isomers is much weaker than in the E *** chemistry demonstrates that favorable protonation site is isomer-dependent with the phosphonate moiety being the favorable protonation site for the E isomers,while protonation occurs preferentially on the amino moiety for Z isomers regardless of the phosphite *** combination of these various methods led an unprecedented level of characterization of oxime isomers,providing a better uderstanding of the isomer-dependent behavior of(α)-hydroxyiminophosphonates.

A new Ni(II) coordination polymer [Ni(L)2(SCN)2]n(L = 1,4-bis(benzimidazol-1-yl)butane) was synthesized hydrothermally and characterized by elemental analysis, IR spectroscopy and single-crystal X-ray diffraction. The title compound crystallizes in the monoclinic system, space group C2/c with a = 9.4760(3), b = 24.0408(8), c = 16.5871(5) A, β = 99.832(3)°, V = 3723.2(2) A3, Z = 4, Dc = 1.348 g/cm3, f(000) = 1576, the final R = 0.0486 and w R = 0.0936 for 2938 observed reflections with I > 2σ(I) for the complex. Structural analysis shows that the coordination polymer possesses a 2D(4,4) layer structure which is composed of Ni(II) centers bridged by L ligand with two kinds of conformations and further assembled into a 3D supramolecular network via π-π stacking interactions. In addition, the fluorescence and catalytic properties, for the degradation of Congo red, of the complex were investigated.

A new Co（Ⅱ） metal-organic coordination polymer based on flexible bis（imidazole） and aromatic dicarboxylate co-ligands, namely [Co（bix）（nph）]n （H2nph = 3-nitrophthalic acid, bix = 1,4-bis（imidazole-l-ylmethyl）benzene）, has been hydrothermally synthesized and characterized by elemental analyses, TG, IR spectroscopy and single-crystal X-ray diffraction. It crystallizes in the triclinic space group Piwith a = 9.3767（14）, b = 10.1451（15）, c = 12.1488（17） A, α = 102.6450（10）, β = 108.856（2）, ）, = 98.807（2）°, V= 1035.3（3） A3, Z = 2, C22H17CoN5O6, Mr = 506.34, Dc = 1.624 g/cm^3, p = 0.882 mm^-1 and f（000） = 518. In the complex, the nph^2- ligands connect neighbouring cobalt atoms to form binuclear [Co（nph）]2 subunits, which are linked by pairs of bix ligands to form a 2D honeycomb-like （6,3） network. In addition, the compound is further extended into a 3D supramolecular architecture by π…π stacking interactions. Moreover, the luminescence and catalytic properties of the complex are investigated.

1.前言神经病理性疼痛是公认的最难治疗的疼痛综合征之一，而且转归往往不尽人意。原因部分是由于神经病变产生的疼痛可能在初级医疗中未被发现，并且药物的使用未达标准。在这一领域的流行病学研究是存在疑问的，例如缺乏一致性，缺乏能真实反映病例状态和应用于人群研究的有效病例定义；再如变量的多样性研究，病例确诊的方法不同；此外，疼痛不是主诉表现，病例如何纳入或排除等。现有的一般人群患病率的评估可能变得更高，可能有更多的患者正经历着神经病理性疼痛的折磨。

The purpose of this paper is to explore the stimulating factors and the obstacles experienced by nurses in community health centres in their work with prevention and health promotion. We conducted the qualitative research design consisting of fifteen nurses in five community health centres in Ghent. There was also a focus group and participant observation. All participants experienced stimulating factors as well as obstacles in their work with preventive interventions. The most strongly facilitators were the Electronic Medical file and the multidisciplinary approach. The obstacles mentioned by the nurses were the communication problems and the financial problems. This study may have an important impact on nursing practice and nursing prevention. further research is needed on nurses working in community health centres and the way they approach their patients in the course of preventive actions.

背景儿童抗生素暴露具有重要的临床相关意义。其中包括对微生物群的破坏、抗生素耐药性以及表现为治疗“失败”的临床工作。目的探讨患急性呼吸道感染的学龄前儿童前1 年的抗生素疗程与后1 年未对抗生素治疗有反应的患急性呼吸道感染之间的关系。设计与场所采用2009—2016 年英国临床实践研究数据链中的初级保健数据进行队列研究。方法对12~60 个月(1 ~ 5 岁)的儿童进行急性呼吸道感染(上、下呼吸道感染或中耳炎)的抗生素治疗。为每个儿童随机选择一个初始抗生素疗程。暴露是指在给予初始抗生素处方前12 个月内服用治疗急性呼吸道感染的抗生素数量。结果是在接受初始抗生素处方后14 d 内出现“反应失败”,定义为:继续开具抗生素处方;转诊;入院;死亡;或3 d 内在急诊科就诊。使用Logistic 回归分析来估计抗生素暴露和“反应失败”的概率。结果在114 329 例接受抗生素治疗的急性呼吸道感染患儿中,前1 年接受≥ 2 次抗生素治疗的急性呼吸道感染患儿有较高的“反应失败”率;1 次抗生素治疗(230 名儿童):调整后的比值比(OR)=1.03〔95%CI(0.88,1.21),P=0.67〕;≥ 2 个抗生素疗程(97 名儿童):调整后OR=1.32〔95%CI(1.04,1.66),P=0.02〕。结论儿童急性呼吸道感染抗生素暴露可能是后续反应失败(但不一定是抗生素治疗失败)的预测因素。需要进行进一步的研究来提高对潜在“反应失败”机制的理解。
