Retrospective analysis of hepatitis C infected patients treated through an integrated care model

AIM To determine if our health system's integrated model reflects sustained virologic response(SVR) outcomes similar to those in clinical trial data, maximizes adherence, and averts drug *** Subjects with chronic hepatitis C had their medical records reviewed from November 1st, 2014 through March 1st, 2016. Patients eligible for treatment were entered into an integrated care model therapy algorithm. The primary outcome was SVR12 based on intention to treat(ITT) analysis. Inclusion criteria consisted of both treatment na?ve and experienced patients over the age of 18 who were at least twelve weeks post-therapy completion with any genotype(GT) or METAVIR score. Secondary outcomes included adherence, adverse events, and number of drug interaction *** At the time of analysis, 133 patients had reached twelve weeks post therapy with ITT. In the ITT analysis 70 patients were GT 1a, 26 GT 1b, 23 could not be differentiated between GT 1a or 1b, 8 GT 2, 4 GT 3, and 2 patients with multiple genotypes. The ITT treatment regimens consisted of 97 sofosbuvir(SOF)/ledipasvir(LdV), 8 SOF/LdV and ribavirin(RBV), 7 SOFand Simeprevir(SMV), 6 3d and RBV, 1 3d, 11 SOF and RBV, and 1 SOF, peg interferon alpha, and RBV. The overall SVR12 rate was 93% in the ITT analysis with a total of 6 patients relapsing. In patients with cirrhosis, 89% obtained SVR12. All 33 patients who were previous treatment failures achieved SVR12. drug-drug interactions were identified in 56.4% of our patient population, 69 of which required interventions made by the pharmacist. The most common side effects were fatigue(41.4%), headache(28.6%), nausea(18.1%), and diarrhea(8.3%). Knowledge of nutrients effect on freezing tolerance is vital for protecting tea plants against cold injury(CI).Freezing injury treatments on tea leaves with different low temperature and nitrogen(N)concentration was evaluated by hyperspectral imaging based on the reflectance as potential analytical ***,quantitative evaluation of the CI was improved by comparing analyzed results using partial least squares regression(PLSR),principle component regression(PCR)and linear model(LM)*** of the CI was obtained by hyperspectral imaging system in the band between 871 and 1766 *** results showed that average reflectance increases with the rise of N concentration.A substantial portion of the leaf from the plant with 100%N dosage had the darkest image and resulted in the highest reflectance because the N weakened the negative effect of freezing *** of the five spectral domains tested,the best predictive accuracy for the CI of the tea leaf was achieved by PCR(R^(2)=0.9971,RMSE=0.0609)in 1410–1766 nm wavelength,followed by LM(R^(2)=0.9999,RMSE=0.0805)in 871–1000 *** whole interval had the worse prediction accuracy which could be caused by large variations in the data for a specific treatment and high absorption peak occurring around 1450 nm in the reflectance *** averages,R^(2) and RMSE for all the three statistical models showed that the worse prediction accuracy occurred in 1410–1766 nm,followed by the whole interval 871–1766 *** prediction accuracy was low which could be due to the strong water absorption peaks that appeared in both range of *** models without absorption bands had improved correlation coefficient and reduced RMSE values between the measured and the predicted CI(R^(2)=0.9676;RMSE=0.3067)compared to the model developed with the reflectance values in the entire waveband(R^(2)=0.9504;RMSE=0.6629).The reflectance values where absorption bands occurred had detrimental effect on the model performance,which

Structural phase-change materials are of great importance for applications in information storage *** driven structural phase transitions are employed in phase-change memory to achieve lower programming voltages and potentially lower energy consumption than mainstream nonvolatile memory ***,the waste heat generated by such thermal mechanisms is often not optimized,and could present a limiting factor to widespread *** potential for electrostatically driven structural phase transitions has recently been predicted and subsequently reported in some two-dimensional materials,providing an athermal mechanism to dynamically control properties of these materials in a nonvolatile fashion while achieving potentially lower energy *** this work,we employ dFT-based calculations to make theoretical comparisons of the energy required to drive electrostatically-induced and thermally-induced phase *** theoretical limits in monolayer MoTe2 and thin films of Ge_(2)Sb_(2)Te_(5),we find that the energy consumption per unit volume of the electrostatically driven phase transition in monolayer MoTe2 at room temperature is 9% of the adiabatic lower limit of the thermally driven phase transition in Ge_(2)Sb_(2)Te_(5).Furthermore,experimentally reported phase change energy consumption of Ge_(2)Sb_(2)Te_(5) is 100–10,000 times larger than the adiabatic lower limit due to waste heat flow out of the material,leaving the possibility for energy consumption in monolayer MoTe2-based devices to be orders of magnitude smaller than Ge_(2)Sb_(2)Te_(5)-based devices.

Indirect traumatic optic neuropathy(ITON) refers to optic nerve injury resulting from impact remote to the optic nerve. The mechanism of injury is not understood, and there are no confirmed protocols for prevention, mitigation or treatment. Most data concerning this condition comes from case series of civilian patients suffering blunt injury, such as from sports- or motor vehicle-related concussion, rather than military-related ballistic or blast damage. Research in this field will likely require the development of robust databases to identify patients with ITON and follow related outcomes, in addition to both in-vivo animal and virtual human models to study the mechanisms of damage and potential therapies.

A new approach to solar flare prediction

All three components of the current density are required to compute the heating rate due to free magnetic energy *** we present a first test of a new model developed to determine if the times of increases in the resistive heating rate in active region(AR)photospheres are correlated with the subsequent occurrence of M and X flares in the corona.A data driven,3d,non-force-free magnetohydrodynamic model restricted to the near-photospheric region is used to compute time series of the complete current density and the resistive heating rate per unit volume[Q(t)]in each pixel in neutral line regions(NLRs)of 14 *** model is driven by time series of the magnetic field B measured by the Helioseismic&Magnetic Imager on the Solar dynamics Observatory(SdO)*** doppler periods due to SdO orbital motion are filtered out of the time series for B in every AR *** each AR,the cumulative distribution function(CdF)of the values of the NLR area integral Qi(t)of Q(t)is found to be a scale invariant power law distribution essentially identical to the observed CdF for the total energy released in coronal *** suggests that coronal flares and the photospheric Qi are correlated,and powered by the same *** model predicts spikes in Qi with values orders of magnitude above background *** spikes are driven by spikes in the non-force free component of the current *** times of these spikes are plausibly correlated with times of subsequent M or X flares a few hours to a few days *** spikes occur on granulation scales,and may be signatures of heating in horizontal current *** is also found that the times of relatively large values of the rate of change of the NLR unsigned magnetic flux are also plausibly correlated with the times of subsequent M and X flares,and spikes in Qi.

Utilizing dimensional Analysis with Observed data to determine the Significance of Hydrodynamic Solutions in Coastal Hydrology

In this paper, the authors present an analysis of the magnitude of the temporal and spatial acceleration (inertial) terms in the surface-water flow equations and determine the conditions under which these inertial terms have sufficient magnitude to be required in the computations. data from two South Florida field sites are examined and the relative magnitudes of temporal acceleration, spatial acceleration, and the gravity and friction terms are compared. Parameters are derived by using dimensionless numbers and applied to quantify the significance of the hydrodynamic effects. The time series of the ratio of the inertial and gravity terms from field sites are presented and compared with both a simplified indicator parameter and a more complex parameter called the Hydrodynamic Significance Number (HSN). Two test-case models were developed by using the SWIFT2d hydrodynamic simulator to examine flow behavior with and without the inertial terms and compute the HSN. The first model represented one of the previously-mentioned field sites during gate operations of a structure-managed coastal canal. The second model was a synthetic test case illustrating the drainage of water down a sloped surface from an initial stage while under constant flow. The analyses indicate that the times of substantial hydrodynamic effects are sporadic but significant. The simplified indicator parameter correlates much better with the hydrodynamic effect magnitude for a constant width channel such as Miami Canal than at the non-uniform North River. Higher HSN values indicate flow situations where the inertial terms are large and need to be taken into account.

Seven new azaphilone pigments named lenormandins A–G were isolated from stromata(fruiting bodies)of the xylariaceous fungus,Hypoxylon lenormandii using preparative High Performance Liquid Chromatography(HPLC)and their structures were elucidated by NMR spectroscopy,high resolution mass spectrometry and other spectral *** occurrence in herbarium specimens(including various type materials collected in the 19th and early 20th century)and several fresh collections was studied by analytical HPLC with diode array and mass spectrometric detection(HPLC-dAd/MS),revealing that this group of pigments is specific for Hypoxylon lenormandii from various geographic regions and otherwise only occurs in closely related taxa,*** and an undescribed fungus from Sri Lanka,for which we propose the name *** status of the new species was established by using molecular phylogenetic data based on ITS and partial beta-tubulin sequences,and by detailed morphological studies.

Bismuth telluride nanostructures： preparation, thermoelectric properties and topological insulating effect

Bismuth telluride is known to wield unique properties for a wide range of device applications. However, as devices migrate to the nanometer scale, significant amount of studies are being conducted to keep up with the rapidly growing nanotechnological field. Bi2Te3 possesses distinctive properties at the nanometer level from its bulk material. Therefore, varying synthesis and characterization techniques are being employed for the realization of various Bi2Te3 nanostructures in the past years. A considerable number of these works have aimed at improving the thermoelectric （TE） figure-of-merit （ZT） of the Bi2Te3 nanostructures and drawing from their topological insulating properties. This paper reviews the various Bi2Te3 and Bi2Te3-based nanos- tructures realized via theoretical and experimental procedures. The study probes the preparation techniques, TE properties and the topological insulating effects of 0d, 1d, 2d and Bi2Te3 nanocomposites. With several applications as a topological insulator （TI）, the topological insulating effect of the Bi2Te3 is reviewed in detail with the time reversal symmetry （TRS） and surface state spins which characterize TIs. Schematics and preparation methods for the various nanostructural dimensions are accordingly categorized.
