Systematic review of health-related quality of life after esophagectomy for esophageal cancer

This study is aimed to assess the long-term healthrelated quality of life （HRQL） of patients after esophagectomy for esophageal cancer in comparison with established norms, and to evaluate changes in HRQL during the different stages of follow-up after esophageal resection. A systematic review was performed by searching medical databases （Medline, Embase and the Cochrane Library） for potentially relevant studies that appeared between January 1975 and March 2011. Studies were included if they addressed the question of HRQL after esophageal resection for esophageal cancer. Two researchers independently performed the study selection, data extraction and analysis processes. Twenty-one observational studies were included with a total of 1282 （12-355） patients. Five studies were performed with short form-36 （SF-36） and 16 with European Organization for Research and Treatment of Cancer （EORTC） QLQ c30 （14 of them also utilized the disease-specific OESI8 or its previous version OES24）.The analysis of long-term generic HRQL with SF-36 showed pooled scores for physical, role and social function after esophagectomy similar to United States norms, but lower pooled scores for physical function, vitality and general health perception. The analysis of HRQL conducted using the Global EORTC C30 global scale during a 6-mo follow-up showed that global scale and physical function were better at the baseline. The symptom scales indicated worsened fatigue, dyspnea and diarrhea 6 mo after esophagectomy. In contrast, however, emotional function had significantly improved after 6 mo. In conclusion, short- and long-term HRQL is deeply affected after esophagectomy for cancer. The impairment of physical function may be a long-term consequence of esophagectomy involving either the respiratory system or the alimentary tract. The short- and long-term improvement in the emotional function of patients who have undergone successful operations may be attributed to the impression that they have survive

The presence of a thick snowpack could interfere with forest stability, especially on steep slopes with potential damages for young and old stands. The study of snow gliding in forests is rather complex be- cause this phenomenon could be influenced not only by forest features, but also by snow/soil interface characteristics, site morphology, meteoro- logical conditions and snow physical properties. Our starting hypothesis is that different forest stands have an influence on the snowpack evolu- tion and on the temperature and moisture at the snow/soil interface, which subsequently could affect snow gliding processes and snow forces. The aim of this work is therefore to analyse the snowpack evolution and snow gliding movements under different forest covers, in order to deter- mine the snow forces acting on single trees.

Vineyard management practices to enhance soil conservation principally focus on increasing carbon(C)input,whereas mitigating impacts of disturbance through reduced tillage has been rarely ***,information is lacking on the effects of soil management practices adopted in the under-vine zone on soil *** this work,we evaluated the long-term effects(22 years)of alley with a sown cover crop and no-tillage(S+NT),alley with a sown cover crop and tillage(S+T),and under-vine zone with no vegetation and tillage(UV)on soil organic matter(SOM),microbial activity,aggregate stability,and their mutual interactions in a California vineyard in *** biomass,microbial biomass and activity,organic C and nitrogen(N)pools,and SOM size fractionation and aggregate stability were *** characteristics only partially reflected the differences in vegetation biomass *** C and N pools and microbial biomass/activity in S+NT were higher than those in S+T,while the values in UV were intermediate between the other two ***,S+NT also exhibited higher particulate organic matter C in *** differences were found in POM C between S+T and UV,but the POM fraction in S+T was characterized by fresher *** stability was decreased in the order:S+NT>UV>S+***,even if shallow and performed infrequently,had a negative effect on organic C and N pools and aggregate ***,the combination of a sown cover crop and reduced tillage still limited SOM accumulation and reduced aggregate stability in the surface soil layer of vineyards,suggesting relatively lower resistance of soils to erosion compared to no-till systems.

Idiopathic pulmonary fibrosis(IPF) is the most common and rapidly fatal among idiopathic interstitial pneumonias. Its clinical course is variable. A significant fraction of the population of patients display a slow disease course and can remain stable for years, while other patients show a rapid progressive course and may die within few months from diagnosis. For these reasons estimating prognosis of IPF patients is extremely difficult and has important clinical repercussions on optimal patients management including patients referral for lung transplantation. Several studies have tried to address this key point in the course of the two last decades analyzing different clinical, functional, radiological and biological variables. The purpose of this review is to assess relevant studies published on this subject and to examine the variety of prognostic predictors proposed along with staging systems.

Abdominosacral resection for locally recurring rectal cancer

AIM To investigate feasibility and outcome of abdominalsacral resection for treatment of locally recurrent rectal *** A population of patients who underwent an abdominalsacral resection for posterior recurrent adenocarcinoma of the rectum at the National Cancer Institute of Milano, between 2005 and 2013, is considered. Retrospectively collected data includes patient characteristics, treatment and pathology details regarding the primary and the recurrent rectal tumor surgical resection. A clinical and instrumental follow-up was performed. Surgical and oncological outcome were investigated. Furthermore an analytical review of literature was conducted in order to compare our case series with other reported *** At the time of abdomino-sacral resection, the mean age of patients was 55(range, 38-64). The median operating time was 380 min(range, 270-480). Sacral resection was performed at S2/S3 level in 3 patients, S3/S4 in 3 patients and S4/S5 in 4 patients. The median operating time was 380 ± 58 min. Mean intraoperative blood loss was 1750 m L(range, 200-680). The median hospital stay was 22 d. Overall morbidity was 80%, mainly type Ⅱ complication according to the ClavienDindo classification. Microscopically negative margins(R0) is obtained in all patients. Overall 5-year survival after first surgical procedure is 60%, with a mediansurvival from the first surgery of 88 ± 56 mo. The most common site of re-recurrence was *** Sacral resection represents a feasible approach to posterior rectal cancer recurrence without evidence of distant spreading. An accurate staging is essential for planning the best therapy.

Preimplantation genetic diagnosis allows to test the genetic status of embryos prior to implantation. In order to obtain genetic material, on which carry out a genetic diagnosis, a procedure named embryo biopsy is required. In the last two decades, embryo biopsy at the cleavage stage has been the mostly performed procedure. However, recently, alternative methods allowing the retrieval of a larger number of cells (blastocyst stage biopsy), or representing a valid alternative to overcome ethical issues (polar body biopsy) have obtained increasing consensus. This article reviews different methods of embryo biopsy and points out their positive and negative aspects.
