Background: On December 26, 2004, the biggest earthquake for 40 years, measuring 9.0 on the Richter scale, triggered a tsunami that pounded the coastal areas of South Asia and East Africa. The effects of the tsunami on skin conditions have not been evaluated. Objective: To determine the influence of the tsunami on skin conditions by evaluating the skin problems of patients presenting at hospitals after the tsunami. Methods: Between 5 and 25 January 2005, two dermatologists evaluated patients who complained of skin problems at an outpatient clinic and emergency room of a general hospital in Banda Aceh, Aceh Province, Indonesia. Results: The total number of patients that presented during the study period was 235 (131 males and 104 females), and they had a total of 265 skin problems. In terms of age distribution, most subjects were in their fourth decade (23.0%), followed by the third (22.6%) and fifth decade (16.6%). The most prevalent skin problems were infections-infestations (32.5%), followed by eczemas (29.8%) and traumatic skin disorders (29.4%). In males, traumatic skin disorders were most common. The great majority of infection-infestation cases involved superficial fungal infections. Contact dermatitis accounted for three-quarters of eczema cases, and mainly involved the arms (40.0%) and legs (27.1%). The majority of traumatic skin disorders were lacerations, punctures and penetrations, and the feet (44.7%) and hands (18.8%) were most frequently affected. Conclusions: Unhygienic conditions, exposure to a hazardous environment and contact with various objects during and after the tsunami probably increased the prevalence of infections-infestations, traumatic skin disorders and contact dermatitis. To prevent these problems and associated secondary bacterial infections, health-related education and early medical management are required.

AIM:To evaluate the incidence and risk factors of Korean tuberculosis(TB) infection in patients with inflammatory bowel disease(IBD) undergoing anti-TNF ***:The data of IBD patients treated with anti-TNFs in 13 tertiary referral hospitals located in the southeastern region of Korea were collected *** failed to show response or were intolerant to conventional treatments,including steroids or *** measures for latent TB infection(LTBI)and the incidence and risk factors ofactive TB infection after treatment with anti-TNFs were ***:Overall,376 IBD patients treated with antiTNF agents were recruited(male 255,mean age of anti-TNF therapy 32.5±13.0 years);277 had Crohn’s disease,99 had ulcerative colitis,294 used infliximab,and 82 used *** anti-TNF treatment,screening tests for LTBI including an interferon gamma release assay or a tuberculin skin test were performed in 82.2%of *** patients(8%)had *** cases of active TB infection including one TB-related mortality occurred during 801 personyears(PY)follow-up(1997.4 cases per 100000 PY)after anti-TNF ***(OR=5.76,95%CI:1.57-21.20,P=0.008)and WBC count<5000 mm3(OR=4.5,95%CI:1.51-13.44,P=0.007)during follow-up were identified as independently associated risk ***:Anti-TNFs significantly increase the risk of TB infection in Korean patients with *** considerable burden of TB and marked immunosuppression might be attributed to this risk.

Medulloblastoma（MBL）,the most common malignant pediatric brain tumor,is incurable in about one-third of patients and can lead to long-term disabilities despite current multimodal *** purpose of this study was to demonstrate in vitro biological effects of neurotrophins-3（NT-3） on MBL cells and to evaluate the growth-inhibitory effect of neurotrophin-3（NT-3）-secreting stem cells on tumor *** confirmed by western blotting that D283-MED cells express tyrosine kinase C,a specific receptor for *** the biological effects of NT-3 on MBL cells,we evaluated autophagy,apoptosis,senescence,and differentiation of tumor cells with *** NT-3 induced a concentrationdependent increase in apoptosis in the tumor cell line（P ＜ 0.001）.The high concentrations of NT-3 increased the expression of class III β-tubulin（P ＜ 0.001） and decreased the expression of Nestin（P ＜ 0.05）.NT-3-secreting stem cells were produced by nucleofecting ***-NT3 into human adipose tissue-derived mesenchymal stem cells（hAT-MSCs） and were confirmed by fluorescence microscopy and ***-layered co-culture experiments with the NT-3-secreting hAT-MSCs and D283-MED MBL cells were performed,and NT-3-induced cell death was studied by 3-（4,5-dimethylathiazol-2-yl）-2,5-dephenyltetrazolium bromide（MTT） ***,the high concentrations of NT-3-secreting hAT-MSCs significantly（P ＜ 0.05） increased the death of D283-MED cells in *** present study demonstrated that both apoptotic cell death and neuronal differentiation of tumor cells were the mechanisms of growth-inhibitory effect of NT-3-secreting hAT-MSCs on MBL cell line.

Gastrointestinal stromal tumor(GIST)is a rare mesenchymal tumor of the gastrointestinal tract that has been associated with the formation of fistulas to adjacent organs in few case ***,GIST with enterohepatic fistula has not been *** we report the case of an enterohepatic fistula that occurred after embolization of a liver mass originating in the distal *** 87-year-old woman was hospitalized for *** initial conventional endoscopy,a bleeding focus in the gastrointestinal tract was not *** of massive hematochezia,enteroscopy was performed through the anus.A protruding,ulcerative mass was found in the distal ileum that was suspected to be the source of the bleeding;a biopsy sample was *** was not successful in controlling the bleeding;therefore,embolization was *** embolization,the patient developed a high fever and severe abdominal tenderness with rebound ***-up abdominopelvic computed tomography revealed an enterohepatic fistula between the liver and distal *** fistula was treated surgically by segmental resection of the distal ileum and unlooping of the liver mass.

＜正＞Twenty-one patients with primary high-grade advanced gastric cancer in whom the presence of stage ? was con- firmed by preoperative diagnostic imaging,were treated with a wide-spectrum regimen,designated as FEPMTX therapy to establish an effective salvage *** therapy consisted of 5-fluorouracil（5-FU）and the triple biochemical modulators;methotrexate（MTX）,calcium leucovorin（LV）,cisplatin（CDDP）,and in addition of epiru- bicin（EPI）.The schedule comprised 3 days continuous administration of 5-FU（350 mg/m2/day）and MTX（35 mg/ m2）on day 1,LV（30 mg/m2）on the day 2,LV（30 mg/m2）and CDDP（30 mg/m2）and EPI（20 mg/m2）on day *** treatment was performed two or more courses with 2 weeks interval in *** partial responses（PR）, five no changes（NC）and five progressive diseases（PD）were obtained,and the response rate was 52%.Ten patients （PR 7,NC 2,PD 1）were received gastrectomy（resectability rate 48%）.The survival in all cases resulted in median survival of 184 days and one-year survival rate of 23%.While there is no significant difference insurvival between the cases with or without the resection of the primary lesion,the survival of responders was significantly longer than that of non-responders（median survival time;responder 356days,non-responder 152days）.Adverse effects such as myelo- suppression and anorexia and fatigue sometimes occurred,but were mild and all the patients well *** therapy is thought to be an effective regimen for neoadjuvant chemotherapy with longer survival and little toxic for patients with stage?gastric cancer.

AIM: To evaluate the diagnostic yield and safety of a modified technique for the histological diagnosis of subepithelial tumors (SETs). METHODS: A retrospective review of patients who underwent a modified technique for the histological diagnosis of gastric SETs, consisting of a mucosal incision with a fixed flexible snare (MIF) and deep-tissue biopsy under conventional endoscopic view, from January 2012 to January 2013 was performed. Eleven patients with gastric SETs 10-30 mm in diameter and originating from the third or fourth layer on endoscopic ultrasonography were included. RESULTS: The mean age was 59.8 (range, 45-76) years, and 5 patients were male. The mean size of the SETs was 21.8 (range, 11-30) mm. The number of biopsy specimens was 6.3 (range 5-8). The mean procedure time was 9.0 min (range, 4-17 min). The diagnostic yield of MIF biopsies was 90.9% (10/11). The histological diagnoses were leiomyoma (4/11, 36.4%), aberrant pancreas (3/11, 27.3%), gastrointestinal stromal tumors (2/11, 18.2%), an inflammatory fibrinoid tumor (1/11, 9.1%); one result was non-diagnostic (1/11, 9.1%). There were six mesenchymal tumors; the specimens obtained in each case were sufficient for an immunohistochemical diagnosis. There was no major bleeding, but one perforation occurred that was successfully controlled by endoscopic clipping. CONCLUSION: The MIF biopsy was simple to perform, safe, and required a shorter procedure time, with a high diagnostic yield for small SETs.
