One-year experience in the retinopathy of prematurity: frequency and risk factors, short-term results and follow-up

· AIM: As a result of the increase in premature births and the advances in neonatal intensive care, retinopathy of prematurity (ROP) remains one of the most important causes of childhood blindness worldwide. The main factors in the development of ROP are gestational age, birth weight and oxygen therapy. ROP continues to gain importance due to the increasing survival rates of more immature babies. · METHODS: Between January 2007 and October 2008, 203 premature infants treated at the Neonatal Intensive Care Unit (NNICU) were prospectively enrolled and the relationship between known risk factors and the occurance of ROP was studied. · RESULTS: ROP in various stages developed in 86 cases (42.4%). Statistically significant correlations were found between the development of ROP and birth weight (P < 0.0001) gestational age (P <0.0001), oxygen treatment and its duration (P <0.0001 and P =0.002), mechanical ventilation (MV) and its duration (P =0,0001 and P =0.0001), apnea(P = 0.001), intraventricular hemorrhage (IVH) (P =0.046), sepsis (P =0.0001), use of erythropoietin (EPO) (P =0.003), the number of blood transfusions and frequency (P =0.0001 and P =0.0001), surfactant application (P =0.0001), the presence of patent ductus arteriosus (PDA) (P =0.001) or bronchopu- lmonary dysplasia (BPD) (P =0.0001). No significant correlations were found between the occurance of ROP and maternal pre-eclampsia (P =0.293), multiple pregnancy (P = 0.218), or hyperbilirubinemia (P =0.494). Severity of ROP was related significantly with birth weight (P =0.0001), but no significant correlation between severity of ROP and gestational age was present. · CONCLUSION: Early description and reduction of the risk factors related with the occurance of ROP with the help of routine screening programs may warrant the prevention of visual loss, however early ophthalmic diagnosis and treatment are still mandatory to provide better visual rehabilitation. ·

BACKGROUND:Primary hepatic carcinosarcoma is a rare malignant tumor containing an intimate mixture of carci- nomatous and sarcomatous *** on risk factors, epidemiology,and pathogenesis of the tumor as well as the experience in its treatment are limited. METHOD:We present a case of primary carcinosarcoma of the liver in a 69-year-old man who complained of right hypochondrial pain and weight loss for two months. RESULTS:Magnetic resonance imaging revealed a 14×12 cm mass in segments 7-8 and 4 of the liver with vena hepatica *** ultrasonography-guided biopsy showed osteoid tissue without osteoblastic *** structures accompanied the osteoid *** patient underwent surgery after a diagnosis of *** frozen sections revealed a carcinosarcoma associated with an osteosarcoma and cholangiocellular carcinoma components. CONCLUSIONS:Preoperative diagnosis of this rare primary hepatic malignant tumor may be difficult by biopsy owing to intratumoral *** mature areas of the osteosarcomatous component may lead to misdiagnosis of metaplastic bone *** features of this rare entity are discussed.

High Energy Studies on the Non-Thermal Emissions from Cassiopeia A &Tycho

Chandra X-ray Observatory and Fermi Gamma Ray Space Telescope analysis of young supernova remnants (SNRs) Cassiopeia A (Cas A) and Tycho were performed to understand the nature of the high energy emission from SNRs and to observationally test the diffuse shock acceleration (DSA) theory. In this study, we present the highest significances of these shell-like and young SNRs in gamma-ray as a strong evidence for cosmic ray accelerators. Gamma-ray analysis of Cas A and Tycho were performed with Fermi-LAT at significances of 26σ and 8σ, respectively. Chandra analyses were performed on the thin filaments from the shells of Cas A and Tycho. Extracted X-ray spectra have been fitted with power-law model predicting the non-thermal emissions. Best fit chi-squared values for the regions from shells of Cas A and Tycho have been presented in the X-ray analyses which are supporting the DSA theory.

Differentiation of malignant and benign lesions of the osseous spinal axis with three dimensional computed tomography image appearences: dirty interface sign

Purpose: We aimed to make a fast and accurate distinction of malignant and benign lesions in cases with predominantly solitary or multifocal involvement using latest technology software and hardware systems in computed tomography. Materials and Methods: 53 cases were included in the study. Primary (n = 42, 31 benign, 11 malignant) or metastatic (n = 11) tumors were detected at various locations in the bone structure of the cervical to coccygeal vertebrae in all cases. 3D CT images taken using the same system and biopsy or post-operative histopathology findings were available for all cases. Thin section images taken retrospectively from the archives were converted to 3D images using the same program and parameters, which were then recorded in the same window settings by two radiologists. Only 3D images were then analyzed to investigate the presence or absence of the dirty interface sign. Results: Dirty interface sign was present in 17 malignant lesions and absent in the remaining 5 lesions. As for benign lesions, the sign was present in only two lesions and the remaining 29 were negative for the sign. There was a high level of consistency between the two radiologists. In conclusion, malignant and benign lesions affecting the bone spinal axis were distinguished based on the presence or absence of the dirty interface sign with 77.3% sensitivity, 93.5% specificity and 86.8% accuracy. Conclusion: When evaluated with standard bone window views, 3D views can be used successfully for the distinction of malignant and benign bone tumors. At least, 3D views generated using low dose regimes in highly developed systems can be used with similar purpose to that of diffusion weighted MRI sequences that give roughly outlined but fast and accurate information about the lesion.

Are Nutrition and Dietetics Students More Prone to Eating Disorders Related Attitudes and Comorbid Depression and Anxiety than Non-Dietetics Students?

It was aimed to investigate the eating attitudes and its relationships with anxiety and depression at Nutrition and Dietetics students. Students of Nutrition and Dietetics Departments were included in study group (n: 135) and non-medical students (n: 69) were in control group. Eating attitudes test (EAT-40), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI) were applied to all students and some anthropometric measurements were taken. Body mass index (BMI) and percent of body fat were higher at students in study group than students in control group. There were no significant differences between groups with respect to EAT, BDI and BAI scores. But, BMI and body fat percentage in girls were significantly lower than boys. Both EAT and BDI scores were significantly higher in girls than boys but BAI scores did not differ significantly. It was concluded that students training on Nutrition and Dietetics might have obsessions related to body image. But results of this study revealed that especially young women were more susceptible to eating disorders and depression.

Resveratrol attenuates oxidative stress and histological alterations induced by liver ischemia/reperfusion in rats

AIM:To investigate the effects of resveratrol on liver ischemia/reperfusion (I/R) injury in rats. METHODS: A total of 40 male Sprague-Dawley rats weighing 240-290 g were randomized into four groups of ten: (1) controls: data from unmanipulated animals; (2) sham group: rats subjected to the surgical procedure, except for liver I/R, and given saline; (3) I/R group: rats underwent liver ischemia for 45 min followed by reperfusion for 45 min; (4) I-R/Resveratrol group: rats pretreat- ed with resveratrol (10 μmol/L,iv). Liver tissues wweerre obtained to determine antioxidant enzyme levels and for biochemical and histological evaluation. RESULTS: Plasma aminotransferase activities were higher in the I/R group than in the I-R/Resveratrol group. Malondialdehyde levels and the hepatic injury score decreased, while superoxide dismutase, catalase ,and glutathione peroxidase levels increased in group 4 compared to group 3. In group 4, histopathological changes were significantly attenuated in resveratrol-treated livers. CONCLUSION: These results suggest that resveratrol has protective effects against hepatic I/R injury, and is a potential therapeutic drug for ischemia reperfusion-related liver injury.

Transplacental transmission of EDTA dependent pseudothrombocytopenia in a neonate

Verifying platelet counts can prevent unwarranted diagnostic tests and transfusions. In case of thrombocytopenia, if the clinical picture and history do not suggest bleeding tendency, one should always perform peripheral blood smear by directly obtaining blood by finger puncture before doing any further tests. If the peripheral blood smear exhibits platelet clumps, pseudothrombocytopenia should always be remembered. In this case, we present a neonate with a diagnosis of transplacental transmission of EDTA-dependant pseudothrombocytopenia.

Evaluation of the Lower Urinary Tract before Renal Transplantation: To Which Patients? How?

Purpose: The evaluation of lower urinary tract (LUT) before transplantation is not required, except in patients with a history of LUT disfunction (LUTD). In this study, we adressed the necessity for lower urinary system evaluation in transplantation candidates classified according to their etiology. Materials and Methods: Sixty-two patients were prospectively evaluated. The patients were evaluated at 2 months preoperatively and at 6 months post transplantation using questionnaires, bladder diary and urodynamic studies. The end stage renal disease (ESRD) etiologies of the patients were divided into three groups as follows: patients with a parenchymal disease, patients with LUTD and those with unknown etiology and the obtained results were evaluated accordingly. Results: The evaluation of 50 patients revealed significant differences in ICIQ-SF (p < 0.001), IPSS (p < 0.001), daily urine volumes (p = 0.05), maximum urinary flow rate (p = 0.03), PVR, the number of patients with VUR and all cystometric parameters between the patient groups divided according to the etiology. The comparison of the groups to each other revealed no statistically significant differences in all parameters between the LUTD group and the patient groups with unknown etiology. Considering that the results from questionnaires of patients with a single voided volume of 100 cc or greater on the bladder diary would be scientifically more significant, they were evaluated separately. Thus, even though the results from questionnaires did not differ between the group with unknown etiology and the LUTD group, there were significant differences when compared to patients who developed ESRD due to a parenchymal disease. Similar findings were found between the results of urodynamic study. Conclusions: The evaluation of lower urinary tract in patients with a history of LUTD and in those with unknown etiology should be initially evaluated by noninvasive methods such as questionnaires and uroflowmetry.
