Compared efficacy of preservation solutions on the outcome of liver transplantation:Meta-analysis

AIM To compare the effects of the four most commonly used preservation solutions on the outcome of liver *** A systematic literature search was performed using MEDLINE, Scopus, EMBASE and the Cochrane Library databases up to January 31^(st), 2017. The inclusion criteria were comparative, randomized controlled trials(RCTs) for deceased donor liver(DDL) allografts with adult and pediatric donors using the gold standard University of Wisconsin(UW) solution or histidinetryptophan-ketoglutarate(HTK), Celsior(CS) and Institut Georges Lopez(IGL-1) solutions. Fifteen RCTs(1830 livers) were included; the primary outcomes were primary non-function(PNF) and one-year posttransplant graft survival(OGS-1). RESULTS All trials were homogenous with respect to donor and recipient characteristics. There was no statistical difference in the incidence of PNF with the use of UW, HTK, CS and IGL-1(RR = 0.02, 95%CI: 0.01-0.03, P = 0.356). Comparing OGS-1 also failed to reveal any difference between UW, HTK, CS and IGL-1(RR = 0.80, 95%CI: 0.80-0.80, P = 0.369). Two trials demonstrated higher PNF levels for UW in comparison with the HTK group, and individual studies described higher rates of biliary complications where HTK and CS were used compared to the UW and IGL-1 solutions. However, the meta-analysis of the data did not prove a statistically significant difference: the UW, CS, HTK and IGL-1 solutions were associated with nearly equivalent *** Alternative solutions for UW yield the same degree of safety and effectiveness for the preservation of DDLs, but further well-designed clinical trials are warranted.

Corneal biomechanical data and biometric parameters measured with Scheimpflug-based devices on normal corneas

Non-pancreatic hyperlipasemia:A puzzling clinical entity

Ocular biomechanical measurements on post- keratoplasty corneas using a Scheimpflug-based noncontact device

AIM： To analyse ocular biomechanical properties, central corneal thickness （CCT） and intraocular pressure （lOP） in post-keratoplasty eyes, as compared to normal subjects, with a new Scheimpflug-based technology. Moreover, biomechanical data were correlated with the size and age of the donor and recipient corneas. METHODS： Measurements were conducted on 46 eyes of 46 healthy patients without any corneal pathology （age： 53.83 ±20.8y） and 30 eyes of 28 patients after penetrating keratoplasty （age： 49.43 ±21.34y）. Ten biomechanical parameters, the CCT and lOP were recorded by corneal visualization Scheimpflug technology （CorVis ST） using high-speed Scheimpflug imaging. Keratometry values were also recorded using Pentacam HR system. Scheimpflug measurements were performed after 43.41± 40.17mo （range： 11-128mo） after the keratoplasty and after 7.64±2.34mo （range： 5-14mo） of suture removal. RESULTS： Regarding the device-specific biomechanical parameters, the highest concavity time and radius values showed a significant decrease between these two groups （P=0.01 and P 〈0.001）. None of other biomechanical parameters disclosed a significant difference. The CCT showed a significant difference between post keratoplasty eyes as compared to normal subjects （P= 0.003） using the CorVis ST device. The lOP was within the normal range in both groups （P=0.84）. There were no significant relationships between the keratometric data, the size of the donor and recipient, age of the donor and recipient and biomechanical properties obtained by CorVis ST. CONCLUSION： The ocular biomechanics remain stable after penetrating keratoplasty according to the CorVis ST measurements. Only two from the ten device-specific parameters have importance in the follow-up period after penetrating keratoplasty.

Scanning-slit topography in patients with keratoconus

AIM： To evaluate the anterior and posterior corneal surfaces using scanning-slit topography and to determine the diagnostic ability of the measured corneal parameters in ***： Orbscan II measurements were taken in 39 keratoconic corneas previously diagnosed by corneal topography and in 39 healthy eyes. The central minimum, maximum, and astigmatic simulated keratometry（K） and anterior axial power values were determined. Spherical and cylindrical mean power diopters were obtained at the central and at the steepest point of the cornea both on anterior and on posterior mean power maps. Pachymetry evaluations were taken at the center and paracentrally in the 3 mm zone from the center at a location of every 45 degrees. Receiver operating characteristic（ROC） analysis was used to determine the best cut-off values and to evaluate the utility of the measured parameters in identifying patients with ***： The minimum, maximum and astigmatic simulated K readings were 44.80±3.06 D, 47.17±3.67 D and 2.42±1.84 D respectively in keratoconus patients and these values differed significantly（P〈0.0001 for all comparisons） from healthy subjects. For all pachymetry measurements and for anterior and posterior mean power values significant differences were found between the two groups. Moreover, anterior central cylindrical power had the best discrimination ability（area under the ROC curve=0.948）.CONCLUSION： The results suggest that scanning-slit topography and pachymetry are accurate methods both for keratoconus screening and for confirmation of the diagnosis.

Objective:Neonates with congenital heart disease(CHD)and perinatal stroke have high mortality and survivors are at risk for poor long-term neurodevelopmental *** aim of this study was to assess the risk factors and outcome of neonates with both CHD and MRI-confirmed perinatal stroke(Study Group)and compare those to the risk factors and outcome of infants matched for CHD without stroke(Control-1)and of infants matched for MRI-confirmed stroke without CHD(Control-2).Methods:We conducted a population-based case-control study enrolling 28 term neonates with CHD and MRI-confirmed acute perinatal stroke born between 2007–2017 in the Central-Hungarian *** of the control groups included 56 *** Bayley Scales of Infant Development-II,the Brunet-Lézine test and the Binet Intelligence scales-V were used for neurodevelopmental follow-up at a median age of 61 ***:Mortality was highest in the Study Group(25%compared to 5%and 2%,respectively,p=0.001).Adverse neurodevelopmental outcome was prevalent in the Study(53%)and Control-2 Groups(52%,p=0.03).Significantly different parameters among the three groups included Apgar scores,mode of delivery,gestational age at birth,cardiac interventions and twin *** a multivariable regression analysis adjusted for clinically relevant parameters,patients in the Study Group had significantly higher odds for mortality compared to patients in the Control-1 Group(OR:6.595%CI:1.1–39.4).Conclusions:Neonates with perinatal stroke and CHD are at a higher risk for dying compared to neonates with CHD without *** addition,the stroke-associated direct insult to the brain likely plays an important role in the development of neurodevelopmental morbidity in these patients.

随着互联网技术的深入发展,第三方评价平台近年来在互联网蓬勃兴起,为消费者带来极大便利,对于降低消费者与商家之间的信息不对称、提升消费者消费选择质量、促进企业对产品质量进行革新等方面,具有极大正面的作用。但是在学术界,关于第三方评价平台的研究仍处于起步阶段,因而在管理实践中还存在较多问题。基于当前第三方评价平台主题缺乏消费者行为学领域研究的现状,本研究着重探讨两个问题:一是何种原因在影响消费者对第三方评价平台进行评价的意愿;二是第三方评价平台的哪些因素在影响消费者的实际购买意愿。本研究本着数据可获取性与产品体验性兼得的原则,选取宾馆业企业为研究对象,并以虚拟第三方评价平台作为研究对象,以第三方评价平台用户和宾馆消费者为研究样本,通过发放网络问卷收集研究所需数据,利用SPSS软件进行回归分析,对数据样本进行分析。实证研究结果表明:第一,消费者参与第三方评价平台进行评价的意愿,受到三方面的影响:消费者特征因素、网络平台因素和个人体验因素,其中,消费者特征因素包括消费者熟悉度、消费者创新性心理和品牌忠诚度等、网络平台因素包括评价奖励金额、评价填写便捷性和信息安全性等、个人体验因素包括功能性体验价值、社会性体验价值和情感性体验价值。第二,两类第三方评价平台评价特征影响消费者的购买意愿,分别是:信息特性因素平台特性因素,其中,信息特性因素包括评价数量、口碑效价和内容丰富度等三个方面;平台特性因素包括反馈积极度、信源可信度和互动交流度等三个方面。最后,根据实证研究结果,基于第三方评价平台的视角,给出了消费者评价意愿提升的策略;以及基于宾馆业企业营销战略的角度,提出了如何利用第三方评价平台提升消费者购买意愿的策略。本研究有一定的理论贡献。第一,鉴于当前网络口碑评价研究的空白之处,本研究从个人体验因素的视角,丰富了第三方评价平台的评价意愿影响因素的研究;第二,针对网络评价平台本身的特性和信息的本质性特点结合的角度出发,从整体性的视角,拓展了第三方评价平台特征对消费者行为的影响研究。第三,本研究从第三方评价平台特点的视角切入,进一步深化了外界信息,特别是网络评价对于消费者行为方式的影响研究。同时,本研究对于第三方评价平台的相关企业运营和宾馆业企业营销战略的制定亦具备一定的理论指导意义:一方面,本研究的实证研究结论有助于引导第三方评价平台企业吸引评论,增长平台活跃度;另一方面,本研究的实证研究结论也有助于宾馆业的企业管理者利用第三方平台提升消费者对宾馆的购买意愿。当然本研究也难免存在着诸多的不足之处,需要在未来的研究中加以改进。第一,研究样本在选取时存在着一定的局限性,未来的研究应进一步扩大中老年样本、低学历样本和低收入样本的选取范围,并适当考虑东西方消费者样本差异研究。第二,影响因素的选取范围存在一定的局限性,未来的研究应从外部信息、消费者特征和产品特性等多角度综合考虑选取影响因素,使研究结果更准确。第三,研究对象的选取存在一定的局限性,未来的研究应继续本着行业体验性强、数据可获取性强的原则,选择除宾馆业之外的其他行业展开研究,从而使研究结果更丰富。第四,对于消费者行为意愿和实际行为间的差异研究仍有不足之处,未来的研究应通过心理学或行为学实验的方式,深入探讨行为意愿和实际行为之间的差异之处,使研究结论更精确。
