m.chanical circulatory and/or respiratory assistance with extracorporeal m.m.rane oxygenation (ECm.) has becom. a standard of care for patients with circulatory (venoarterial) and/or respiratory (venovenous) failure refractory to standard therapies. Adult patients with congenital heart disease are an increasingly recognized and growing population and include various groups, such as undiagnosed cases in childhood and palliated and/or corrected cases, which require subsequent care because of residual lesions, cardiac arrest/insufficiency, and arrhythm.as, am.ng other conditions. In addition, these patients are prone to developing pathologies that are typical of adulthood with a generally increased risk of m.rbidity and m.rtality because of their low reserves and organic dam.ge associated with the underlying heart disease, which m.kes them.candidates for ECm.. These patients represent an additional challenge in this therapy because m.lform.tions and the presence of a shunt can generally affect the usual cannulation m.thods and hem.dynam.c and oxim.try m.nitoring. Thus, the configuration decision m.st be m.de on a case-by-case basis. Here, we present a cannulation m.thod, venopulm.nary artery ECm., which provides hem.dynam.c and respiratory support, and m.y be ideal for patients with shunts and/or right ventricular dysfunction. To our knowledge, this is the first report of this configuration in patients with congenital heart diseases.

Alzheim.r’s disease is a progressive neurodegenerative disorder, characterized by deposition of am.loid beta, neurofibrillary tangles, astrogliosis and m.crogliosis, leading to neuronal dysfunction and loss in the brain. Current treatm.nts for Alzheim.r’s disease prim.rily focus on enhancem.nt of cholinergic transm.ssion. However, these treatm.nts are only sym.tom.tic, and no disease-m.difying drug is available for Alzheim.r’s disease patients. This review will provide an overview of the proven antioxidant, anti-inflam.atory, anti-am.loidogenic, neuroprotective, and cognition-enhancing effects of curcum.n and apigenin and discuss the potential of these com.ounds for Alzheim.r’s disease prevention and treatm.nt. We suggest that these com.ounds m.ght delay the onset of Alzheim.r’s disease or slow down its progression, and they should enter clinical trials as soon as possible.

We report the case of a 67-year-old fem.le who presented with a large renal m.ss. Gross exam.nation of the nephrectom. specim.n dem.nstrated a 6-cm.renal m.ss that invaded into the renal sinus and perinephric fat. Histologic exam.nation revealed two distinct tum.r types. The first type was a conventional(clear cell) renal cell carcinom. that was of low nuclear grade and com.rised the m.nority of the overall tum.r. The second type was a high-grade collecting duct carcinom. with glandular/tubular differentiation and com.osed the m.jority of the tum.r. Im.unohistochem.cal studies dem.nstrated distinctive patterns of the two tum.r types, thus confirm.ng two distinct lineages. Five m.nths postoperatively, the patient developed m.tastasis to the lungs and right hilar lym.h node region. A fine needle aspiration of a lung nodule dem.nstrated a m.tastatic, poorly differentiated carcinom., sim.lar to the collecting duct carcinom. com.onent in the kidney. Collision tum.rs of the kidney are rare with fewer than 10 cases reported in the literature. Our report further expands the spectrum.of this rare phenom.non.

Background: Physical activity(PA) during pregnancy provides physical and psychological benefits for m.ther and child. U.S. guidelines recom.end ≥30 m.n of m.derate exercise for healthy pregnant wom.n m.st days of the week; however, m.st wom.n do not m.et these recom.endations. Theory assists in identifying salient determ.nants of health behavior to guide health prom.tion interventions; however, the application of theory to exam.ne PA am.ng pregnant wom.n has not been exam.ned cohesively am.ng m.ltiple levels of influence(e.g.,intrapersonal, interpersonal, neighborhood/environm.ntal, and organizational/political). Subsequently, this system.tic review aim. to identify and evaluate the use of health behavior theory in studies that exam.ne PA during ***: Articles published before July 2014 were obtained from.Pub m.d and Web of Science. Inclusion criteria applied were:(1) em.iricallybased;(2) peer-reviewed;(3) m.asured factors related to PA;(4) com.rised a pregnant sam.le; and(5) applied theory. Fourteen studies were included. Each study's application of theory and theoretical constructs were ***: Various theories were utilized to explain and predict PA during pregnancy; yet, the m.jority of these studies only focused on intrapersonal level determ.nants. Five theoretical fram.works were applied across the studies—all but one at the intrapersonal level. Few determ.nants identified were from.the interpersonal, neighborhood/environm.ntal, or organizational/political ***: This system.tic review synthesized the literature on theoretical constructs related to PA during pregnancy. Interpersonal, com.unity,and societal levels rem.in understudied. Future research should em.loy theory-driven m.lti-level determ.nants of PA to reflect the interacting factors influencing PA during this critical period in the life course.

Background: We hypothesised that supplem.ntation of green tea extract(GTE) in dairy cows during the transition period can attenuate proinflam.atory conditions and prevent endoplasm.c reticulum.ER) stress in the liver of these cows. Thirty Holstein cows with an average parity of 3.06(± 1.31, SD) were divided into a control group and a group that received a daily am.unt of 10 g of GTE from.d 7 before the calving day and a daily am.unt of 20 g of GTE from.the day of calving until d 7 of ***: Cows supplem.nted with GTE did not show differences in energy intake or m.lk yield in weeks 2–7 of lactation. However, these cows had a lower m.lk fat concentration and a lower energy corrected m.lk yield than the control cows and showed a trend of im.roved energy balance. The relative m.RNA concentrations of proinflam.atory genes, genes involved in the acute phase reaction and antioxidant genes in the liver in weeks 1, 4 and 7 of lactation were not different between the two groups of cows. The concentrations of α-tocopherol and the Trolox equivalent antioxidant capacity in plasm. were not different between the two groups. However, the group supplem.nted with GTE showed significant reductions of som. genes of the unfolded protein response(UPR) in week 1 and a trend of lower liver triacylglycerol(TAG) concentrations in the liver com.ared to the control ***: This study shows that supplem.ntation of GTE in dairy cows lowers the fat concentration in the m.lk but overall has no effect on the expression of inflam.atory genes and the antioxidative status in dairy cows during early lactation. The finding of reduced m.RNA levels of genes involved in the UPR at week 1, however, supports other results showing that supplem.ntation of polyphenols could prevent the developm.nt of ER stress in the liver of cows during early lactation. Background: Com.unity-based health insurance (CBHI) schem.s are increasingly im.lem.nted in low-incom. settings. These schem.s lim.t the coverage they offer both by the types of care considered, and by applying thresholds and/or caps to costs reim.ursed. The consequences of these thresholds and/or caps on insurance coverage have hitherto been usually ignored, for lack of data on the distributions of healthcare costs or understanding of their im.act on effective coverage levels. This article describes a theoretical m.del to obtain the distributions even without data collection in the field, and dem.nstrates the quantitative im.act of thresholds and/or caps on claim.reim.ursem.nts. m.thods: This m.del applies to applications on healthcare expenditures in low-incom. settings, following research m.thods exam.ned in the Western world. We looked at hospitalizations and tests;we com.ared the sim.lated distributions to em.irical data obtained through 11 household surveys conducted between 2008 and 2010 in rural locations (9 in India and 2 in Nepal). Results: We found that the shape of the distributions was very sim.lar in all locations for both benefits, and could be represented by a m.del based on a lognorm.l distribution. The agreem.nt between theoretical and em.irical results was satisfactory (m.stly within 10% difference). Conclusions: The m.del m.kes it possible to sim.late the expected perform.nce of the CBHI (represented by the percentage of costs or bills covered). The aim.is to m.tch costs with local levels of willingness-to-pay for health insurance. This m.del m.kes it possible to determ.ne at the stage of package-design the optim.l levels of thresholds and/or caps for each benefit-type included.
