AIM:to evaluate the safety and effectiveness of our new cytoreductive surgery(CRS)and hyperthermic intraperitoneal chemoperfusion(HIPEC)***:Retrospective review of patients with gastrointestinal malignancies who were suitable candidates for CRS and HIPEC between 12/1/2009 and 10/1/*** clinicopathologic data were reviewed with a special focus on the surgical outcome and the postoperative morbidity and ***:Fourteen patients were *** age was 64 years;seven were *** primary tumors were:colonic(29%),appendiceal(36%),peritoneal mesothelioma(14%),gastric(7%),adenocarcinoma of unknown primary(7%),and gastrointestinal stromal tumor(7%).Eleven patients(79%)received CRS/HIPEC,three for *** patients that did not undergo CRS/HIPEC had an average peritoneal cancer index(PCI)of *** eight patients that underwent curative CRS/HIPEC had an average PCI of 10 and a completeness of cytoreduction score of 0(87.5%) or 1(12.5%).Postoperative morbidity was 36%;the worst adverse event was Grade 3 *** rate was 0%.CONCLUSION:CRS with HIPEC is safe and feasible at tertiary institutions with fledgling *** is an accurate predictor of surgical outcomes.

the use of Spontaneous Respiration using IntraVEnous anesthesia and High-flow nasal oxygen (StRIVE-Hi) in laryngeal surgery has become more widely reported. this method eliminates the endotracheal tube as a fuel for a potential fire. However, little has been published on its use in the pediatric population. Our case report describes its use in a 2-year-old undergoing micro-direct laryngoscopy with CO2 assisted supraglottoplasty and rigid bronchoscopy for airway obstruction from congenital laryngomalacia. the StRIVE-Hi technique was modified for the pediatric patient by using a lower flow through the nasal cannula (4 L). No major changes in SpO2 were detected during the 30-minute procedure. With back up airway safety equipment in place, StRIVE is proving to be a safe technique with major advantages when used in this unique scenario.

Purpose: One assumption of the uniocular drug trial in glaucoma management is that fellow-eye pairs exhibit symmetric intraocular pr essure (IOP) responseswhen both eyes are treated with the same topical IOP-lowe ring medication. the purpose of this study was to determine if this assumption i s true. Design: Observational case series. Participants and Controls: Fortythree patients with bilateral glaucoma or ocular hypertension who underwent bilateral treatment with a topical IOP-lowering medication. Methods: Patients were on 0 (81%) or 1 (19%)-IOP-lowering medication at the time the medication was adde d in this study. Baseline IOP was the IOP at the treatment initiation visit, and IOP on treatment was assessed in both eyes on the first visit after addition of the medication. Linear regression of right-eye versus left-eye IOP reductions was performed. Main Outcome Measure: Correlation of right-eye and left-eye IOP responses to therapy. Results: Right eyes demonstrated a 5.0-mmHg (26.9%) red uction, and left eyes demonstrated a 4.3-mmHg (23.7%) reduction. Linear regres sion of right-eye IOP reduction against left-eye IOP reduction gave a slope of 0.77 (Pearson correlation coefficient, r=0.84; r2=0.70; P ted marked symmetry in IOP responses to IOP-lowering medications.

Background Dairy cows experiencing ketosis after calving suffer greater disease incidence and are at greater risk of leaving the herd. In vitro administration of beta-hydroxybutyric acid(BHBA;the primary blood ketone) has inhibitory effects on the function of bovine leukocytes. BHBA is a ligand of HCAR2 and the activation of these receptors promotes an anti-inflammatory response which may be related with immunosuppression observed in transition dairy cattle. the objective of this study was to identify and test antagonists for HCAR2 in bovine immune cells cultured with *** We observed expression of HCAR2 at the protein level within lymphocytes, monocytes, and granulocytes. the proportion of cells expressing HCAR2 tended to be greater in mid-lactation compared to early lactation cows;the increase was a result of increased proportion of t and B cells expressing HCAR2. Stimulation of HCAR2 with niacin or BHBA promoted Ca^(2+) mobilization in neutrophils and mononuclear cells. Mononuclear cells treated with BHBA had diminished intracellular Ca^(2+) responses when HCAR2 was knocked down by si RNA silencing, indicating Ca^(2+) mobilization was mediated by HCAR2 signaling. two candidate antagonists for HCAR2, synthesized from niacin(NA-1 and NA-5), were tested;monocytes and neutrophils pre-treated with NA-1 and NA-5 had reduced Ca^(2+) mobilization after incubation with BHBA. Furthermore, NA-5 but not NA-1 prevented BHBA-associated reductions in cyclic *** We demonstrated that HCAR2 is present on bovine leukocytes and has greater expression later in lactation. We confirmed that BHBA and niacin derived HCAR2 antagonists alter bovine leukocyte activity. AIM to determine the scope of acute hypoglycemic effects for certain anti-rheumatic medications in a large retrospective observational study. MEtHODS Patients enrolled in the Veterans Affairs Rheumatoid Arthritis (VARA) registry were selected who, during follow-up, initiated treatment with tumor necrosis factor inhibitors (tNFi's, including etanercept, adalimumab, infliximab, golimumab, or certolizumab), prednisone, or conventional disease-modifying anti-rheumatic drugs(DMARDs), and for whom proximate random blood glucose (RBG) measurements were available within a window 2-wk prior to, and 6 mo following, medication initiation. Similar data were obtained for patients with proximate values available for glycosylated hemoglobin A1C values within a window 2 mo preceding, and 12 mo following, medication initiation. RBG and A1C measurements were compared before and after initiation events using paired t-tests, and multivariate regression analysis was performed including established comorbidities and *** two thousands one hundred and eleven patients contributed at least one proximate measurement surrounding the initiation of any examined medication. A significant decrease in RBG was noted surrounding 653 individual hydroxychloroquine-initiation events(-3.68 mg/dL, P = 0.04), while an increase was noted for RBG surrounding 665 prednisone-initiation events(+5.85 mg/d L, P tatistically significant decrease in A1C was noted for sulfasalazine initiation, as measured by 49 individual initiation events(-0.70%, P tivariate regression analyses, using methotrexate as the referent, suggest sulfasalazine (β =-0.58, P = 0.01) and hydroxychloroquine(β =-5.78, P = 0.01) use as predictors of lower post-medicationinitiation RBG and A1C values, respectively. Analysis by drug class suggested prednisone (or glucocorticoids) as predictive of higher medication-initiation event RBG among all start events as compared to DMARDs, while this analysis did not

Karst habitats are hotspots of diversity and endemism. their naturally fragmented distributions across broad geographic landscapes have led to the complex array of smaller evolutionary ecosystems that present unique challenges from a conservation *** assessments of karst habitats have revealed that these ecosystems contain an almost unparalleled level of endemism, and many site-restricted species remain undescribed, thus posing considerable challenges for effective conservation *** rock-dwelling species, such as geckos, may be particularly prone to such isolation. In this paper, we discuss one such genus, i.e.,Hemiphyllodactylus, and explore its diversity across karst landforms in Yunnan Province, southwestern China. Based on morphological and genetic data, we describe two new species of Hemiphyllodactylus from karst habitats in Simao District and Yanshan County. A phylogenetic tree for Hemiphyllodactylus was constructed using 1?039 base pairs(bp) of the mitochondrial NADH dehydrogenase subunit 2 gene(ND2). the Simao and Yanshan specimens can be distinguished from all other congeners within their respective subclades based on uncorrected genetic pairwise distances greater than 6.3% and 4.3%respectively, as well as significant morphological differences. the discovery and description of these two new species brings the total number of described Hemiphyllodactylus species in China to 14and indicates many more undescribed species from unsurveyed karst regions await *** findings suggest that karst ecosystems in Yunnan support a higher diversity of Hemiphyllodactylus than previously *** study also highlights the importance of karst ecosystems as refugia for sitespecific endemic species and the need for heightened conservation efforts.
