BACKGROUND Treatments involving stem cell(SC)usage represent novel and potentially interesting alternatives in facial nerve *** literature includes the use of SC in animal model studies to promote graft survival by enhancing nerve fiber growth,spreading,myelinization,in addition to limiting fibrotic degeneration after ***,the effectiveness of the clinical use of SC in facial nerve reanimation has not been clarified *** To investigate the histological,neurophysiological,and functional outcomes in facial reanimation using SC,compared to *** Our study is a systematic review of the literature,consistently conducted according to the preferred reporting items for systematic reviews and meta-analyses statement *** review question was:In facial nerve reanimation on rats,has the use of stem cells revealed as effective when compared to autograft,in terms of histological,neurophysiological,and functional outcomes?Random-effect meta-analysis was conducted on histological and neurophysiological data from the included comparative *** After screening 148 manuscript,five papers were included in our study.43 subjects were included in the SC group,while 40 in the autograft *** meta-analysis showed no significative differences between the two groups in terms of myelin thickness[CI:-0.10(-0.20,0.00);I^(2)=29%;P=0.06],nerve fibers diameter[CI:0.72(-0.93,3.36);I^(2)=72%;P=0.6],compound muscle action potential amplitude[CI:1.59(0.59,3.77);I^(2)=89%;P=0.15]and latency[CI:0.66(-1.01,2.32);I^(2)=67%;P=0.44].The mean axonal diameter was higher in the autograft group[CI:0.94(0.60,1.27);I^(2)=0%;P≤0.001].CONCLUSION The role of stem cells in facial reanimation is still relatively poorly studied,in animal models,and available results should not discourage their use in future studies on human subjects.

Effects of a general practitioner cooperative co-located with an emergency department on patient throughput

BACKGROUND:In 2013 a General Practitioner Cooperative(GPC) was introduced at the Emergency Department(ED) of our *** of the aims of this co-located GPC was to improve throughput of the remaining patients at the *** determine the change in patient flow,we assessed the number of self-referrals,redirection of self-referrals to the GPC and back to the ED,as well as ward and ICU admission rates and length of stay of the remaining ED ***:We conducted a four months' pre-post comparison before and after the implementation of a co-located GPC with an urban ED in the ***:More than half of our ED patients were *** triage,54.5%of these selfreferrals were redirected to the *** assessment at the GPC,8.5%of them were referred back to the *** number of patients treated at the ED declined with 20.3%after the introduction of the *** the remaining ED population,there was a significant increase of highly urgent patients(Period to 14 962 hours in the two months' intervention period,a total increase of 270 hours in two months(P<0.001).CONCLUSION:Introduction of a GPC led to efficient redirection of self-referrals but failed to improve throughput of the remaining patients at the ED.

Allergic rhinitis (AR) is the inflammation of nasal mucosa due to the type 1 hypersensitivity reactions mediated by immunoglobulin E (IgE) and triggered by certain allergens. The latest concept in allergic disease is the role of regulatory T cells (Treg). Interleukin-2 enhances the function and survival of Treg to perform its function as a controller of effector for forming a tolerant system by suppressing and regulating the homeostasis system. Treg has a transcription factor FoxP3 which plays a role in developing major function of Treg and progression to produce IL-10 and TGF-?. The atopic diseases are caused by a deficiency of Treg. The new perspective is low-dose IL-2 therapy towards autoimmune disease and allergic inflammation. Low-dose IL-2 therapy requires further clinical studies to optimize the dose, time, and the schedule of the IL-2 treatment. FoxP3 has the potential to assist in evaluating the active process of immunological process, which cannot be evaluated by Th1 and Th2 markers, and FoxP3 can be a successful immunotherapy marker.

Analysis of Forkhead Box Protein-3 (Foxp3) in Allergic Rhinitis Patients

Background: A new concept in understanding allergic diseases is regulatory T cells (Tregs), which control the immune reaction caused by Th2 cytokine production. Forkhead Box Protein 3 (FoxP3) is a marker that has a critical role in the development and function of Tregs. Some studies found differences in FoxP3 level and in Tregs capacity to control immune reactions in allergic diseases. The aim of this study was to investigate FoxP3 level in Allergic rhinitis (AR) patients compared to atopic and healthy/normal persons in Jakarta. Methodology: This study used observation to analyze the level of FoxP3 in AR, atopic and healthy/normal persons, and used ELISA to measure the FoxP3 level. Results: The study had sixty participants divided into three groups: 21 in the Normal group, 16 in the Atopic group, and 23 in the AR group. The mean FoxP3 levels were 0.81 ± 0.35 in the normal group, 3.42 ± 0.15 in the atopic group, and 3.40 ± 0.13 in the AR group. Statistical analysis with Mann-Whitney tests indicated significant differences, with AR and atopic groups having higher FoxP3 levels than the normal group, (p = 0.001), and no statistically significant differences between the AR and atopic groups, (p = 0.92). Conclusion: Our study results suggested that FoxP3 was active in the control of inflammatory processes due to allergies, and decrease level of FoxP3 indicated severe AR, but suggested another mechanism caused differences in the clinical phenotypes of AR and atopic patients, despite them having equally high levels of FoxP3.

AIM:To establish a population pharmacokinetic/pharma- codynamic(PK/PD)model for valproate(VPA)in chil- dren with epilepsy in China,and promote reasonable use of antiepileptie drug in clinical practice. METHODS:Sparse data of VPA serum concentrations from 417 pediatric children were *** patients were divided into three groups:Population PK-Index group,n=317;Population PK-Valid group,n=100;115 of the total 417 subjects were also included in the Population PD *** PK parameters of VPA were estimated based on the data from population PK-Index *** the validation procedure,the serum concentra- tions of VPA from the population PK-Valid group were pre- dicted by base and final models *** assess the accuracy and precision of the predictions,mean prediction error(MPE),mean square(t prediction error(MSPE), root mean squared prediction error(RMSPE),weight-resi- dues(WRES)and its 95% confidence intervals(95%CI) were all calculated,then compared between the two mode- *** population PD group,’all of the 115 patients were VPA *** of epilepsy treatment was di- vided into 5 grades according to the percentage of seizure frequency decreased(PSFD%).The value of PSFD% 100%,75%-100%,50%-75%,25%-50%,or less than 25 % are corresponding to grade 1 to *** pop- ulation PD group,the quantitative relationship between the VPA serum concentrations and the probability for its effica- cy score was characterized by logistic regression.
