Background:Vestibular migraine(VM)is a common cause of dizziness that is underrecognized,underdiagnosed,and challenging to effectively *** prevalence,appropriate diagnostic workup,and therapies for VM in low-and lower-middle-income countries(LLMICs)remain *** objective of this scoping review is to evaluate the current state of VM research in ***:PubMed,Embase,and Web of Science databases were searched to execute a scoping review of ***“vestibular diseases”and“vertigo”were used in combination with terms referring to LLMICs as defined by the World *** and abstract screening,full-text review,and data collection were conducted by two authors ***:Twenty-six studies were included in the scoping *** studies were cross-sectional(57.7%)or case studies/series(23.1%)and were conducted in urban settings only(92.3%).Geographic distribution of studies was skewed,with 65.4%of articles originating from *** prevalence of VM among clinic patients ranged from 0.3%to 33.3%.VM most frequently presented as headache,nausea and vomiting,and *** imaging,audiometry,and electronystagmography/videonystagmography were the three most commonly utilized diagnostic modalities in the dizziness *** most studied pharmacotherapies included calcium channel blockers,followed by beta-blockers and *** studies and series discussed less common VM pharmacotherapies,such as ayurvedic ***:There is a need for more VM research in LLMICs,including innovative diagnostic approaches and therapies that can improve VM care *** partnerships between LLMIC and highincome country researchers must expand vestibular research capacity and productivity in LLMICs.

Cost of open access publishing in otolaryngology‐head and neck surgery

Objective:Open access(OA)publishing makes research more accessible but is associated with steep article processing charges(APCs).The study objective was to characterize the APCs of OA publishing in otolaryngology‐head and neck surgery(OHNS)***:We conducted a cross‐sectional analysis of published policies of 110 OHNS journals collated from three *** primary outcomes were the publishing model,APC for original research,and APC waiver ***:We identified 110 OHNS journals(57 fully OA,47 hybrid,2 subscription‐only,4 unknown model).After excluding 12 journals(2 subscription‐only,4 unknown model,5 OA with unspecified APCs,and 1 OA that accepts publications only from society members),we analyzed 98 journals,23 of which did not charge *** 75 journals with nonzero APCs,the mean and median APCs were$2452 and$2900(interquartile range:$1082–3520).Twenty‐five journals(33.3%)offered APC subsidies for authors in low‐and middle‐income countries(LMICs)and/or on a case‐by‐case ***‐five and 25 journals were based in high‐income countries(HICs)and LMICs,*** mean APC was higher among HIC journals than LMIC journals($2606 vs.$958,p<0.001).Conclusion:APCs range from tens to thousands of dollars with limited waivers for authors in LMICs.

Impact of an oligosaccharide-based polymer on the metabolic profiles and microbial ecology of weanling pigs experimentally infected with a pathogenic ***

Background Our previous study has reported that supplementation of oligosaccharide-based polymer enhances gut health and disease resistance of pigs infected with enterotoxigenic ***(ETEC)F18 in a manner similar to *** objective of this study was to investigate the impacts of oligosaccharide-based polymer or antibiotic on the host metabolic profiles and colon microbiota of weaned pigs experimentally infected with ETEC *** Multivariate analysis highlighted the differences in the metabolic profiles of serum and colon digesta which were predominantly found between pigs supplemented with oligosaccharide-based polymer and *** relative abundance of metabolic markers of immune responses and nutrient metabolisms,such as amino acids and carbohydrates,were significantly differentiated between the oligosaccharide-based polymer and antibiotic groups(q2.0).In addition,pigs in antibiotic had a reduced(P<0.05)relative abundance of Lachnospiraceae and Lactobacillaceae,whereas had greater(P<0.05)Clostridiaceae and Streptococcaceae in the colon digesta on d 11 post-inoculation(PI)compared with d 5 *** The impact of oligosaccharide-based polymer on the metabolic and microbial profiles of pigs is not fully understood,and further exploration is ***,current research suggest that various mechanisms are involved in the enhanced disease resistance and performance in ETEC-challenged pigs by supplementing this polymer.

In the United States(US),the shift in the treatment paradigm following the introduction of second generation direct-acting antiviral(DAA)agents for hepatitis C virus(HCV)infection has favorably transformed the landscape of end-stage liver disease(ESLD)with a rapid decline in HCV-related liver transplantation;HCV had been the leading indication for liver transplantation for the last two decades(1).While the rate of liver transplantation in patients with HCV infection has plummeted in the US following the widespread application of DAA-based regimens in the pre-transplant setting,the rise in alcohol use disorder and obesity has catapulted alcoholic liver disease(ALD)to become the leading indication for liver transplantation in the US in 2016 followed closely by nonalcoholic steatohepatitis(NASH),a progressive subset of nonalcoholic liver disease(NAFLD)(1).Despite the decline in HCV-related liver transplantation in the US,the total number of liver transplants per year are on an upward trajectory as a result of marked increase in ESLD associated with ALD and NASH—an ominous trend.

Supplementation of oligosaccharide-based polymer enhanced growth and disease resistance of weaned pigs by modulating intestinal integrity and systemic immunity

Clinical epidemiology and disease burden of nonalcoholic fatty liver disease

Nonalcoholic fatty liver disease(NAFLD) is defined as the presence of hepatic fat accumulation after the exclusion of other causes of hepatic steatosis, including other causes of liver disease, excessive alcohol consumption, and other conditions that may lead to hepatic steatosis. NAFLD encompasses a broad clinical spectrum ranging from nonalcoholic fatty liver to nonalcoholic steatohepatitis(NASH), advanced fibrosis, cirrhosis, and finally hepatocellular carcinoma(HCC). NAFLD is the most common liver disease in the world and NASH may soon become the most common indication for liver transplantation. Ongoing persistence of obesity with increasing rate of diabetes will increase the prevalence of NAFLD, and as this population ages, many will develop cirrhosis and end-stage liver disease. There has been a general increase in the prevalence of NAFLD, with Asia leading the rise, yet the United States is following closely behind with a rising prevalence from 15% in 2005 to 25% within 5 years. NAFLD is commonly associated with metabolic comorbidities, including obesity, type Ⅱ diabetes, dyslipidemia, and metabolic syndrome. Our understanding of the pathophysiology of NAFLD is constantly evolving. Based on NAFLD subtypes, it has the potential to progress into advanced fibrosis, end-stage liver disease and HCC. The increasing prevalence of NAFLD with advanced fibrosis, is concerning because patients appear toexperience higher liver-related and non-liver-related mortality than the general population. The increased morbidity and mortality, healthcare costs and declining health related quality of life associated with NAFLD makes it a formidable disease, and one that requires more in-depth analysis.

New therapeutic strategies targeting D1-type dopamine receptors for neuropsychiatric disease

The neurotransmitter dopamine acts via two major classes of receptors, Dl-type and D2-type. D1 receptors are highly expressed in the striatum and can also be found in the cerebral cortex. Here we review the role of D1 dopamine signaling in two major domains： L-DOPA-induced dyskinesias in Parkinson＇s disease and cognition in neuropsychiatric disorders. While there are many drugs targeting D2-type receptors, there are no drugs that specifically target D1 receptors. It has been difficult to use selective Dl-receptor agonists for clinical applications due to issues with bioavailability, binding affinity, pharmacological kinetics, and side effects. We propose potential therapies that selectively modulate D1 dopamine signaling by targeting second messengers downstream of D1 receptors, aUosteric modulators, or by making targeted modifications to Dl-receptor machinery. The development of therapies specific to Dl-receptor signaling could be a new frontier in the treatment of neurological and psychiatric disorders.

The use of 5-alpha reductase inhibitors in the treatment of benign prostatic hyperplasia

Benign prostatic hyperplasia(BPH)is characterized by an enlarged prostate,lower urinary tract symptoms(LUTS),and a decreased urinary flow *** in older men,BPH is a progressive disease that can eventually lead to complications including acute urinary retention(AUR)and the need for BPH-related *** normal and abnormal prostate growth is driven by the androgen dihydrotestosterone(DHT),which is formed from testosterone under the influence of 5-alpha ***,5-alpha reductase inhibitors(5-ARIs)effectively reduce the serum and intraprostatic concentration of DHT,causing an involution of prostate *** 5-ARIs are currently available for the treatment of BPHdfinasteride and *** have been demonstrated to decrease prostate volume,improve LUTS and urinary flow rates,which ultimately reduces the risk of AUR and BPH-related ***,either alone or in combination with other BPH medications,5-ARIs are a mainstay of BPH management.

In crisis management, cross-sector collaboration exercises are perceived as improving preparedness and develop team-integration efforts. However, studies show that exercises may tend to produce results with limited learning and usefulness. The purpose of this nonexperimental, survey-based study was to measure the difference in perceived exercise effect between participants belonging to the exercise planning organizations and participants belonging to other participating groups. Surveys were distributed and collected from participants in a 2017 chemical oil-spill exercise set off the southern coast of Norway. The target population was operational staff,excluding exercise management and directing staff. The sample population consisted of operatives associated with the exercise organizer organization and others belonging to external public and nongovernmental emergency organizations. The data collection instrument was the 'Collaboration, Learning, and Utility Scale’’(CLU-scale). Findings indicated that the levels of CLU were higher among external participants than among those individuals who belong to the exercise planning organizations. This study recommends the development and adoption of a national maritime collaboration exercise framework. A practical implication is a recommendation to evaluate exercises to secure the outcome regarding collaboration skill using the same instrument.
