Quality of life after surgical and endoscopic management of severe acute pancreatitis: A systematic review

Colonic diverticulosis is one of the most common causes of lower gastrointestinal bleeding. Endoscopy is not only a useful diagnostic tool for localizing the bleeding site, but also a therapeutic modality for its management. To date, haemostatic methods have included adrenaline injection, mechanical clipping, thermal and electrical coagulation or combinations of them. The results of all published data are herein reviewed.

Pancreatic cystic lesions are increasingly recognised due to the widespread use of different imaging *** papillary mucinous neoplasms(IPMNs)of the pancreas represent a common,but also heterogeneous group of cystic tumors with a significant malignant *** neoplasms must be differentiated from other cystic tumors and properly classified into their different types,main-duct IPMNs vs branchduct *** types have a different malignant potential and therefore,different treatment strategies need to be *** ultrasound(EUS)offers the highest resolution of the pancreas and can aid in the differential diagnosis,classification and differentiation between benign and malignant *** addition of EUS fine-needle aspiration can supply further information by obtaining fluid for cytology,measurement of tumor markers and perhaps DNA *** techniques,such as the use of contrast and sophisticated equipment,like intraductal probes can provide information regarding malignant features and extent of these ***,EUS is a valuable tool in the diagnosis and appropriate management of these tumors.

Non-alcoholic fatty liver disease(NAFLD)and diabetes mellitus type 2 commonly coexist as a manifestation of metabolic *** presence of diabetes promotes the progression of simple fatty liver to non-alcoholic steatohepatitis(NASH)and cirrhosis,and the presence of NAFLD increases the risk of diabetic *** coexistence affects a large part of the population,imposing a great burden on health care systems *** from diet modification and exercise,recent advances in the pharmacotherapy of diabetes offer new prospects regarding liver steatosis and steatohepatitis improvement,enriching the existing algorithm and supporting a multifaceted approach to diabetic patients with fatty liver *** agents mainly include members of the families of glucagon-like peptide-1 analogues and the sodium-glucose co-transporter-2 *** addition,agents acting on more than one receptor simultaneously are presently under study,in an attempt to further enhance our available options.

Semaglutide for nonalcoholic steatohepatitis: closer to a solution?

In the issue of March 25,2021 of the New England Journal of Medicine,Newsome et *** the results of a phase II clinical trial of semaglutide,a glucagon-like peptide-1 receptor agonist(GLP-1RA),given once-daily,subcutaneously for 72 weeks for the treatment of non-alcoholic steatohepatitis(NASH)(1).The authors found that among 230 NASH patients with fibrosis stage of F2 or F3 the primary endpoint of the study(NASH resolution with no worsening of fibrosis)was achieved in 59%of patients with semaglutide at a dose of 0.4 mg compared to 17%in the placebo group(OR 6.87;95%CI:2.60-17.63;P<0.001).

Extending the decision-making process during yellow phase from human drivers to autonomous vehicles:A microsimulation study with safety considerations

One of the main factors affecting the safety of signalised intersections is the stop/go behaviour during the yellow *** previous research has exhaustively examined drivers’stop/go decision-making,the expected autonomous vehicles’(AVs’)stop/go behaviour has not yet been thoroughly *** a series of simulation experiments developed for conventional and autonomous vehicles using different carfollowing,lane-changing,lateral placement and stop/go model parameter values,we examine here whether the default VISSIM stop/go parameter values can adequately replicate the observed drivers’behaviour at the considered intersection and assess the suitability of using the currently available options,albeit referring to human drivers,to simulate the expected stop/go behaviour of *** also propose a policy framework for determining the desired behaviour of AVs in yellow interval,which is integrated into an AVs logic and achieved in the last simulation to explore the effect of automation on the stop/go outcome and,hence,on the safety level of signalised *** data analysis and modeling techniques were used for the formulation of certain scenarios,including binary choice *** default stop/go parameter values were found unfit to replicate the observed stop/go behaviour and subjected to *** to the currently available options,the proposed AVs logic proved to produce the most accurate results,in terms of the stop/go simulation *** the impact of automation on the stop/go outcome,the simulation experiments showed that AVs preferred a more conservative behaviour in favor of road safety,as indicated by the significant reduction(≈15%)in the number of vehicles crossing the stop line during the yellow light and zero instances of red light ***,compared to the conservative drivers represented by the default stop/go parameter values,AVs preferred a more rational behaviour in favor of intersection capacit

Thermal Energy Storage Contribution to the Economic Dispatch of Island Power Systems

In this paper the provision of flexible generation is investigated by extracting steam from Rankine-cycle power stations during off-peak demand in order to charge thermal tanks that contain suitable phase-change materials(PCMs);at a later time when this is required and/or is economically effective,these thermal energy storage(TES)tanks can act as the heat sources of secondary thermal power plants in order to generate power,for example as evaporators of,e.g.,organic Rankine cycle(ORC)plants that are suitable for power generation at reduced temperatures and smaller *** type of solution offers greater flexibility than TES-only technologies that store thermal energy and release it back to the base power station,since it allows both derating but also over-generation compared to the base power-station *** solution is applied in a case study of a 50-MW rated oil-fired power station unit at the autonomous system of *** optimal operation of the TES system is investigated,by solving a modified Unit Commitment-Economic Dispatch optimization problem,which includes the TES operating *** results indicate that for most of the scenarios the discounted payback period is lower than 12 years,while in few cases the payback period is 5 years.

Design of a Prototype EHD Air Pump for Electronic Chip Cooling Applications

This paper presents the design,optimization and fabrication of an EHD air pump intended for high-power electronic chip cooling *** high-voltage electrode configurations were selected and studied,in terms of the characteristics of the generated electric field,which play an important role in ionic wind *** this purpose,dedicated software is used to implement finite element *** design parameters,such as the electric field intensity,wind velocity,current flow and power consumption are *** different laboratory prototypes are fabricated and their performances experimentally *** procedure leads to the fabrication of a final prototype,which is then tested as a replacement of a typical fan for cooling a high power density electronic *** assist towards that end,an experimental thermal testing setup is designed and constructed to simulate the size of a personal computer’s CPU core of variable *** parametric study leads to the fabrication of experimental single-stage EHD pumps,the optimal design of which is capable of delivering an air flow of 51 CFM with an operating voltage of 10.5 ***,the theoretical and experimental results are evaluated and potential applications are proposed.

A combined risk-based and condition monitoring approach:developing a dynamic model for the case of marine engine lubrication

This paper focuses on the creation of a dynamic probabilistic model which simulates deterioration trends of a marine engine lubricationsystem. The approach is based on risk and the implementation is achieved through a dynamic Bayesian network (dBN).Risk can be useful for decision making, while dBNs are a powerful tool for risk modelling and prediction models. The model takesinto account deterioration of engine components, oil degradation and the off-line condition monitoring technique of oil analysis, inthe context of predictive maintenance. The paper aims to efficiently predict probability evolution for main engine lubrication failureand to decide upon the most beneficial schemes from a variety of lubrication oil analysis interval schemes by introducing monetarycosts and producing the risk model. Real data and respective analysis, along with expert elicitation, are utilized for achieving modelquantification, while themodel is materialized through a code in the Matlab environment. Results from the probabilistic model showa realistic simulation for the system and indicate the obvious, that with more frequent oil analyses and respective maintenance orrepairs, the probability of failure drops significantly. However, the results from the risk model highlight that the costs can redefinescheme suggestions, as they can correspond to low probabilities of failure but also to higher costs. A two-month interval scheme issuggested, in contrast to the most preferred practice among shipping companies of a three-month interval. The developed model isin general identified as a failure prediction tool focusing on marine engine lubrication failure.
