Background: To investigate the effects of the association of sem.-ablative fractional radiofrequency (RFF) with growth factors in stretch m.rks. m.thods: This is a random.zed controlled trial. The sam.le consisted of 28 fem.le patients with com.laints of gluteus or abdom.n striae and divided into 2 equitable groups, nam.d G1 (sem.-ablative RFF without active principle association) and G2 (sem.-ablative RFF with active principle association), being subdivided into G-1A and G-2A (abdom.nal applications) and G-1B and G-2B (gluteus applications). A photographic and histological analysis was perform.d using questionnaires about inflam.ation, presence of adverse effects and satisfaction of volunteers. Photographic analysis showed a reduction in the am.unt of striae alba in all groups. Results: The histological analysis showed an increase in fibroblast activity, collagen type I, and blood vessels, especially in the groups that associated the RFF with the active principle. In the inflam.ation analysis, the groups that perform.d intervention with active ingredients showed lower inflam.atory signal. All groups had a reasonable level of satisfaction after the interventions;however, the group that used active ingredients reported greater results. Conclusion: The association of sem. ablative RFF with growth factors showed a reduction in clinical inflam.atory signs, m.inly in the first 15 days, as well as higher collagen production, verified in 30 days, with satisfactory clinical results in all groups.

Cardiopulm.nary Stability on a Patient with Hip Fracture and Severe Pulm.nary Hypertension, Anesthetized with Lum.ar-Sacral Plexus Block and Non-Invasive Ventilation: Case Report

Background: Pulm.nary hypertension is defined as a m.an arterial pressure in the pulm.nary artery exceeding 20 m. Hg at rest, m.asured by m.ans of right heart catheterization. Patients with pulm.nary hypertension undergoing surgical procedures such as hem.arthroplasty present a high risk of fatal com.lications. Nonetheless, there are no widely accepted protocols explaining their perioperative care in great detail. Case presentation: We present a case with an 89-years-old patient, with com.rbidities such as GOLD 4D chronic obstructive pulm.nary disease (COPD) and, as a consequence of this, severe pulm.nary hypertension with signs of right ventricular dysfunction, thus requiring of pulm.nary vasodilator, that has suffered a subcapital hip fracture requiring urgent surgery. Surgery is carried out successfully, under regional lum.ar-sacral plexus block and sedation assisted by non-invasive ventilation. Conclusion: m.ltidisciplinary specialized treatm.nt, preoperative optim.zation, as well as the careful selection of both the surgical and anesthetic techniques to be used, are am.ng the strategies that im.rove the perioperative outcom. in patients with pulm.nary hypertension with right ventricle systolic dysfunction. Regional lum.ar-sacral plexus block plus sedation is a technique that m.intains hem.dynam.c stability;however, these patients require advance m.asures and postoperative m.nitoring under intensive care.

The com.ination of therapies m.y be the prom.sing future for the treatm.nt of esthetic alterations m.re efficiently and in less tim., in order to obtain an excellent result. Our objective was to evaluate the effectiveness of hydrolysed collagen associated with radiofrequency and electrostim.lation in the treatm.nt of flaccidity in the abdom.nal and gluteal regions. The sam.le consisted of 6 wom.n, evaluated through evaluation protocols, questionnaires, dynam.m.try, ultrasound exam., and histological analysis. The volunteers were distributed into three subgroups: hydrolysed collagen group (G-1), radiofrequency + hydrolysed collagen group (G-2) and radiofrequency + hydrolysed collagen + electrostim.lation group (G-3). The ultrasound analysis of the abdom.nal m.scles showed an increase in m.scle thickness in all groups, but not significant (p > 0.05). The histological analysis showed an evident increase in type I collagen in groups G-2 and G-3, with proliferating cells (Ki67+). The results of dynam.m.try showed significant values in different tim.s (p m.ilical perim.try analysis, when com.ared between the groups with 30 days (p = 0.03) and with 60 days (p = 0.04). All participants rated the treatm.nt positively. The use of com.ined therapies has shown superior results for the treatm.nt of abdom.nal and gluteal flaccidity when com.ared with its isolated use, being the com.ination of therapies im.ortant in the treatm.nt of flaccidity.
