To evaluate the sensitivity and specificity of the usage of the FMF On-Line Risk Calculator with first-trimester ultrasound, in screening assessment for preeclampsia (PE), without serum markers. To define the best risk cut-off values for early, intermediate and late preeclampsia. Diagnostic accuracy study of pregnant women who had first-trimester ultrasounds between 11 and 13 weeks. The index test was the first-trimester ultrasound scan plus the FMF On-Line Risk Calculator to assess the risk for PE. The reference standard was the confirmation of actual development of early, intermediate or late PE. For calculations of sensitivity and specificity to determine the best cut-off values for early, intermediate and late PE, all the information was processed into ROC curves. The assessment of preeclampsia risk in the first trimester using an ultrasound plus the FMF On-Line Risk Calculator demonstrated a significant (p < 0.05) area under the ROC curve for early, intermediate and late preeclampsia. The best risk cut-off values were defined as 2.1% for early, 2.5% for intermediate and 3.5% for late preeclampsia. The first trimester US plus the FMF On-Line Risk Calculator tool was useful and applicable when assessing the risk for preeclampsia in a specific pregnant Brazilian population.

Background: In recent years, so-called “non-ablative rejuvenation” has been carried out with the use of lasers or intense pulsed light (IPL) to stimulate collagen production by dermal fibroblasts. Intradermal infusion of CO2 stimulates fibroblasts and the synthesis of collagen and elastin, contributing to the retraction of the skin and tissue rejuvenation. Objectives: To evaluate the effects of IPL and the intradermal infusion of CO2 on fibroblast proliferation and collagen in the skin of female rats. Methods: Sixteen adult female Wistar rats were divided into two groups of eight animals. Group 1 underwent IPL and group 2 underwent intradermal CO2 infusion. There was a total of 8 weeks of treatment. We conducted a punch in each animal before any procedure (T0), another punch in the middle of treatment at 4 weeks post-procedure (T1) and a punch at the end of treatment at 8 weeks post-procedure (T2). The cells involved in inflammation, fibrosis and vascularization of the injured tissue by histopathology were analyzed. Results: There was statistically significant fibroblast proliferation and collagen proliferation noted when analyzing all 16 animals together and also when considering the two study groups separately. In both groups, the greatest proliferation of fibroblasts coincided with periods of increased collagen production. Conclusion: Both IPL and intradermal CO2 infusion stimulated fibroblast and collagen proliferation in the skin of the rats studied.

The high rates of depression,burnout,and increased risk of suicide among medical students,residents,and physicians in comparison with other careers signal a mental health crisis within our profession.We contend that this crisis coupled with the inadequate acquisition of interpersonal skills during medical education results from the interaction between a challenging environment and the mental capital of individuals.Additionally,we posit that mindfulness-based practices are instrumental for the development of major components of mental capital,such as resilience,flexibility of mind,and learning skills,while also serving as a pathway to enhance empathy,compassion,self-awareness,conflict resolution,and relational abilities.Importantly,the evidence base supporting the effectiveness of mindfulness-based interventions has been increasing over the years,and a growing number of medical schools have already integrated mindfulness into their curricula.While we acknowledge that mindfulness is not a panacea for all educational and mental health problems in this field,we argue that there is currently an unprecedented opportunity to gather momentum,spread and study mindfulness-based programs in medical schools around the world as a way to address some longstanding shortcomings of the medical profession and the health and educational systems upon which it is rooted.

Landslides are highly dangerous phenomena that occur in different parts of the world and pose significant threats to human populations. Intense rainfall events are the main triggering process for landslides in urbanized slope regions, especially those considered high-risk areas. Various other factors contribute to the process;thus, it is essential to analyze the causes of such incidents in all possible ways. Soil moisture plays a critical role in the Earth’s surface-atmosphere interaction systems;hence, measurements and their estimations are crucial for understanding all processes involved in the water balance, especially those related to landslides. Soil moisture can be estimated from in-situ measurements using different sensors and techniques, satellite remote sensing, hydrological modeling, and indicators to index moisture conditions. Antecedent soil moisture can significantly impact runoff for the same rainfall event in a watershed. The Antecedent Precipitation Index (API) or “retained rainfall,” along with the antecedent moisture condition from the Natural Resources Conservation Service, is generally applied to estimate runoff in watersheds where data is limited or unavailable. This work aims to explore API in estimating soil moisture and establish thresholds based on landslide occurrences. The estimated soil moisture will be compared and calibrated using measurements obtained through multisensor capacitance probes installed in a high-risk area located in the mountainous region of Campos do Jordão municipality, São Paulo, Brazil. The API used in the calculation has been modified, where the recession coefficient depends on air temperature variability as well as the climatological mean temperature, which can be considered as losses in the water balance due to evapotranspiration. Once the API is calibrated, it will be used to extrapolate to the entire watershed and consequently estimate soil moisture. Evaluation of the regional distribution of the pediatric surgery workforce and surgical load in Brazil

Objective The purpose of this study is to examine the regional distribution of the pediatric surgery workforce and the expected local demand for pediatric surgical procedures in Brazil.Methods We collected data on the pediatric surgical workforce,surgical volume,Gross Domestic Product per capita,and mortality for gastrointestinal tract malformations(MGITM)across the different regions of Brazil for 2019.Results Data from the Federal Medical Council reported 1515 pediatric surgery registries in Brazil,corresponding to 1414 pediatric surgeons(some pediatric surgeons are registered in more than one state),or 2.4 pediatric surgeons per 100000 children 14 years of age and younger.There were 828 men and 586 women.The mean age was 51.5±12.8 years,and the mean time from graduation was 3.4±5.7 years.There is a higher concentration of pediatric surgeons in the wealthier Central-West,South,and Southeast regions.Individual surgical volume ranged from 88 to 245 operations/year(average 146 operations/year)depending on the region.Of these,only nine(6.1%)were high-complexity(including neonatal)operations.MGITM tended to be higher in the poorer North and Northeast regions than in other regions of Brazil.Conclusions Our findings suggest significant disparities in the surgical workforce and workload across Brazil related to socioeconomic status.Regions with an increased surgical workforce were associated with lower MGITM.The average number of complex operations performed annually by each pediatric surgeon was considerably low.Strategic investment and well-defined health policies are imperative to enhance the quality of surgical care in the different regions of Brazil.

In the Brazilian drylands, there are tens of thousands of small dams. Despite their paramount importance to the rural population, they are rarely monitored. Water demand increases with time while, simultaneously, siltation reduces reservoir water capacity and availability. Reservoir siltation models are, therefore, mandatory to manage the numerous ungauged small dams in these drylands.The objective of the current study is to improve sediment trap efficiency(TE) modeling by including resuspension as a key probabilistic process. The Shannon entropy was used to estimate the sediment resuspension probability, which was merged with the Camp model and generated the RETSED entropy model. To validate the RETSED model, an experimental check dam(ECD, 300 m3), located in the Gilbu es desertification site, Brazil, was monitored hourly during one hydrological year(July 2018-July 2019). Measurements show that the annual volumetric decline of the check dam was 12%; and that the average trap efficiency equaled 86%. Only 9.5% of the hourly sediment concentration outflow exceeded the average plus one standard deviation, showing that the reservoir is well mixed; a fact which highlights the relevance of the resuspension process. Three empirical models failed to mimic the experimental results: Churchill(TE = 99%), Brune(TE = 75%), and Maryland(TE = 94%). According to the RETSED entropy model, the resuspension probability during the experiment was 10%and TE = 81%, a value only 6% below the measured one. The Camp model simulated TE = 89%, only 3%higher than the measured value, but confirmed the Camp model’s tendency to overestimate TE due to a disregard of resuspension. The deterministic model showed low sensitivity concerning the hydrodynamic effects, whereas the entropy formulation proved to be more consistent with physical behavior: the resuspension probability clearly increased and trap efficiency decreased with rising reservoir discharge.

We report the application of a solid lanthanum-ceria-zinc catalyst in the catalytic regeneration of diesel particulate filters （DPF） in diesel engines. We synthesized a CeO2- ZnO-La2O3 （Ce-Zn-La） mixed oxide by a lactic acid-mediated sol-gel method, which efficiently coated cordierite substrates for soot capture and combustion. We studied the effects of La loading on the physicochemical and catalytic properties of Ce-Zn mixed oxide during lowtemperature soot combustion processes. We characterized the synthesized catalysts by X-ray diffraction （XRD）, Fourier-transform infrared spectroscopy （FTIR）, N2 adsorption, Raman spectroscopy, oxygen storage capacity （OSG）, and scanning and transmission electron microscopy （SEM and TEM）. Thermogravimetric and differential thermal analysis （TGA/DTA） confirmed that the catalysts effectively reduced the soot oxidation temperature. The ternary Ce-Zn-La mixed oxide catalyst with Ce/Zn/La atomic ratio of 2：1：0.5 had the highest catalytic activity and promoted soot oxidation at temperatures below 390℃. This indicated that the large number of oxygen vacancies in the catalyst structure generated oxygen species at low temperatures. Raman spectroscopy measurements revealed the presence of oxygen vacancies and lattice defects in Ce-Zn-La samples, which were key parameters concerning the stability and redox properties of the prepared catalysts.

We review the management of Eucalyptus species under a coppice-with-standards （CWS） silvicultural system. CWS management results in product diversification, permitting production of small and large scale timber from the same stand. Eucalyptus species are suitable candidates for CWS management because： there are large worldwide plantation areas, sprouting capacity is high, and eucalypts are multipur- pose species. We discuss （1） short rotation Eucalyptus coppice manage- ment for energy and pulping and （2） Eucalyptus seedling management for solid wood products. We review the literature and discuss experi- ences with Eucalyptus managed under the CWS system. We also assess projects dealing with Eucalyptus coppice management, stand density regulation, pruning, and stand and wood quality. The growth environ- ment of the standard trees （heavy competition up to the first harvest, free growth afterwards） coupled with long rotations （〉20 years） results in high quality logs for solid wood products. Early pruning should be ap- plied to enhance wood quality. We propose a system for the silvicultural management of Eucalyptus under the CWS system, elaborating on the consequences of initial planting density, site productivity, and standard tree densities as well as timing of basic silvicultural applications.

The rational design of emulsions requires study of the main factors that influence their formation, physicochemical properties and, consequently, stability and performance. The use of vegetable oils in the pharmaceutical and cosmetic industries has recently become attractive. Dipteryx alata Vogel(D. alata) is an oleaginous species native to Brazil. The seeds of this species contain highly unsaturated oil with significant amounts of tocopherols and phytosterols, representing an important source of agents capable of combatting oxidative processes. In this work, a lamellar gel phase emulsion using oil extracted from the seeds of D. alata(baru) was developed. The steps involved in the development of this research were as follows: 1) development of formulations and 2) in vitro assays by simulating the evaporation of the final product after application to the skin and Electron paramagnetic resonance spectroscopy(EPR) of fatty acid spin labels was used to investigate the profile of interaction of the dispersed systems with stratum corneum(SC) lipids. The results indicate that the developed system shows no signs of instability during the storage period. Moreover, EPR studies indicated that D. alata oil and especially the developed formulation were able to increase SC lipid fluidity and extract a fatty-acid spin label from the lipid domain structures of SC, demonstrating its potential to act as a drug or skin care vehicle.

The liver is a complex organ that performs several functions to maintain homeostasis.These functions are modulated by calcium,a second messenger that regulates several intracellular events.In hepatocytes and cholangiocytes,which are the epithelial cell types in the liver,inositol 1,4,5-trisphosphate(InsP3)receptors(ITPR)are the only intracellular calcium release channels.Three isoforms of the ITPR have been described,named type 1,type 2 and type 3.These ITPR isoforms are differentially expressed in liver cells where they regulate distinct physiological functions.Changes in the expression level of these receptors correlate with several liver diseases and hepatic dysfunctions.In this review,we highlight how the expression level,modulation,and localization of ITPR isoforms in hepatocytes and cholangiocytes play a role in hepatic homeostasis and liver pathology.
