BACKGROUND: Although low central venous pressure (CVP) has been used to minimize blood loss during hepatectomy the impact of variations of CVP on the rate of blood loss and on the perfusion of end-organs has not been evaluated This animal study aimed to evaluate the hemodynamics and oxygen transport changes during hepatic resection at different CVP levels. METHODS: Forty-eight anesthetized Bama miniature pigs were divided into 8 groups with CVP during hepatic resection controlled at 0 to <1, 1 to <2, 2 to <3, 3 to <4, 4 to <5, 5 to <6, 6 to <7, and 7 to <8 cmH 2 O. Intergroup comparisons were made for hemodynamic parameters, oxygen transport dynamics, and the rate of blood loss. RESULTS: The rate of blood loss and the hepatic venous pressure during hepatic resection were almost linearly related to the CVP. A significant drop in the mean arterial pressure cardiac output, and cardiac index occurred between CVP ≥2 and <2 cmH 2 O. Oxygen delivery (DO 2 ), oxygen consumption (VO 2 ) and oxygen extraction ratio (ERO 2 ) remained relatively constant between CVPs of 2 to <8 cmH 2 O. There was a significant drop in DO 2 when the CVP was <2 cmH 2 O. There was also a significant drop in VO 2 and ExO 2 when the CVP was <1 cmH 2 ***: The optimal CVP for hepatic resection is 2to 3 cmH2O.

In this paper,we present an efficient computational methodology for diffusion and convection-diffusion problems in highly heterogeneous media as well as convection-dominated diffusion *** is well known that the numerical computation for these problems requires a significant amount of computermemory and ***,the solutions to these problems typically contain a coarse component,which is usually the quantity of interest and can be represented with a small number of degrees of *** are many methods that aim at the computation of the coarse component without resolving the full details of the *** proposed method falls into the framework of interior penalty discontinuous Galerkin method,which is proved to be an effective and accurate class of methods for numerical solutions of partial differential equations.A distinctive feature of our method is that the solution space contains two components,namely a coarse space that gives a polynomial approximation to the coarse component in the traditional way and a multiscale space which contains sub-grid structures of the solution and is essential to the computation of the coarse *** addition,stability of the method is *** numerical results indicate that the method can accurately capture the coarse behavior of the solution for problems in highly heterogeneous media as well as boundary and internal layers for convection-dominated problems.

BACKGROUND： Associating liver partition and portal vein ligation for staged hepatectomy （ALPPS） has recently been developed to induce rapid liver hypertrophy and reduce posthepatectomy liver failure in patients with insufficient future liver remnant （FLR）. ALPPS is still considered to be in an early developmental phase because surgical indications and techniques have not been standardized. This article aimed to review the current role and future developments of ALPPS. DATA SOURCES： Studies were identified by searching MED- LINE and PubMed for articles from January 2007 to October 2016 using the keywords ＂associating liver partition and portal vein ligation for staged hepatectomy＂ and ＂ALPPS＂ Addi- tional papers were identified by a manual search of references from key articles. RESULTS： ALPPS induces more hypertrophy of the FLR in less time than portal vein embolization or portal vein ligation. The benefits of ALPPS include rapid hypertrophy 47%-110% of the liver over a median of 6-16.4 days, and 95%-100% com- pletion rate of the second stage of ALPPS. The main criticisms of ALPPS are centered on its high morbidity and mortality rates. Morbidity rates after ALPPS have been reported to be 15.3%-100%, with ≥ the Clavien-Dindo grade III morbidity of 13.6%-44%. Mortality rates have been reported to be 0%-29%. The important questions to ask even if oncologic long-term results are acceptable are： whether the gain in quality and quantity of life can be off balance by the substantial risks of morbidity and mortality, and whether stimulation of rapid liver hypertrophy also accelerates rapid tumor progression and spread. Up till now, the documentations of the ALPPS procedure come mainly from case series, and most of these series include heterogeneous groups of malignancies. The numbers are also too small to separately evaluate survival for different tumor etiologies. CONCLUSIONS： Currently, knowledge on ALPPS is limited, and prospective randomized studies are lacking. Phenomenology ordinarily signifies the investigation （logos） of that which appears （phainomenon）. It is frequently understood, outside of phenomenology as a philosophical movement, to be experiential description from the first person perspective. Critics of phenomenology, including contemporary critics as diverse as Daniel Dennett and John Searle, as well as speculative realism and object oriented ontology, construe it as being intrinsically subjective, idealistic, and trapped in the first-person point of view （Dennett 2003, 19-30; Searle 2008, 107-36; Sparrow 2014; Zahavi 289-309）.

Robotic distal pancreatectomy versus conventional laparoscopic distal pancreatectomy： a comparative study for short-term outcomes

Robotic system has been increasingly used in pancreatectomy. However, the effectiveness of this method remains uncertain. This study compared the surgical outcomes between robot-assisted laparoscopic distal pancreatectomy and conventional laparoscopic distal pancreatectomy. During a 15-year period, 35 patients underwent minimally invasive approach of distal pancreatectomy in our center. Seventeen of these patients had robot-assisted laparoscopic approach, and the remaining 18 had conventional laparoscopic approach. Their operative parameters and perioperative outcomes were analyzed retrospectively in a prospective database. The mean operating time in the robotic group （221.4 min） was significantly longer than that in the laparoscopic group （173.6 min）（P = 0.026）. Both robotic and conventional laparoscopic groups presented no significant difference in spleen-preservation rate （52.9% vs. 38.9%）（P = 0.505）, operative blood loss （100.3 ml vs. 268.3 ml）（P = 0.29）, overall morbidity rate （47.1% vs. 38.9%）（P = 0.73）, and post-operative hospital stay （11.4 days vs. 14.2 days）（P = 0.46）. Both groups also showed no perioperative mortality. Similar outcomes were observed in robotic distal pancreatectomy and conventional laparoscopic approach. However, robotic approach tended to have the advantages of less blood loss and shorter hospital stay. Further studies are necessary to determine the clinical position of robotic distal pancreatectomy.
