The role of emergency incident type in identifying first responders' health exposure risks

Hospital readmissions in decompensated cirrhotics: Factors pointing toward a prevention strategy

AIm To reduce readmissions and improve patient outcomes in cirrhotic patients through better understanding of readmission *** We performed a single-center retrospective study of patients admitted with decompensated cirrhosis from January 1, 2011 to December 31, 2013(n = 222). Primary outcomes were time to first readmission and 30-d readmission rate due to complications of cirrhosis. Clinical and demographic data were collected to help describe predictors of readmission, along with care coordination measures such as post-discharge status and outpatient follow-up. Univariate and multivariateanalyses were performed to describe variables associated with *** One hundred thirty-two patients(59.4%) were readmitted at least once during the study period. median time to first and second readmissions were 54 and 93 d, respectively. Thirty and 90-d readmission rates were 20.7 and 30.1 percent, respectively. Predictors of 30-d readmission included education level, hepatic encephalopathy at index, ALT more than upper normal limit and medicare coverage. There were no statistically significant differences in readmission rates when stratified by discharge disposition, outpatient follow-up provider or time to first outpatient *** Readmissions are challenging aspect of care for cirrhotic patients and risk continues beyond 30 d. more initiatives are needed to develop enhanced, longitudinal post-discharge systems.

Plan evaluation is of utmost importance as a function of good governance. It provides a means to improve the institutional basis for implementing land use controls, provides an important opportunity to improve future plans to reduce risk, and improves the vision for sustainable development and management. This article provides an overview of the methods and findings of a plan evaluation project undertaken in New Zealand. The project analyzed 99 operative plans, provided in-depth analysis of ten plans, and included a capability and capacity study of councils. This is the first time all operative plans in New Zealand have had their natural hazard provisions assessed in this manner. The information provides an important baseline for future policy improvements, and a basis for future research and policy directions. The project found that, while New Zealand land use plans appear to be improving over time, there are still opportunities for improvement. These include improving linkages between objectives, policies, and rules within land use plans; and strengthening the linkages between land use and emergency management plans. The largest challenge is the accessibility, understanding of, and updating of hazard information.

metabolic dysfunction-associated fatty liver disease(mAFLD)is the liver manifestation of metabolic syndrome,a rapidly growing clinical and public health issue mirrored by epidemic rates of obesity,type 2 diabetes mellitus,hypertension and cardiovascular disease(CVD).Besides mAFLD,nomenclature used to capture fatty liver disease not primarily due to alcohol excess include non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated steatotic liver disease(mASLD).Regardless of the terminology used,these diagnoses describe the most common cause of chronic liver disease ***,mAFLD is poised to become the leading cause of hepatocellular carcinoma(HCC)and end-stage liver disease requiring liver transplantation around the world(1,2).In addition to this,CVD and extra-hepatic cancers are the two main causes of death in the mAFLD ***,mAFLD poses a severe clinical burden which is projected to worsen in the future and intervention is urgently needed.

Identification of predictive mRI and functional biomarkers in a pediatric piglet traumatic brain injury model

Traumatic brain injury(TBI) at a young age can lead to the development of long-term functional impairments. Severity of injury is well demonstrated to have a strong influence on the extent of functional impairments;however, identification of specific magnetic resonance imaging(mRI) biomarkers that are most reflective of injury severity and functional prognosis remain elusive. Therefore, the objective of this study was to utilize advanced statistical approaches to identify clinically relevant mRI biomarkers and predict functional outcomes using mRI metrics in a translational large animal piglet TBI model. TBI was induced via controlled cortical impact and multiparametric mRI was performed at 24 hours and 12 weeks post-TBI using T1-weighted, T2-weighted, T2-weighted fluid attenuated inversion recovery, diffusion-weighted imaging, and diffusion tensor imaging. Changes in spatiotemporal gait parameters were also assessed using an automated gait mat at 24 hours and 12 weeks post-TBI. Principal component analysis was performed to determine the mRI metrics and spatiotemporal gait parameters that explain the largest sources of variation within the datasets. We found that linear combinations of lesion size and midline shift acquired using T2-weighted imaging explained most of the variability of the data at both 24 hours and 12 weeks post-TBI. In addition, linear combinations of velocity, cadence, and stride length were found to explain most of the gait data variability at 24 hours and 12 weeks post-TBI. Linear regression analysis was performed to determine if mRI metrics are predictive of changes in gait. We found that both lesion size and midline shift are significantly correlated with decreases in stride and step length. These results from this study provide an important first step at identifying relevant mRI and functional biomarkers that are predictive of functional outcomes in a clinically relevant piglet TBI model.
